    As a layman/patient/somebody simply taking one of these drugs, I found this explanation to be extremely useful and understandable and really spelled out the differences in mechanism of action between the successive classes of antidepressant drugs. I firmly believe that being able to explain something complicated to a non-expert audience is the hallmark of good teaching!

      Just wonderful, I have been researching "all natural depression treatments" for a while now, and I think this has helped. Have you heard people talk about - Hanincoln Nanlivia Framework - (do a google search ) ? Ive heard some pretty good things about it and my buddy got cool results with it.

      Mine is strongly genetic. So glad my dr knew this and didnt make me suffer through years of phsicotherapy. I was the first woman in my family line to receive help to live a normal life. Those before mt were tortured in mental hospitals. My mother was just a shell of herself after months of archaic treatments when I was eight years old. I'm so grateful for my treatment path. I got mentally I'll at age 27 just like all of my ancestors and was treated and able to have a wonderful marriage raise to boys and have grands and great grands. I did have to change drugs about every 7 to 10 yrs . That wasn't pleasant but better than the alternative.

      its all done on purpose, pharma is a billion dollar business and have corrupt governement officials on their payroll. being the most powerful country in the world we have so many smart people. they know that curing disease is a loosing business. they surpress natural medicine and dont approve treatmnets that they know will help people and they no longer have their business. make drugs that keep people from getting better and add side effects to make them worse and on more drugs.

      I used escitalopram for 6 month.
      It's been 5 month sience i stopped using this medicine, and now i can't concentrate and can't remember nothing. Today I cut my hair an hour ago, and for a few moments I thought I cut it yesterday. It was very strange.
      The psychiatrist told me that i should have accompanied escitalopram with brain vitamins.
      Tomorrow I will go buy some vitamins and hopefully I will regain my memory.

      here are several ideas you can try
      Find the reasons why you suffer from depression - the first step in solving an issue is understanding why you have it.
      Be at ease with yourself - this makes it easier
      Challenge yourself - you will feel better by doing things - especially by doing things that push your limits.
      (I read these and more tips on Martos Magic Method site )

      here's several suggestions you can try
      Work out why you are shy - the first step in solving a problem is understanding why it is there.
      Be comfortable with yourself - this makes it less difficult
      Just do it - you will feel better by taking action - especially by doing activities which stretch your boundaries.

    Lithium was actually forst used as a replacement for table salt. However unprescribed dosing of lithium has potentially hazardous consequences including Lithium Stroke as a result of toxicity when levels are high

    I'm on day 5 of lexapro ( escitalopram). I'm hoping I start to feel better in a few weeks. I've been depressed for a decade. I've had drowsiness, chills, nausea this week. And a severe panic attack on day 3 of this medication. I'm starting to feel better physically, but looking forward to feeling better mentally. I've been in low, foul mood for so long. 😞

      If you haven't already, I would recommend getting bloodwork done to make sure you don't have any thyroid issues or vitamin deficiencies that could be making the depression worse. I would also recommend asking your psychiatrist about genetic testing (like genesight) that can help you weed out the medications that may have worse side effects for you and can also tell you if you have a mutation in the mthfr gene (in which case you may need to take methylated b12 and L methylfolate)
      Also, don't take your medications with a drink containing citric acid. Only water or milk. The citric acid could make the meds less effective.

      Hang on in there because they normally take 2 to 6 weeks to feel better , you will feel your mood getting better, I've been on those for a while, I changed to mirtazipine, sertraline and now fluoxetine, anyway I hope you give them time to get into your system you will feel better

      I'm on day 5 of Reconter (which is escitalopram as well). I'm surprised that you had so many effects with Lexapro, I've been told it is more expensive because it brings milder side effects. Anyway, it's been 6 months... How are you? Do you still take it? Are you feeling better? :)

    This video is quite excellent in summarising such a complex field of mood disorders and the different treatments.
    I predict that atypical antidepressants will become the dominant “class” of medicine marketed. There are just so many different molecules that seem to have some positive effect on mood. What class people respond to is still a lottery; even if geneotyping becomes involved in predicting the probability of success to one drug over another.

      A form of one-on-one counseling that suits you has been great for me, just slow, slowly understanding things better and being far more able to control or accept or handle sudden issues without freaking out and feeling hopeless.
      But YRMV with all that stuff, it has to suit you (just like medications I guess, but more personal).

