She was trying to reassure Claire, that Frank’s death will not be her fault, and it was because of Frank’s sudden death at an early age, that set Claire free to pursue in finding Jaime, which she finally did. She also knew that Claire’s healing education would expand, which did take her from being a nurse, to a surgeon, and her hair was turning white, when she returned to Jaime after them being 20 years apart.
This wise woman knew that Claire was something special, she may have even known the whole truth about Claire being a time traveller, and I love the way she looks at Claire, with such admiration.
She also knew that Brianna had arrived in their time, and was close by, when the two women were talking to each other by the river. Claire had no idea at the time, that Brianna had also time travelled, and was searching for her mother.
Such a great, and wonderful show!

What? No... From Claire's perspective this is after Frank is already dead, and she has already returned to Jamie. She was trying to reassure Claire that her own death a few months later would not be Claire's fault. I haven't seen this far into the TV series so maybe it isn't so clear in the series, but the meaning of this line is made very clear in the book.
