Reading all of these has put me a bit at ease as I have recently started the pump and on my third site change and am having some serious difficulties with the infusion set. As someone whose had complications with diabetes for awhile, I was in tears tonight feeling like an absolute failure and feeling like I couldn’t sleep until I had this right. It’s not the best for anyone’s mental health feeling like you’re failing. But I’ll keep trying, it’s early days and being transparent with my doctors as well.

My oldest son is in his early 20's and he has type 1 diabetes. But thank God for this piece of article ua-cam.com/users/postUgkxVYhghKWmrUgXARZ_ydZTvmmcrw5L0I5i At first he thought he had the flu and was lying down on the bed for three days until his sister took him to the hospital. They took his blood and it was 600. What I do not understand is how he could have gotten it, since no one in the family has it. But he is winning the battle now. This is good stuff.

I am glad I found this chat about the infusion sets. I'm highly sensitive to insulin variations and new to the Tandem system. Was a Medtronic user through three generations of pumps, but my endocrinologist highly recommended the t slime 2 as "far superior" to anything Medtronic makes. I agree the pump is far superior to anything I've seen from them. However, the set change has so many weakness! For example: having to build your own hypodermic (potential problems with plastic screws), not being able to look into the reservoir to see how successful you have been at draining the air from the reservoir, the injection port on the cartridge having minimal tolerance for error, the packaging of the applicator and tube which is not always easy to open, the spiral paper cover over the adhesive of the applicator (wants to lift), the wrap-around tubing which sometimes dislodges the applicator from its seat in the insertion mechanism, the "release ridges" which have to be squeezed while you drawn back the applicator, the two hollow areas where you press to release the applicator to insert the needle and finally, my personal favorite, the flimsy structure that holds the appllicator and the insertion mechanism. Yes, I wrote "my personal favorite," actually the insertion system seems to be the worst part of the system. It bends when the applicator is pulled back, making a 90-degree insertion difficult. In fact, I have had five attempts that either failed or may have failed. In the last three weeks I have tried five times to get the applicator to properly insert. Five times my blood glucose shot up hundreds of points after insertion, only to find that the needle had bent and did not penetrate through my skin. Disappointment is an understatement. I will try other Tandem infusion sets before deciding whether to go back to my former pump provider.

SK, I can't express how much I appreciate your detailed message. Having been a Medtronic pump user for the past 10 years (and still on it today), I was ready to order a new Tandem pump. I have all the paperwork completed and the distributor is ready to send me a Tandem Tslim X2 with IQ. They are waiting for me to return the final AOB paper with my signature. I have delayed signing it until I get my questions answered. I have been searching for answers to many questions I have regarding Tandems change sets. I have spoken to 5 different people, 3 of which were Tandem reps and 2 from my endo's office. I was left frustrated without information to my questions. You answered my biggest question in your last 7 sentences in your post above. I can tell, that you and I are both very analytical and detailed thinkers. Lucky for me, that here, I finally found the answers I've been searching for, over many, many months. It has kept me frozen on the fence undecided. It certainly has put the BRAKES on and I will NOT proceed with my order until I find out if there are complaints about the VariSoft Infusion Set (20°-45°). I really was hoping to avoid angle type change sets. I would really like to know if you have found a different Tandem infusion set that you are happy with and meets your expectations. If yes, it may be a good choice for me too. You are my last hope before I reorder a needed Rx for my Medtronic pump supplies which are getting low. I don't have much time left to make my final decision. I hope and pray you respond, especially while there is still a tiny bit of time left. God's blessing to you for having taken out a lot of your time to post your smart and very believable negative experience. It certainly opened the door for many others to post their negative experiences that followed your post. I appreciated all the comments that followed as well.

The pump is pretty good overall but the infusion sets are clunky and unreliable. I've gotten insulin leaking out of them more times than I can count and have accidentally pulled the set off the inserter several times ruining it. Also, the belt clip doesn't grip very well and has dropped off many times, often pulling the infusion set off with it.

In regards to the pump catching and falling and pulling the tube or canula out, you can minimize the disaster by taping up the final 4cm part of the tube near the infusion site with good strong medical tape used to adhere bandages etc, this should take the load off the icanula if the pimp falls to the ground. Works every time for me !

