Other people have said, he noticed that his leg slightly 'bent' in like a Goat's leg. Other people have said it's because Milton is purposely wearing shoes with 'lifts' to make him seem taller. What do YOU see that Kevin notices?
it’s definetley about the shoes, Milton states later in the movie that vanity is his favourite sin. Kevin is noticing for the first time that Milton is vain and would even wear lifts on his shoes to make himself seem taller, as he is already quite short.

This entire scene the Devil is getting to know his son and most importantly how sinful he is. Notice how he asks questions about his belief in God or how it was growing up without a dad or the relation he had with his mom. In other words if he feels bitter about that.
The Devil already knew the answers to those questions. He was making sure that Kevin knew that he, Milton, knew. It is also parallel to the Devils temptation of Christ, when the Devil taunted Jesus to throw himself off of a cliff and have his Father's angels rescue him.
Al Pacino's Performance as a Devil is Manipulative, Cunning and Devilishly Charismatic. De Niro's Performance as The Devil is Magnetic and Surreal. Peter Stormare's Performance as The Devil is Nightmarish, Insane with dark sense of humor (obviously) and Absolutely terrifying. These 3 Actors in my opinion gave the best performances as Lucifer and showed all 3 sides of that persona. I mean Viggo Mortensen in The Prophecy also gave a pretty good performance as Lucifer.
