  • * This topic is also given in First Aid 2015 page no. 538 and First Aid 2016 page no. 543.
    * Between 4:30 to 4:40 i said metabolic acidosis is due to hco3- , I meant metabolic acidosis is due to decrease in hco3-. at 5:01 I said co2 level is '22', it should be '28'. At 5:12 it should 'arbitrary. slip of tongue, sorry.
    * In my notes I wrote that metabolic acidosis start 3 hours after aspirin toxicity but change it to 12 hours, thats more correct and appropriate.

    * If you are first or second year medical student or if you have more than one year to prepare for USMLE Step 1 or any other exam then I strongly recommend you to watch Dr. Najeeb lectures to make your concepts very clear. If you don't have enough time to prepare for USMLE Step 1 then I would not recommend you to go through all the videos of Dr. Najeeb, just watch Neurology and Immunology.

    The first example you gave is mixed metabolic acidosis with respiratory alkalosis ( not compensated .. b/c Compensated rang if PaCo2= 1.5[HCO3]+8+/-2 = 30 to 34.

    At 1:11 when you said that when CO2 increases the H+ formed also increases so it causes acidosis. But at the same time the HCO3- IS ALSO INCREASING according to the reaction then why don't we consider that too.

    In metabolic alkalosis you said chloride resistant in excess mineralocorticoid is due to inc SERUM chloride levels. Did you mean URINE chloride levels? This confused me.

      Hello Zara. I will try to make but the compensation mechanism for step 1 are quite straightforward and mostly asked from fa and uw...for step 2 it will be slightly tougher.

    nice video! but tell me this, how can giving normal saline correct alkalosis?I can understand that it will improve chloride levels and volume, but how the alkalosis?

      apaar dadlani thank you. giving normal saline will inhibit release of aldosterone which plays key role in alkalosis and it will help in regenerating HCl if the cause of alkalosis is vomiting.

      ANP and BNP are released from the heart that inhibit renin-aldosterone axis along with increasing natriuresis and diuresis.

    In aspirin case.. Bro co2 will increase due to hyperventilation
