There's actually an even more rare item. Tortoiseshell glasses. They are only on one person in the entire game, but in a patch 11 years ago, they got rid of them. The only way to get them is to go back to the original version of New Vegas and get then out of Vault 17, I believe it is
I was on board until you told me there's an Evil Gnome in the Lucky 38 Presidential Suite. The wiki agrees with you. How do I not remember this? Cass and Boone especially like to hang out in that room, so I've been in there a lot and I've never once seen it.
While the wiki mentions it, all the reddit mentions are of it being in the caves with the legendary gecko. I think either a mod adds it to the Lucky 38, or the video maker is pranking us.
Watching some videos of fallout nv on UA-cam, i noticed that a lot of cut content or patch or whataver, is pretty much all in the first version of the game. The evil gnome in lucky 38 is not there im my unpatched dvd of fallout nv.
Thats because it doesnt spawn there. The UA-camr has mods installed that restores cut content. It was meant to spawn there but the developers cut it before the release.
I did not know about the red plate... very cool. I'd also add an honorable mention. The unique weapon Cram Opener that is used by Little Buster from camp McCarran. After he leaves because the player collected the bounty on the raider leaders; he will be found a few days later outside the mormon fort deceased. If the player doesnt get to his gear in time, there is a high likelyhood that the gear will despawn.
You can also get it by blowing up the truck next to him while he's punching the training dummy, him getting killed by the explosion doesn't count as you having attacked him so no one turns hostile and you can go over to his body and loot it.
Might want to disable your mods if you are going to do these types of videos. A few scenarios and items you mentioned are because of mods. Otherwise good video. 1. The second evil gnome in the lucky 38 is cut content, it doesn't spawn there without a mod. 2. Vipers don't spawn in Nipton. That, again, is a mod

I've never seen the second evil gnome in my 1000 hours spent playing New Vegas so i went to fallout wiki and here's what I found: "When viewed in the G.E.C.K., the Lucky 38 presidential suite contains 2 suitcases in the master bedroom as well as a liquor cabinet and a gnome in the recreational room. They have been disabled and can be enabled with console commands." CamBam probably has some unofficial patch that enables the second evil gnome.
The miniature Sunset Sarsaparilla bottle looks like one of those little hot-tamale tequila bottles that you can buy in packs of eight from gas stations.

Me personally, I've always considered the Fist of the North Rawr in the running for one of the rarest items. Main reasons for this being that it, along with the Fist of Rawr, are the only (obtainable) deathclaw gauntlets that exist in the entire game. Both items require you to kill Rawr, one of the strongest Deathclaws in the game, to get its talon, then take that talon to a workbench, which requires 75 Unarmed to craft. The Fist of the North Rawr takes it a step further and requires Wild Wasteland, but is mostly just a name/ID change and doesn't have any stat differences.

There's another nuka cola quantum in old world blues. I didn't know how rare they were when I found one so I didn't log where i found it, but looking at the wiki it still says its only in lonesome road. I remember finding it inside of a wooden crate; not the large ones you search though. The ones on shelves or the floor you can pick up and move around. Ill update this if I can reload an old save and find it.

It's a leftover from Fallout 3. There are about 5 in various locations. After completing the quest Agatha's Song, you can give one to Agatha and she'll give you the Blackhawk, a unique .44 Magnum.
I have just found a larger than normal Damaged Garden Gnome behind the Whittaker Ranch right by Vault 19. I had another gnome and put him at Goodsprings, and when I fast traveled to drop the large one and put him next to it to compare, it returned to normal size. However, if I fast travel to Vault 19 and drop it, it's still large. Meaning you can't load a zone through a menu or it will go back to normal. That's why exiting the Sunset Sarsaparilla resets the bottle. If I walk my gnome back to Goodsprings or take short fast travels however, I should be able to keep him! Wonder what other objects are resized other than this gnome, bottle and plate..
i think the house mod he uses adds mannequins to put armor on, which he seemingly did. that's why there's also ranger armor and fnv textures. i'd chalk the better looks up to ENB and texture mods. possibly model edits too, don't know.

I got so annoyed with the game that I used no clip whenever just to get passed certain parts. It's been almost 2 years since I played and I'll probably never play it again

Also, a Glaswegian term for an alcoholic is a Jake or Jakey so Jake Juice would be considered a slang term for something that gets you drunk. Perhaps they had a Scottish person in the development team and this was an Easter egg of sorts.

I loved filling Voctors Shack to the brim with rare and unique items. Garden gnomes guarded the outside and Seymour defended the loot inside.
Theres alot of random 1 offs. Like the music sheet or ping pong balls.

It's certainly a very useful weapon regardless. Can't think of any other weapon that can kill Caesar with a single hit.

Tip for you sir. When grabbing items, the physics are effected by where you grabbed it. Try also crouching before grabbing sometimes

You can also get Quantum if you have the Nuka Chemist perk.

You don't need to kill the Lonesome Drifter. You can just steal the guitar when he isn't looking.
