The Origin of the Painful Shoulder and Neck: The Right BC Patterned Scapula

  • Shoulder and neck pain often involves the muscles of the trunk, scapula, and neck. In particular, the serratus and low trapezius, the SCM and the latissimus. The imbalances contained among these muscle groups causes strain throughout the body. As I explained in the last video, the origin of the imbalances are found in the inherent asymmetry of the human body, but in particular the asymmetrical function of our left and right diaphragm.
    In the Right BC pattern, we know that the left diaphragm is no longer being used for respiration, and our center of mass is shifted to the right. As our center of mass continues to be shifted to the right, our entire right side begins to tighten. This tightening occurs because the Left AIC pattern prevents our pelvis from ever completely shifting to the left. When our body can't move to the left, the right side can't get a chance to ever fully decompress.
    The picture of the right shoulder being pulled forward is a common postural distortion that occurs because of this Right BC and Left AIC patterning. It indicates the inability to use both diaphragms for respiration, instability through the left hip musculature, the inability to inflate the right lung, an unstable right scapula, and overactive right neck and latissimus muscles.

    Almost 50 years I've had thoracic outlet syndrome, which is closely related to your subject. Understanding the muscular dynamics is incredibly helpful as I continue to overcome my postural misunderstandings. Thank you! At 65 I'm in much less pain and disability than in my 20's.

    Awesome breakdown and explanation. I’ve had this issue for almost 7 years and could never figure out why my body is so torqued. I literally sat on the couch and watched this and my shoulders looked the exact same as the picture, and I just followed along and could feel every muscle and bone on my right side react exactly as you stated.

    Thank you. I've worked with a great PRI therapist, and it's great to review this video. I now realize how important it is for me to understand this complicated situation. My right temporal bone is displaced. My PRI positions can "cure" the pain...as long as I keep doing them. So thank you millions of times.

    Excellent information! This is exactly my case. I don't have lots of neck or shoulder pain any longer but I do historically get asthma on my right side. I have believed for some time it was structural and I can see that it's probably due to this right sided compression and malalignment you describe. (My doctor doesn't believe me.) Your videos have made everything I thought more concrete and make complete sense. I can continue to work towards more freedom. Thank you. 🙏

    What's interesting is I have this because of all the surgeries I've had on my right neck right abs right lower leg. Also one missing muscle in my neck right side and part of my right 6 pack muscle made into a flap for a nerve replacement to help fix my right diagram with a partial plication of it as well. However, my pain is in my left shoulder, seems a nerve keeps getting traped there or in my neck. Not my right side at all, except sometimes muscle tension in my left or in the lat trap area but not directly in the right shoulder at all, I have times my body goes into a good normal position and I breath just fine no issues. Otherwise it's a struggle, but I'm way better than I was just 1 year ago let alone 3, right after just after my most recent major surgery, thanks to all the workouts I've done on my own, since no pt even knew what to do with me, learning about my body and what it needs on my own. Having been a horse trainer before all this has helped me too and getting back into riding recently again as I've gotten stronger. I think for me the exercises have helped push that air into my lungs and created more strength and flexibility, but I still have bad days and I wish I could get more constancy. I don't think all the scar tissue helps either pulling me here there and everywhere.

    This is actually extremely similar to my own shoulder. I've never seen nor heard it being broken down and explained like this. Great video with helpful illustrations.
    Is it possible to correct this? And if so, how would one go about doing that?

    Dear Neal. I live in Perth, Australia. I cannot find any practitioners in Oz at all. Would an online session work? Everything you are saying makes perfect sense. I cannot thank you enough. I've only started the PRI journey and although in a lot of pain (from moving things back to better I guess) I'm feeling just that tiny bit of hope I've been hanging onto for at least the last 3-4 years. I used to work as a photojournalist so a lot of tension stored on top of being hit by a car 3 years ago. I will keep studying the work and doing some exercises but as you say, until someone looks at one, it's impossible to know exactly what to do and when. I am appalled by how little physios and chiros and myos and whatnot-os know of this. When I mentioned the ribs to the physio he went 'that's nonsense'. Change is always difficult. Doctors I gave up on luckily. All they want to do is inject and cut and if not, medicate. Thank you.