I have been using Tandem for 5 months with the Dexcom G6. The CGM element is far superior to Medtronic but the ease of use of the infusion set up is very difficult! I have arthritic hands and after 5 months of practice pulling back on the loading set up for insertion it is still immensely difficult for my hands as is disconnecting the infusion tubing from the cannula for bathing. It would also be very helpful to have a wider adhesive dressing around the infusion set to keep it more securely anchored but know that this would make the infusion insertion devise even more difficult to handle. I feel it is a design flaw to be so challenging to use the infusion set up and require additional tape to keep the infusion set secured to the skin.

Having now got used to my new pump I have found that the t:slim is a well designed and very easy pump to use. I really wish I could say the same of the AutoSoft 90 cannulas. As others have previously commented, they are very complicated and poorly designed. I do wonder if the designers actually have cause to use them themselves! I so wish I could still use my separately packaged cannula which was applied with a sturdy re-usable applicator, this would surely also be more cost effective and less wasteful in these days of reducing waste? Plus the added bonus of saving wastage and cost due to the numerous failed attempts attempting to apply the AutoSoft cannula.

Hi Andrew. Therefore, by filling the tubing, you are filling the cannula.

It can stress your muscles and give you pain during daytime. Best for me is a stretch band on the thigh. Pomp a bit inwards and it will not disturb you while sleeping. Succes. Best regards, Leo, The Netherlands.

I have worn a pump for 30+ years, and I remember having the same worry years ago. Its surprising how the brain subconsciously "trains" itself to handle these things. For the first 10 years of using the pump, there was no way to disconnect it while showering, so I had to hang it in a ziplock bag in the shower. To this day, I still automatically prevent myself from turning all the way around in the shower to prevent from winding myself up in the (nonexistent) tubing. While sleeping, my pump just lays in the bed next to me, and I generally just turn back and forth, without rotating all the way around, and this prevents being "tangled up". Occasionally, I feel a tug, and I just unwind it by tossing the pump around my body. It's not nearly as much of an issue as it sounds.

I have been using the X2 for about 4 months (after over 4 years with Medtronic, and over 36 years as a T1 diabetic), and in most ways it is a great improvement over the 670G. However, I have had several issues with the Autosoft 90 cannula bending back on insertion, which was never an issue with the Minimed system. Since the Tandem system does not recognize a problem, I get no warning until hours later, when my BG just keeps going up and up. Is there a way to prevent this, or comparable alternatives?
In addition, in all the years of using pumps I have never before heard that you have to fill the infusion set when reconnecting after a shower or something. It was not even addressed in ftf training with a diabetes specialist. Is this done under the "Load" menu?

Thanks for your question. Products such as Tegaderm, IV Prep, and Skin Tac can be used to help prevent the site from falling off the body, while also serving as a barrier between the set’s adhesive and your skin. We advise using scissors to cut a hole in the center of the Tegaderm/IV Prep adhesive pad. This hole should be large enough to cover the site adhesive, but not the site connector. Before applying the adhesive pad, insert the site into the body and disconnect the infusion set tubing from the site. Apply the Tegaderm/IV prep only over the adhesive portion of your site and ensure the site connector remains visible and uncovered. Once theTegaderm/IV prep is applied, reconnect your infusion set to the site. Skin Tac can also be applied directly to the skin (prior to inserting the site) to help strengthen the bond of the site adhesive. Prior to use, we suggest speaking with your physician regarding the adhesive chemical used in Skin Tac, Tegaderm and IV Prep to avoid a potential allergic reaction.

I've had the same issue of going high after changing my insulin cartridge. I reached out to the local Tandem Facebook group (where many recognised the issue), and they fixed it by giving an extra bolus. From that, I concluded it might be a 'flow' issue, so I started to not only prime the tube with the 10 units it takes to get a drop at the end, but give it an extra 5 units. That seems to have taken away most highs after a change. Somehow there aren't any air bubbles showing, despite the fact that there apparently seem to be some. Anyway - worth giving it a try ;)

I had super high, never like with my Medtronic infusion sets, and when I changed it out the cannula was actually bent in a 90 degree angle. May try different type but sure miss the Medtronic infusion sets, which also allowed you to re-use the same insertion device.

