Also, you can contact program coordinator of your department of interest to see if there is funding. The asistantship comes with waived tuition and stipend.

Also, choosing a state depends on your education criteria and the type of climate and neighborhood you prefer. They usually match the schools but you can choose to put your preference in the application and talk to your Program officer to check/apply if your preferred schools select you!

You can do PhD in your domain, for example public health (in broader sense). For this scholarship your research can not include clinical trials/experiments - but any other aspect!

Fulbright is a graduate degree program. MBBS is not counted in that. Have a look some options in Europe. That might be the best option!

There is an option in the application where you can mention universities of your choice.
Applicants can give 3 university names. However, its up to IIE where they send successful candidates.

You can try for other scholarships that are offered by the US universities. Mostly, they are not fully funded but a help.

On Fulbright, you get J1 visa. You cannot work full time on this Visa