I have a question; if this pattern was originally instigated by ganglion cyst in the TMJ on the right side, (following dental work) would you say that the cysts need to be addressed first before pri techniques can be effective?       If your scm gets tight it can refer pain into the back of the eye.

    Interesting video! What do we do in order to introduce stability in the left side to allow the lungs on the right side to make space for breathing and releasing the right traps/lats? And does this have to do with breathing into the left diaphragm and releasing the left hip flexor patterns from right lateralization?

    I have that right Shoulder forward pattern with SCM Scalenes (I also have Atlantooccipital problems), but for me the left flank and chest are already compressed. Also my right Shoulder joint (GH-Joint) is already in External Rotation, so the subscapularis is overstretched. How can I approach this pattern? thank you very much! Interesting Video as always.

    I have left shoulder winging, but definitely lean to the right. I’ve got a right labral hip tear which seemed to precede left shoulder impingement. Was a massage therapist leaning left to right a lot. Have throat & cheek/jaw issues now on left side. Is this my issue still?

    I have everything that was said in this video and in the previous one, including the pterygoid scapula and the tense subclavian muscle, but all this is on the left side, while there is an expansion of the chest on the left and compression on the right, could you make a video about such a situation, from-
    why can this happen from the point of view of PRI? on the right side everything fits logically, but what about the left?

      If you are right handed, try doing more things with your left hand: using the computer mouse, eating, brushing your teeth (promotes left wrist/hand pronation).....and try to stop using your right index and middle fingers. Tap your right thumb and right pinky finger together while your arms swings forward while walking (promotes supination of the right wrist/hand)

    Are there conventional exercises which may complement PRI exercises? For example:
    - one arm landmine presses (split stance one arm landmine presses?)
    - one arm kettelbell cleans (split stance one arm kettelbell cleans?)
    - clubbell one arm inside and outside circles
    - animal walks
    - lunges
    - etc

      It really depends on how you do them. Any exercise can be complimentary as long as it takes "patterning" into account.

      You need a Postural Restoration program. There is no one or two techniques that will work for everyone.

    i can understand it for a right bc pattern, but what if you are a patho pec? means both sides of the ribcage are already internally rotated and compressed. still prioritizing opening the right side, and then also the left back and forth, till we get the desired ribcage dynamics?

      If you are patho PEC, or just PEC, the ribs are in an externally oriented position (which corresponds with an extended spine). They aren't internally oriented. Patho PEC need more internal orientation of just about everything (femurs, pelvis, ribcage, arms) they are too externally oriented overall. It's confusing, I know, but in the Right BC pattern, the right ribs are in an internally rotated position, but the left are externally rotated (thus the common left rib flare). In this position the right arm is limited in IR, but the left arm would not be limited. If I were to push the left ribs down (like I did in today's video) and then have him breathe into the right side, the right ribs would ER and thus the right arm would gain internal rotation.

    Hello Neal, is it possible that the right lat compensates for the duty of the right serratus, I notice that when I get air into my right side, my upper trap flares up, I believe this happens bc my right lat goes into overdrive, (the stronger the pull of the right lat, the stronger the pull of the the upper trap ?) I noticed that it’s very hard for me to get my serratus to activate, but when it does, I have to have my lat in a lengthened position. What is the lat doing in compensation for the serratus. If the lat does not compensate for the serratus directly, what do you recommend I address?

      Yes, the right lat takes over for internal rotation of the arm, and the right serratus drops off. Perhaps search for a 90-90 with Right Arm Reach on social media.

      Anything that produces either extension of the shoulder or external rotation of the shoulder would help stabilize the scapula. But, to make it truly effective, it should be paired with right arch, left heel, and left ab sensory awareness.