Interesting reading the comments about the sets and their issues. They and the reservoir fill let down an otherwise great system. It’s all just way too fiddly. Like others here, I love the T:Slim pump, but am rather disenfranchised with the three types of infusion sets I've used so far. I was ordered the AutoSoft 30 which is difficult for me to even open the packaging for. Once opened, the whole process for inserting the needle is cumbersome, requiring both hands to be engaged during the whole process. I was hoping to finally get back to using my low back and buttocks with switching to the tandem, but this is impossible with this set up. I'm also sad that the cannula is so very long. It is leaving me with hard lumps that last over 48 hours upon removal. And that's with changing my sets ever two days.
Next I was given 5 packages of the AutoSoft 90 by my sister. I'm glad the cannula is shorter, but going in at a 90° angle has caused 3 of the 5 cannula to bend and putting me into ketoacidosis

    • @aprilbartle3247
      @aprilbartle3247 Рік тому

      Oops! Continuing on...
The packaging/introduced arrangement set up is again a rather difficult thing for me to deal with. One set the tubing would notcome out of the housing. I have trouble getting the needle to stay cooked, as well.
Finally,I was given a TruSteel by my doctor's office. Nice simple hand insertion rather than plastic inserters.plus a short cannula, but it just plain and simply hurts! I can't do my exercises without the needle tearing through tissue every time I move. Surely Tandem can come up with a shorter cannula auto injector or hand inserted infusion set for those of us who are thin and don't have a lot of fat tissue and that can be easily inserted in any part of the recommended insertion sites.

I very much would like to start with the tslim. Seems to be a great pump. But if i look at this video i am back in doubt. Doesn’t look good.
Far to complicated for what is needed. I regurlaly read complaints about your infusion set and in the end a great pump with a “worthless” infusion set gives a lot of stress. In practice the importance of a good infusion set which is easy to place is not to be underestimated. I hope Tandem picks up thies messages and does something about it in the near future. Look at Roche... how simple it can be...
Also that strange thick connection 5 cm above the pomp. Ugly. Why not at the pomp itself and a bit more elegant. The moment you look at it you don’t want to go there...

We have recently gotten the t slim pump for my nephew, and it has been nothing but a headache for us. Everything works amazing aside from the infusion set. It is so flimsy and we have so many failed infusion sites. We already pay a hefty amount for his monthly kits for the t slim and we have had so many failed infusion sets, that we run short during the month, so yet again we are having to pay yet another hefty amount to replace these failed infusion sets. We have taken in person training courses, watched numerous videos to make sure we were doing everything right. We do it exactly how we were trained too, but these infusion sets keep failing. My nephew had to go to the ER twice because his glucose was over 500 and we had no other way to give him insulin. I absolutely hate that all infusion sets that Medtronic has available, are available for Tandem, except for the one that is far superior: The Mio Advance. Since switching to Tandem I miss this infusion set every time I have to replace mine. Please release a similar model, or offer an option with a luer-lock instead of the T-lock, so I can still use this pump, but don't have constant irritation, blockage, loss of sites or even a visit to the emergency room due to infection from Autosoft 90... Otherwise, I'm not sure my next pump will be a Tandem again (that's how annoyed I am with it, and I've got the scars to prove it).

My endocronologist suggested I look into the T-Slim/ Dexcom pump. I have been with Medtronic for over 12 years and with Dexcom sensors for 3 years. Reading the reviews of the T-Slim I find that problems with their infusion sets have now spanned 4 Years !
During my first weeks with Medtronic I had bent cannula problems with their MMT-398 infusion sets. (20% failed and I did get "No Delivery" alarms). My doctor suggested I try a Sure-T model MMT-840 and gave me samples. It has a steel 90 degree cannula so the problem I had with bent cannulas is not a factor. The steel cannula for me is also painless. The connector is on another patch separated by about 3" of tubing. This allows an easy disconnect of the pump without disturbing the cannula, and it also takes the shock if the pump is accidentally dropped. I have been very pleased with Medtronic since making that change.
I note also that reviewers had high BGs after newly installing their sets. I sometimes find the Dexcom sensor does that, and needs calibration. I see nothing said about calibration capability with the T-slim/Dexcom.
I also would not prefer a rechargeable battery over a quickly replaceable AA. And you need syringes to load the pump?
I have to conclude that the T-slim is far from being ready for me.

