From the starting point, Nijooji Minami 2-chome intersection in Nara City, proceed through the city center of Nara to the north of Mt. Wakakusa. It changes course south near Yagyu, the same city, intersects with National Route 25 (Meihan National Highway) and National Route 165, passes through Soni Village, and enters Mitsue Village, Uda District, near the Nara Prefecture border.
After that, from the Shikitsu intersection (Mitsue Village, Uda District), which is the intersection with National Route 368, the route overlaps with National Route 368 until the midway Kaumi Akataki intersection (Matsusaka City), and then from the Kaumi Akataki intersection to the final point, The line overlaps with National Route 166 up to the Miyamachi intersection, and continues straight to the end of the line.
