This is a massive lore drop, I was not expecting his military friend to be the colorful spawner person!! from what I saw, it seems like either Thomas and his friend had started growing apart as time went by, or things got really intense after they joined the military, before the incident?? Either way, this makes a lot more sense for why Thomas is seen hugging the colorful spawner in the "we forgive you" area in-game, it's possible he's finally started to forgive himself for his actions. (I could be wrong about things here, this is just my interpretation)
I view it as what thomas did to his friends face, since i see it like a censor of some sort to on his face to what thomas did to it. And maybe the colorful person from the dream is like a coping mechanism? To forget on what he did. Idk its just my sort of view and probably wrong

He also sees the world as a very happy place when in reality its all dark in the last part of the game. i dont know who the guy on the left is but im assuming this all means that thomas used to take meds and he didnt notice that the meds took over him resulting him killing his friend, flameguy jr, and flameguy’s wife? Because at the third part of the game there is thomas sitting at a grave with a picture of flameguy’s family. And his friend back then is like a spirit that sees that thomas didnt mean to do that to him and gives thomas meds in the cave. Or maybe its to show how the meds is ruining himself. EDIT: so his last video he made shows that he ate his friend because he didnt have any food. So back then he ate his friend and didnt take many meds. But then he met flameguy i guess and took meds from him to make himself happy and forget about it. this is not 100% but this is what im guessing what happened.
