    Hi, please make one on excisional debridement. My biggest hurdle is understanding the depth as it is not always clear. Does down to the level of the bone “include bone”?? I have had denials saying “down to” does not include that layer. Understanding the layers can be tricky so I would love your advice on ALL the layers please. Sometimes the OP report doesn’t specify depth at which point we would have to deny using A11 and A8. Is this correct? Sometimes it is not as clear or they mention depth other than in the OP report. Is this acceptable? Thank you so much!!!

      Hi! That is a great topic, I will add it to my list. Figuring out the deepest layer of a debridement can be tricky. "Down to bone" often means they stopped when they reached the bone but did not actually excise any bone. You would want it to say "down to and including bone" in order to code to bone.

    when it comes to removal of an intradural spinal tumor, is this excision of extirpation? For some reason I get hung up on the thought that they aren't removing the spinal cord, but rather solid material in, on or surrounding the cord. Am I overthinking here?

      Hi! The location of the tumor being of the spinal cord would not impact root operation selection. The method used to remove the tumor would be the determining factor. If the method to remove the spinal tumor meets the the definition of root operation excision then yes, it would be coded to excision, body part spinal cord. Think of it as you would any debulking of a tumor. The objective is to remove the tumor from the body part that it is attached to and therefore is coded to that body part. I hope that helps!

      I am also working as an IP coder in India,I am feeling somewhat difficult to remind the coding clinics,so is there any tricks to learn/remember coding clinics thoroughly?

      Hi,
      Ok, I see what you are asking. I would say that just like anything repetition is best way to remember. Read them whenever you get a chance. As you are coding if you are unsure of how to code a diagnosis or procedure, do a search of coding clinic for relevant topics, that way you can read them a bit throughout your day as you code. Do you know how to search them for the topic that you are trying to learn more about? Also, do you work in 3M encoder? There is a way to set your view options so that when you enter a code it shows if there is a relevant coding clinic on that subject so you can find them easily as you need them.
