I know this was done awhile ago but just some info. The holes you see in your leaves are a potassium deficiency. You mentioned adding Java fern recently and it is a potassium hog. Maybe related.
The problem I have is this. Seachem flurish is NOT a comprehensive fertiliser it's a trace element supplement so unless you were adding P, K, N on there own you were starving your plants. Now I have and use Easy Green and love the stuff. Easy Green does have P, K, N and is an all in one comprehensive fertiliser. If you have a small to medium tank it's all you need. If you have a large tank it can get expensive. On my 120 heavily planted tankI use it in combination with dry ferts and seachem flourish. The plants seem to like the combo. You should also try Easy Iron it goes along well with Easy Green.
