Honestly, while it's kinda meme-y into a mage like Victor, I could see a build like this actually have some legs as a pick into assassins. It's much easier to hit the autos and thanks to having 2 heals and 2 shields, you're quite resistant to being blown up as quickly as other midlane autoattackers.
Your explanation of the build and playstyle makes this actually seem somewhat viable. People don't play ezreal mid much anymore (at least from my games) but with milios playstyle the comparison really works and w the range seems really oppressive into most melee matchups. I wonder how it would work with manamune, taking biscuts footwear and just spam roaming.

I just feel like his utility is too good to not be a roamer, so manamune would let him spam spells to shove wave (mainly w), and with biscuts he would have bonus mana for mune ad. Both are valid, i think it just comes down to playstyle pref (i play alot of assassins so im just used to roaming)
if youre going to play this again, i suggest you try manamune, helps with early laning, lategame with mana issues, gives around 90 AD which boosts even more the passive damage, and i believe it applies its on hit physical dmg to the passive extra dmg but im not sure, should test it, though its still extra damage on autoattacks.

I still think nashors is good (if you’re gonna go attack speed Milio) because the ap will help your passive buff and the buffs on your other abilities.
You’ll also want to take the shield bash rune and abuse his two part shield ability. This strategy also makes ardent censer a viable item even on AD Milio since buffing his shields makes his auto stronger and the ardent attack speed buff is great
toh the ideal way to play milio is support with a cait/an ashe to abuse his range buff, or a jhin who gets a ton of extra ad and super long range spells to proc your passive

Pekin have you tried bloodthirster with this build? I'm not sure if lifesteal healing counts for his passive but i think the shield from bloodthirster could be worth looking into
i think shield bash would be better rune, more early poke and resistances with conditioning or bone plating depending how dangerous is your matchup. so that every heal / shield has more value. ap ratios are quit bad but actually buying resistances is a good way to have ton of value out of the base values. and as you don't need attack speed going for more resistances 3rd and 4th items seems quite good. maw and dd good be very nice for a tankier setup as healing and shielding adds up while resistances are multiplicative to those. another good item on a less tanky build could be stormrazor for the extra dmg on 1 auto + slow, rfc doesn't seems good enough as it as no ad.
it's okay pekin, side characters are often people's favorites anyway. also wouldn't mind seeing him actually play support if you happen to get a good showcase game
Would be fun to see a oneshot Caitlyn or Senna build with milio support and rapid fire cannon to auto attack oneshot the enemy across the entire screen

Everything would work with manamune but trouble is he has good mana costs and would be missing out on next item for build.

Rapid Fire Cannon for last Item would be nice. Even more Damage on an Auto and that extra length on attacks in-between your abilities.

A year late, but milio can only stack tear with his Q, so manamune is rather inconsistent

I think Millio as support with first item Ardent Censor, and Nashor's would be fine, but you need someone to stick to for the Ardent to proc.

A funny thing about Milio's R is that it gives 65 tenacity so with Merc Treads and Legend Tenacity, you reach 100% tenacity.
Tenacity does have deminishing returns -> you get as much actual CC reduction if you have a lot of tenacity
