      Funny that those are the spots that facial fillers look horrible anyway… They’re supposed to be tight/thin skin in that area … although some people would rather look like an alien, then look old

    I only use a needle to bone. I knew the Cannula would come out as unsafe. I’ll stay old school thank you. I get amazing results with a needle

      As far as I’m concerned, cannula tears through tissue, you can’t aspirate using a cannula and it’s quite unsafe

      Superficially is just under the dermis. Deep is on or just off the bone!

    Fillers or threads? Is this a guide for using fillers or threads? Do these principles apply to threads too? I have always been confused about how deep threads need to go. Into the dermis or subcutaneous fat layer?

    Thank you so much. This is very much needed. As us women across the globe are opting to do this instead of going to a doctor's office because of the high prices some of us can afford the price tag at this moment especially during this pandemic. Beauty is a cause of big business and I guess you can say we're willing to pay just about any price to have it so you doing this and teaching us a safer way really saves a lot of disappointments maybe and unreversible consequences.     Great videos.
    My question is about maxillary artery . When injecting lateral cheek bone with needle down on the bone 90 degree around the swift point . Are not we close to maxillary artery? How can you avoid that? What is your landmark to locate this artery? Also can you show us your technique for anterior cheek augmentation around mid pupillary line ( to be specific your technique to augment deep malar fat pad). Thanks a lot

      Great question.
      1. The maxillary artery runs under the cheek bone and comes out of the infra orbital hole or foramen, now called the infra orbital artery. It also connects to the temporal arteries, but you will be safe along the cheek bone.
      2. I usually use a cannula and eye this area. I’m simply filling in the space that’s hollowed out, but I’m always conscious of where I want my point of light reflex to be (apex point or Swift’s point). This prevents me from overfilling.

    Thank you for the video. But there is some conflict between what you say here on this video at 5:08 min and your danger zone filler video. Here you are saying anything medial to mid pupillary line to be deep to be safe but on another video ,the danger zone video( filler) at 6:48 min you are saying to be safe there are two way : one to be lateral to mid pupillary line or if you have to be medial to mid pupillary line Don’t be deep.
    So can you please clarify this. When we are treating medial deep fat pad or anything medial to mid pupillary line should we go deep or superficial? Thanks

    Is this just for fillers or does this apply to Botox too? A doctor is doing Botox for migraines on me next week and I am worried about it.

    Hey dan in one of your videos you said inject superficial in upper half of the nasolabial fold and then here in this video you said inject deep in the nasolabial fold ,iam too confused could you explain it more ,thanks.

      The nasolabial folds is a really tricky area to treat. I start deep, with a bolus to the piriform fossas. Then I’ll fan superficially with a cannula.

    Dr. what if one wants to fix a recessed midface area? say one had an underbite in the past which got fixed with orthodontics but now they want to fix their sunken midface look, can filler there be injected deeply right on the maxillary bone to mimic bone growth?

      Peah P Hi! Sorry for the delay. We don’t have that product in Canada, but it’s essentially a very thin non cross linked HA filler. There are reports of medications in suspension, which are essentially just liquid, causing vascular occlusion when injected in the wrong location. So yes, it still applies. And why an intimate knowledge of vascular anatomy is key with injectables.

      Barbara Grubbs This is only my opinion. I believe that, with exception to the nose, a 25g cannula or larger is safer than using a needle.

    Thank you Dan! So just to be clear, if one were to inject the apple of the cheek with a needle (the “apple” to me, is the center of the cheek at the point that meets from the middle of the tragus to the outer rim of the iris) , we would need to insure to go straight at a 90 degree angle, to the bone, and inject there. This would be a “safe” area to deposit the filler?

      As long as you inject on the bone (periosteum) and don't go medial of the mid-pupillary line (to avoid the infraorbital foramen), I would say it is pretty safe. However, I would also always use a 25-27 g needle, aspirate, and personally, I never deposit more than 0.1 ml at each site.

    I want to get cheek Fillers. I Am male and my zygomatic arch needs to be filled. I am in Germany. The Doctor i was going for an consultation told me he would prefer to inject Radiesse over an Hyaluronfiller. I cant find very much in the Web about it. Is it generally not so common around the world or does it have so many drawbacks that it's not a favorite Thing to use? Never had injections and just want to have my zygomatic arch balanced. Thanks

      Radiesse has it’s pros and cons. Pros = it builds calcium along the bone and dissolves after 1 year. Cons = if you don’t like it, you’re stuck with it. There’s also no dissolver for this solution. Because of this, it’s slightly riskier if there’s an arterial occlusion. But the cheekbones are a relatively safer area to inject.

      @@danestheticsmedical Thank you for your feedback. So after all hyaluronic acid like juvederm voluma is safer because it can be dissolved, right? I Think i am mostly afraid of general Side Effects like granuloma wich maybe occurs after a few Month. Is there a difference between hyaluronic acid and Radiesse when it comes to tolerability? My doc just told me there is almost no risk and that the Risk of granuloma or intolerability only occurs when the filler is not placed in the right layer or depth. Can u agree on that? Sorry to bother you

      dv Am Great question. Adding nitro paste and taking ASA are recommendations as best practices, based on theory. However, none of these are evidence based recommendations. I recommend keeping it in your crash cart.

    Why are the cheeks injected deeply when using a microcannula? Wouldn't less filler be needed if it was more superficial?

      Arielle great question! You’re correct, it may be done superficially, and is at times done this way. I prefer not to because our superficial fat moves with our facial expressions. The deep fat doesn’t. By injecting into the SOOF (deep fat) it helps prevent the product from migrating, and provides a more natural looking result.