    My Psych Doc has been practicing Forensic Psychiatry/Addiction Medicine since 1978. He won’t prescribe Lithium anymore, he said 90% of patients end up with Kidney/Thyroid problems down the road. It’s inevitable. 😔

      P J it has the best evidence for mood stabilisation in the long term. Renal effects are eventually unavoidable, but that’s why it is monitored. Like many things, it’s a balance - without it you may experience a greater number of relapses and brittle illness, and thus worse quality of life

      @@ixiaorage its junk science.safe alternatives are being supressed and the goverenment refuses to fund real reliable studies towards much more safer and effective treatments. its digusting theymanaged to convince millions of citizens to except the use of these drugs that have no cure and have deadly side effects and cause horrible conditions way worse the mental disorder. i refuse to accept that in 2020 there only remedy are drugs barely studied given to us by doctors that hardly have an idea about these drugs they push. pharma is a mass genocide on humanity.

    Most studies show that the serotonin transporter is actually (already) reduced in patients with MDD (medication free). This would argue against the idea that SSRIs are really normalizing that system. As mentioned, other theories, may be more likely relevant.

    There is another explanation for slower onset of action by these drugs. This explanation is quite logical. It says that these drugs tend to bind more avidly to the presynaptic receptors initally inhibiting neurotransmitter release. Only when these receptors get desensitized they start showing there effect. Kindly correct me if I'm wrong

    MAOIs now diet-safe. No decarboxylating enzymes in foods anymore to create tyramine, meaning no tyramine to lead to high blood pressure. Food and nutrition journals have been doing these analyses for 20 years now.

    I have been on almost every antidepressant under the sun. SSRIs, SNRIs, and a couple of TCAs. None have worked so far besides my ADHD medication(Vyvanse) but that only lasted until my body adapted to the medication. My next class of drugs to try are MAOIs, and I'm excited to try them. I hope my psychiatrist will prescribe them but if not I'll find someone who will, because when you have lived with depression your whole life as I have, it's life or death to fix it and feel better. Also, I have tried the newer medication Auvelity and I'm on suboxone(for that's for past drug addiction it did help for depression at first but stopped).

      I would second-guess textbook explanations regarding MAOIs. I have reviewed professional pharmacology textbooks on this point and none has shown any signs of having read any advances on this subject in the last 30 years. If they had, they would have discovered that not only do MAOIs increase serotonin levels to a greater degree than the newer SSRIs, but also that MAOIs are highly unlikely to cause dangerous Serotonin Syndrome by themselves unless taken with a Serotonin Reuptake Inhibitor, or to a lesser degree a serotonin releaser like MDMA. There's also nowadays very little danger of dangerous hypertension with MAOIs, as foods have practically no tyramine anymore.

    I have tried anti depressants in the past and disagree that they work with everyone as the longer I took them the worse I became ! All I was being told by doctors is after so many weeks they will definitely start to work but eventually after months of taking antidepressants I stopped and eventually got better naturally

      I do believe for now that taking anti depressants doesn't cure you instead it make the problem even worse. Did you know some mother's who take these drugs prescribe by their doctor and commit a crime such as Killing their own kids. It's just another piece of evidence that these antidepressants drugs doesn't really work.

      People just react differently to the same meds. I took escitalopram for about 3 months without effect. While a friend got better almost immediately from it.

    I study computer science which I like it don't have an actual passion for. I've always found the brain intriguing and recently I've been watching videos like these instead of studying for my actual degree. Thinking about switching to psychology or neuroscience or something.

      @@sanjaisrao484 I can't easily switch bc I go to university in Germany and switching to psychology is impossible unless you got a near perfect GPA on your "Abitur" (German high school diploma). And because my GPA is pretty good but not good enough I'd have to go to uni in the Netherlands which I unfortunately can't afford, therefore I've decided to finish my bachelor's and then see if I can move to the Netherlands and study psychology while partly working as a developer or something.

    Hi excellent video i just didn't get the g protien part 5:49
    Does ssri cause delay in relief due to getting accumulated in lipid rafts or
    Due to slow release of g protien its delayed
    Im confused please help me out with this

      From what I gather, depression is associated with decreased cAMP in the brain. Activating G-protein Coupled Receptors is cAMP dependent. G proteins can stimulate synthesis of cAMP.
      So, by displacing G proteins from lipid rafts, antidepressants enhance GPCR activity.
      How that helps depression is beyond my pay grade.

