Thank you for the information! Can pearl ganglion occur on palm side of thumb above the first joint? And be quite tender?

Yes they certainly can. Just like in the fingers.

Thank you for sharing this video.
I have a Ganglion Cyst on my tendon sheath, (flexor) palm of hand below middle finger, I had an ultrasound guided cortisone injection almost 3 weeks ago, the consulting radiologist also punctured the Cyst whilst injecting the celestone, after the procedure the lump had totally disappeared and felt kind of mushy, 5 days later the lump started to harden though, and now it's back to its original size. My question is over time could this Cyst entirely disappear by itself or just persevere with another injection?

Hi Mark. This is a pearl ganglion. Unfortunately they can refill after aspiration. I would leave it at least 6 weeks just to see if settles down but if it persists then yes, I would get it injected/aspirated again. Next time try daily massage over the puncture site to help prevent any further swelling becoming organised into a cyst again.

@@PhysioMSK Thanks, I appreciate your knowledge, I'm currently using heat packs and also immersing my hand in a jug of Epsom salts and rock salt, the water is as hot as I can tolerate, I tried this same method on a ganglion that formed on my right toe last year, the doctor initially said it was gout because of the location and the time that it occurred (early hrs during the night) I was woken by immense pain. The cyst on my toe happened because of over use holding down a sustain pedal on a large grand piano for prolonged periods, the action was very tight unlike most pianos I've played on. It was aspirated last year with the cyst filling up a few days later, I ended up using a magnifying glass to tap it a few times after a hot shower it just burst, I'm hoping it doesn't return. From what I've found is that the cyst wall must eventually weakened over time (correct me if I'm wrong) and just pop - so to speak...
With regards to these pearl cysts do they have a firm wall that is the same or are these more difficult to treat?

The wall is no firmer. It’s just that the internal pressure is more substantial giving the feeling of a hard lump. They usually do very well with aspiration. You’ve been a bit unlucky as most go permanently after a single attempt.

I think I was unlucky, at first it felt like a wet sponge, I could actually feel the contents swishing around with no lump, but it hardened back again, I'll continue with the heat packs and get another referral to the radiography clinic for a second aspiration treatment in a month if I can tolerate the pain for that long. I still don't know what triggered it in the first place, I'm blaming the lifting of heavy buckets to fertilise the garden, but I suspect the piano has more to do with it

I am almost 54 and I've had 3 mucous/myxoid cysts in the last few years, and i have no idea why. I do crack my knuckles (for 40 years now) but I haven't cracked the first joint in at least 20 years. The first one i noticed was on my left thumb. It was the size of a small pea. It went down but it's still there. Never went away 100%. The second one I got was on my right middle finger. That too was larger than got smaller and if I rub my finger over it I can feel it. After checking the other 8 fingers, there is a small one on my right thumb as well but hasn't gotten bigger yet.
I wonder how I will look when I'm 80 lol.

Yep I’m the same age and I’ve got a couple of them too. Nothing to do with knuckle cracking. Just early arthritis of the end joints causing some roughening of the joint marjins which in turn wears a hole in the joint capsule so the joint fluid can escape. Just one of those things 🤷‍♂️

I have ganglion cysts on both ankles. They’re unsightly and soft but luckily not painful and atm I can still wear shoes.

That’s good. Always best left alone if they aren’t bothering you.
