- Heparin vs warfarin (Coumadin) nursing NCLEX review of anticoagulants. What is the difference between heparin and warfarin anticoagulants? What do nurses and nursing students need to know?
Heparin is part of the indirect antithrombin inhibitor family of anticoagulants. While Warfarin (Coumadin) is part of the vitamin K antagonist family.
Both medications are anticoagulants that help treat and prevent blood clots. However, these medications do NOT lysis or break-up an existing clot.
Heparin has a fast onset and short duration, but Warfarin has a slow onset and long duration. It is important to monitor the aPTT (activated partial thromboplastin time) for patients on Heparin. However, it is important to monitor the PT/INR for patients on Warfarin.
The antidote for Heparin is protamine sulfate, and the antidote for Warfarin is Vitamin K.

I have been on Warfarin for 22 years. It's not that you need to avoid green leafy vegetables, it's that you want to be consistent with your diet of green leafy vegetables once your Warfarin dosage has been dialed in by your doctor. If you want to increase (or decrease) your weekly intake of green leafy vegetables, you need to do it slowly in tandem with your INR checks because your doctor will need to adjust your Warfarin dosage to compensate for your change of vitamin K intake. Bottom line is communicate with your doctor if plan to make a lifestyle change and increase (or decrease) your normal weekly diet of green leafy vegetables, but don't avoid them.

REckon me a pro at this one. As far as I know Heparin is given during operations in order to lessen chance of clots. As for Warfarin. Took it for decades switched to Xeralto about 2 years ago. My INR sometimes for no reason would drop to 1 or lower, PT would go up about 5 and down to about .8. Diet yep no green leafy nothing, a 5 year old would love that. The monitoring sucks. Since taking the crap and no one has ever been able to tell me why, except for having DVT in both legs and PE to my right lung.
