  • Soft glans syndrome is a rather unknown cause of erectile dysfunction. In this video on UroChannel, Dr. med. Dr. phil. Stefan Buntrock, a urologist from Göttingen, addresses this important condition. Genereally, erectile dysfunction (ED) is a prevalent issue affecting many men globally, defined as the inability to achieve or maintain an erection sufficient for satisfactory sexual performance. Among the various manifestations of ED, one particularly challenging form is soft glans syndrome, also known as cold glans syndrome or glans insufficiency syndrome. This condition is characterized by the penis becoming erect while the glans, or the tip of the penis, remains soft, leading to instability and complications during intercourse.
    Anatomically, the penis consists of three primary tubes of erectile tissue: the two cavernous bodies and the corpus spongiosum. The glans is part of the corpus spongiosum, which, although not crucial for overall penile rigidity, is essential for stability during an erection. While soft glans syndrome is relatively rare, it is crucial for healthcare professionals to inquire about it when assessing erectile dysfunction in patients.
    In a detailed interview with Prof. Goldstein at the 2024 annual congress of the International Society for Medical Shockwave Treatment, several aspects of this condition were explored. Prof. Goldstein, a leading expert in sexual medicine, discussed the difficulties in treating soft glans syndrome and highlighted potential causes such as fibrosis of the corpus spongiosum. He noted that this fibrosis could result from various factors, including certain medications, physical trauma, or activities like excessive bicycling and jelqing.
    Treatment options for soft glans syndrome are complex and varied. They include intracavernous and intraspongiosal injections of vasoactive agents to promote glans rigidity, as well as mechanical devices to enhance glans firmness. Prof. Goldstein emphasized the importance of addressing both the corpus spongiosum and the corpus cavernosum to manage this condition effectively. His insights underline the necessity for comprehensive evaluation and innovative treatment strategies in addressing erectile dysfunction, particularly soft glans syndrome.

    Thank you for posting this Doc. I have this and never knew it had a name or was a condition. When I use a C-Ring, it helps some, but I am going to make sure to bring the topic up at my next Urology visit.

    Hi this is what I have unfortunately, in terms of reversing fibrosis are the current options basically shockwave, and colloganase injections? Shockwave therapy tends to be done in a handful of treatments over a few months, would more persistent treatment for longer periods potentially yield better long term results? Given stem cells benefits for peyronies do you see it being useful for reversing fibrosis in conjunction with other therapies such as shockwave? can traction therapy be used in conjunction with these therapies to improve their effectiveness (by disrupting the fibrotic tissue)? Do you think you will make another video on the vertica in future? Any help would be much appreciated
