Thank you for the video, it is so helpful. I am wondering why the final character is Z for no qualifier instead of X for diagnostic. At first I struggled to decide what root operation would be used in this case, so I decided to look up the word "biopsy" in the alphabetical index. Biopsy in the index then states "see [drainage, excision, or extraction] with qualifier diagnostic." So by following the index, wouldn't the seventh qualifier then be X instead of Z? Or would this only be determined by reading the doctor's notes to understand if the intent of the biopsy is truly for the purpose of diagnosis (if not diagnosis, what else could it be? Is a biopsy ever for a purpose other than diagnosis?)

yes, with the new coding guidelines all biopsies have the qualifier X for diagnostic.

For the corneal biopsy, wouldn't you choose qualifier diagnostic? Biopsies are done for diagnostic purpose.