Then i decided to remove this set and found out that the (needle) was bent and delivered no insulin. Later on i checked my ketone acid and it was higher than what it used to be although its was not high for long time .
You have to develop a sensor which can sense if the insulin going out from the infusion set and of course develop that sh** plastic needle.
I have asked other diabeteces and many got that issue at least once in their life. Some of them were really tired of this set and started using infusion sets from another pump and they said Tandem is the first pump with this issue although they have used different pumps in many years.

I've always wondered why the cannulas can end up bent and I think I'm starting to realize why they might be. When I was with my tandem rep practicing the first time, when the rep pulled the blue cover protector off the cannula/needle, it stuck and she pulled the whole inset right off the inserter. I didn't think anything of it but when I did it, it did the same. So the rep just opened a new one. I've gone on to later experience having a bent cannula after checking and taking it out from having major high blood sugars.
I'm thinking that the needles get stuck some to the material the cannula is made of, not sure if from manufacturing or what... but they are sometimes really hard to pull off and if that happens, i'm sure pulling the needle out when inserted it probably is pulling the cannula backwards and bending it too! That's my thoughts on it anyways. :( I think the best bet is to report the lot to Tandem.

Hey there fellow diabetic - I've been having the same type of problem (I hope Tandem chimes in here). The problem isn't that the set comes off after installation. It's that when I insert the needle (and cannula), no matter how long I wait before removing the needle or how methodical/careful I am about pulling the needle out, about half the time I find out (after my sugar has gone sky high and realize there's something wrong) that the cannula came out (or partway out) when I removed the needle. The way I know this is that when I check after I've removed it from my skin, I find that the cannula is bent. Maybe I should opt for the shorter cannula size? Should I pull the needle out more quickly? Not sure, but If anyone has any ideas or tips, please share. Love everything else about this pump, especially the integration with the Dexcom G6.

Holy crap! How do you expect stupid people to use this??? I work in engineering, and my eyes are glazing over.

Exactly. It is much easier to make this fail with 100% certainty than to insert it correctly with 100% certainty.

On another matter, the medical device industry had better get with the program and start incorporating components in their products. I'm trying to imagine the thousands of tons of plastic waste you and Dexcom will be accountable if you guys take over a larger portion of the diabetes pump market. Take a leading role. I know it's hard and not a "business" priority. But think of the great press and great feeling if you showed your concern for the planet!

Unfortunately, we don't have any recycling program. The waste is not our preference, but to keep products clean and safe there are measures we must take. We can’t speak to any plans for future packaging plans, but we will pass on your message to our Supplies team. In the meantime, please let us know if you hear of any recycling programs for this type of medical waste that might be able to be shared to our customers.

Tandem Diabetes thanks for reply! But the burden is clearly on you guys to make your packaging recyclable not on your customers. I’m certain that you can have clean and recyclable plastic at the same time for example. Again, you may be responsible for tons and tons of plastic in our waterways and landfills. Just please pass this issue up your chain of command. You could do a world of good!

@@alansimpson7734 Agreed, this is one of my biggest issues with both Dexcom and Tandem. The first time I opened a Dexcom insertion device, unfortunately my first thought was "how wasteful"

Completely agree! Why does each infusion set need it's own insertion device? Medtronic's "Serter" works just great, and I have used the same device for the life of multiple pumps. Use recyclable plastic in the shells that hold the infusion sets, and keep the metal (introducer needle) out of the packaging. The infusion set tubing and reservoir should also be recyclable.

I feel the same way about the amount of wasted plastic. We don't need to fill the dumps with tons more plastic. I am disappointed with that feature.

I wish you could just TURN OFF THE ANNOYING AND USELESS "BACKGROUND" NOISE (masquerading as "music"), way too loud and overbearing, why have it ? Like we really need a happy disco atmosphere to dance during an important and complicated medical procedure made by the same manufacturers !