    Left AIC, Right BC, Right TMCC - Right foot is supinated, and people in PEC are on their lateral sides of feet , which is supination, they are extended right? But on left foot in the normal pattern, people are pronated, but their back is extended on left rather then right ... in the pattern both sides are extended... just at different parts ?

    Let’s say that true left stance is obtainable, and the appropriate musculature pattern is engaged/ disengaged and right side is inflated. Is the protracted shoulder, still a neurological weakness or would that then be a musculature issue? I just want to know how to approach the volume of how often I need to work on the right trap

      You'd probably have to work on right shoulder ER while in left AF/IR and maintaining left ZOA.

    Doesnt the 90-90 hip shift with right arm reach contradict this? Since we are reaching w/ right arm, it is exacerbating the problem? Please advise

      No because of the inhalation into the right upper chest. Once air goes into the upper chest/right side, you now have a re-engaged right scapula. The picture is of someone with a disengaged right scapula bc of no air in the upper right chest.

      Like in case of a facial nerve neuropathy, there might be a motor weakness from buccinator to mandibular raphe to pharyngeal constrictor 1 to the posterior longitudonal ligament. Another route may be the digastric post to hyoid bone and omohyoid, not forgetting platysma. As to vision and oculomotors you have suboccipitals SCM and traps that impact back intrinsic muscles....not forgetting the vagus issues that come as the icing on the cake. All these are from my case and still fighting to put the pieces together...thanks.

    A lot of people with this problem actually have a gallbladder issue. So just check on that first before trying to fix yourself because it’s extremely common.

    Hey Neil, do you have straight clavicles meaning they are not downardly turned? Most people have downward turned because of traps and you seem to have big traps, so I am curious

    I have a rounded forward left shoulder, not right. All my pain is left sided . The bottom tip of my left scap feels like it is sticking out. My ribcage feels twisted. My scalenes on left are always tight, levator scap and upper traps continually contracted. My right lower shoulder, my scm is always tight w/a right head tilt. My ql on my left side now is giving me trouble and the whole shoulder girdle just feels out of place. Does this sound typical to you for this dysfunction? Thank you

      Also Neal, Like you mentioned can’t breath on the right side anteriorly. Everything on my right side is compressed including face, ear etc. I have a big right arch in foot, don’t feel the ground.
      I recently had a 3d reconstructed ct scan of my boney thorax. According to the doc that read it, he said my ribs(thoracic) on the left look more compressed/narrower which to me doesn’t make sense . Wouldn’t it be the right side ribcage more compressed? This is what is soo confusing. Which side then should I be focusing on for air. Also my 4th rib has prominent calcification which I never had before and now also levoscoliosis which I know is coming from all this misalignment of my ribcage. Does this sound right to you?

    In a left aic right bc pattern the right hip is hiking upwards there for the right shoulder drops and therefore the right UPPER trap is under active?
    What am I missing here?
    There are so many contradictions to this right upper and lower trap I don't even bother with this PRI stuff anymore

      What are the contradictions you encounter? In general, the right low trap is inhibited and the upper trap is overactive because of the position of the head relative to the neck, as an accessory muscle for breathing, and for scapular control.

      You need to see a PRI therapist in your area so they can find the solution for you. It’s a different origin for everyone

    Very helpful Neal,I trust you,but tell me how to get air in upper right chest..i have seen lot of your videos,still have no clue how to engage upper right trapezius and and right lats as showed picture in your video.Kindly reply with a suitable solution

      You don't want to engage the right upper trapezius nor the right lat. Every video on this channel talks about the solutions. There is a playlist devoted to techniques that, if done properly, will get air into your upper right chest.

    I bet he also has a hump on the right posterior side of rib cage, that's the reason he's shoulder is so off the ground

      @@adamchmelovsky9941 no, it's because you're in the pattern - left AIC right BC pattern, and generally you breath more into the left side

      @@adamchmelovsky9941 it comes from pelvis, so you need to reposition your pelvis also.
      Try some Feldenkrais exercises also for gaining mobility in pelvis and thoracic spine
