Sherlock responds with "You are my first and only best friend, I not only enjoy, but require your companionship. If you left me I would be back on drugs, you keep me clean and happy. I love you as my friend or even family, and I hope you never leave me again." Without really thinking, and John's surprised because he never thought Sherlock would admit it more than the very few times he did, that he needed someone.
