Having owned a W163 ML320 for over 10 years, despite doing my own oil changes, it always required $1-2k per year in other maintenance/repairs. In the end, odd electrical issues and creeping rust made me get rid of mechanically sound ML. I feel spoiled now by my used CX-5 that hasn't needed anything other than regular maintenance. Another example of older German vehicles outside of warranty will empty your pockets.
$1-$2k is not that much. Considering the car is dirt cheap to buy.
I prefer to pay-as-you drive instead of all-up-front for a newish car.
Don’t put ALL Mercedes models in a category with ALL German cars. They’re old now and require maintenance and repairs like all aging cars do. The W163 had lots of issues early in production, but was a great vehicle in its final years.. 2004-5.
in all honesty an ML320 is much less reliable than an M113 ML55

I think it's just the MAF, because that measure the air and then the ecu knows how much fuel that has to be injected.
In idling it's bypassing the maf.
And the ecu can't know how much air that's going into the engine and has to rely on the maf.
So if that gives wrong information (low air flow) then it will inject to less fuel with result in misfiring and stalling.
I have a 99 ML320, it's absolutely bulletproof-- similar components (M112 vs M113 for ML55). You should look into the fuel pump (it's probably worn out from a clogged fuel filter) or look at the throttle cable -- that's the most common reason for loss of throttle on the ML's. You already addressed spark and air -- the only thing that fails as a result is likely the fuel pump (relatively easy to change out). Very rarely do any of the gaskets, intakes, throttle bodies, etc. actually fail. The most common failure points at high mileage is the fuel filter, fuel pump, CPS, and MAF.

You are more brave than me. If my friend was in the middle of working on my car I would NOT start it not knowing what he’s in the middle of 😂. My luck there’s zero oil in the damn thing!
It's 100% a fuel delivery issue. Idling ok, but under load it falls flat, then that is an issue with fuel. Fuel pump would be the first thing I'd replace. It could be an issue with timing as well, being that it's misfiring and stuttering.
That's kinda what I've been gathering from the comments so far. It sounds like the car may find a new home with a good buddy of mine that owns a Euro service shop down in IL. They'll get it sorted out and we'll finally find out what was wrong all this time.

Instead of throwing all those parts at it, did you ever scan it with an MB scan tool to actually find out what is wrong? Just wondering
I replaced $650 for the sensors because the code reader said that is the problem. It didn't work

Fuel pump control module related. I own a G55 of the similar engine(M113k) and this happened to me.
I replaced the fuel pump control module and that fixed it. It was an expensive job.
My truck had misfires, slow shifts, low throttle, and finally it won’t start at all.
The fuel pump module is cheaper to replace for the ML55. It’s inside the gas tank.
I have a 2003 AMG ml55 with the same problem. I have replaced over $700 and sensors. I've taken it to the mechanic he got it running it ran for two weeks and now it's back to the same thing. You could start it out it sounds great until you put it in gear. And apply some gas to it. Then it sputters and sputters and sputters. I have reached my Wit's End also.
It turned out to be the fuel pump! I sold the car to a MB Master tech, he fixed it, and now daily drives the car with no issues.
No... I already had the fuel pump and filter changed out ..no the radiator is leaking...aaaaaaaaa

It's just a dirty MAF sensor, they are cheap and take five minutes to replace. Or spray some MAF cleaner on it. Super common.

Yes that’s the problem.
Guess failure related to the brakes (-system) ABS. Might be the brakes switch sensor, lmm, accelerator, poti, or similar. Sensor issue or worst thing Ecu. No mechanical issue, definitely.

Yep, and he is not talking about diagnostic (proper one like STAR) and check the "live data", but directly started changing parts.
Sooooo many loose ends under the hood and you wonder why the thang does not want to obey you.......Hmmmmmm I feel the car is frustrated at the owner(s)!!!🤣 Fuel pump, MAF Sensor....It is probably related to ignition , fuelling or air ...