      The key point in the video is that while the serotonin/norepinephrine effects of these drugs should begin within hours, the symptom relief takes weeks, so we need some other casual factor to explain that delay. If the kinetics of the g proteins match that timescale, they're a good candidate.
      Again, I don't know first hand how well the kinetics really match, and I don't know what alternative explanations there might be.

    Here's on for you. If 5-ht2a agonism is bad for depression, then why do psychedelic drugs (like psilocybin) appear to demonstrate positive effects in treatment resistant depression?

      they keep these treatments illegal and demonize them knowing they have no history of any bad effects and can actually improve lives. but keep these mild altering dangerous drugs on the market and easily available. obviously teh governement is corrupted with greed at the expense of our safety and well being

      Although psychedelic drugs are actually all partial (not full) agonists at that receptor AFAIK. Since partial agonists can also block and serve more to hold the level of receptor activation at a steady point, their effects may not be due to agonism but rather antagonism or stabilisation. Just an idea IDK.

      Or the positive effects against depression are due to the very intense and personal nature of the experience - like months of counseling in one day - and not at all due to the pharmacological effects but rather psychological - the things the user experiences and learns during the experience.

      @@DaFinkingOrk I've read recent articles too that you can tease apart the psychedellic and antidepressant effects of psilocybin. The antidepressant effect may have nothing to do with 5-ht2a agonism. Perhaps it activates Trk-b or TAAR or something.

    You forgot the most important MAOi inhibitor: Moclobemide, which is reversible, and doesn´t has the problems of old MAOi inhibitors.

      Moclobemide also doesn't increase serotonin NEARLY to the degree of the irreversible MAOIs like Tranylcypromine.

      Moclobemide is not the "most important MAO inhibitor", it is simply the weakest and most recent. Have a look at a journal article for once in your life.

      @Star Shaikh This sounds like OCD (obsessive compulsive disorder) . Please go to an actual Psychiatrist , instead of taking advice here . Its a very common psychological problem and there is proper treatment required . I have a person in family with OCD too . Hope you recover IA .
      P.S and please remember despite all the stigma , this can happen to anyone and is nothing to be ashamed of .

    I had horrendously bad depression. It was completely cured with antibiotics meant for a sinus infection. I knew that my gut was involved, and had to avoid certain foods which gave me depression. SSRIs didn't help in my case. With the antibiotics, it was completely over with.

    MAO actually reduce BP mildly- I think this is the same reason why this is so - this also explains the bisphasic response of adrenaline.
    We have less of B2 receptors and more of Alpha 1 receptors - But the B2 receptors having a stronger affinity for Adrenaline and noradrenaline - so when there is a mild increase in adrenaline/noradrenaline the effect of B2 is dominant leading to vasodilation and reduction in BP
    At higher doses(say when we take tyramine) when we have enough adrenaline/noradrenaline to bind to both Alpha1 and Beta 2 the effect of alpha1 is dominant simply because we have more of alpha1 receptors in the blood vessel.
    I remember having read this on wikipedia though I cannot find it now so maybe I am extrapolating a bit.

    It is a shame that selective reversible MAOIs like moclobemide (auroix) aren’t mentioned as an alternative to irreversible non selective MAOIs. Moclobemide is considered “weak”, but at the higher doses I got quite a boost from it, with very little side effects.

    The video states that activating “the bad” serotonin receptor, 5-HT2A, actually induces depression, yet research on Psilocibin (magic mushrooms) microdosing suggests the contrary. Any neuroscience/psychopharmacology grad students got any input on this? 🤔

      I'm not a neuroscientist but this confuses me too. 5hta2 agonists like shrooms somehow bring about cascading events leading to neurogenesis. But then 5hta2 antagonists such as seroquel are sometimes used as an augment treatment for major depression disorder. I think ultimately, we can't isolate receptors since I think there is cross talk between them. 5hta2 antagonists for example often being anti psychotics shutting down trips but psychosis being more associated with dopamine elevation.

    What I've learned from Elon Musk's biography is that very often, Science can't progress further because of the lack of technological advancement (the tools to make science) in that area. If we had better technological tools to understand the brain better AND to make better interventions I think we could get further with mental disorders.
