The Queen had no love for her second child, so she was okay with her baby being swapped by Aa Duo. Jinshi himself never knew that he was the current Emperor's son, as he thought himself that he's the Emperor's brother. As for the Emperor himself, it has been 'hinted' that he knew (maybe not immediately, but he eventually does after years) that Jinshi was his son. During a certain occassion involving a tower, he let Jinshi to challenge the tower, even pairing him with Maomao to increase his success. The Emperor then admitted he himself never succeeded, in which Jinshi did (with Maomao's help). The problem is, the tower was used to determine which brother should rule the empire, and after Jinshi succeeded, the Emperor no longer has right to the throne, because his younger brother managed to beat the challenge. But he wasn't worried at all, because he knew that Jinshi will only have the right to the throne after his father, himself, retired or died.
About the question of whether Ah Duo knows, that might be the reason for her words on the wall talking about the stupidity of it all -- basically she knows her head lady-in-waiting and servant girl both threw away their lives for nothing amd her son is actually fine. Also explains why Jinshi is the emperor's "best friend" but only in private...

Since Fengming was executed for a serious crime. all her extended family was under a cloud, and anyone working for the palace was to be released. [As a deterrent (and to punish bad upbringing), crimes often extended beyond the individual under both Confucian and Legalism ideals., and thus whole families could suffer from the actions of an individual.] So Maomao was on the list of those servants that was going to be let go. Gaoshun, being an intelligent and thoughtful intendant, brought this information directly to the attention of Jinshi (since the actual names of those serving girls would have normally been beneath the notice of someone at Jimshi's level). I wonder what is going to happen now?
One more interesting detail is the mother-daughter like relationship between Lady Ah-Dou and Lady Lishu. The emperor took young Lishu as consort is not because he is into really young girl. Lishu's family plan to sell their daughter into a political marriage with a very old pediphile after she was sent back home. It's Lady Ah-Dou who asked the emperor to take Lishu as royal consort since no one dare to lay a finger on the woman emperor wanted. Lady Ah-Dou, the emperor, and Lishu's mother are all childhood friends. They took the friend's daughter simply to protect her (with the most strange way). Unfortunately, there are more bullying and hardships waiting the young Lishu until she find her own courage to face them.
Ri Shu was the daughter Aa Duo never had. Also about the childhood friends thingy, it was actually the Emperor, Aa Duo, and Gao Shun (all three of them are milk siblings). It was probably a fate because after she got released as a royal consort, Ri Shu later is in relationship with Ba Sen (Gao Shun's youngest son, as well as Jinshi's childhood friend). Also one of the reason why Aa Duo asked the Emperor to take Ri Shu back was because she was unfavored by her own father, and also Ri Shu was the only one (except maybe the emperor) that was able to talk with Aa Duo with all her heart open.

Killing any of the Emperor's children would be a major F up, doesn't matter if it's the Empress or a concubine as all of them have the potential to become an heir if they are a boy as it doesn't always go to the oldest. The Emperor is appointed by the heavens in mythology so it's essentially killing a child of a God, an unforgivable sin punishable by only death. Had they found out that she was responsible then she wouldn't live for long. It's possible that Ah Duo knew what happened and never bought it up to protect her. We see other concubine's showing the same affection and mercy towards their maids when they screw up badly.
I'm betting Jinshi's Dad (current emperor) initiated the switch, and hiding the truth from the Empress. Now, he hid Ah-Duo outside the palace to protect her before the truth spills out.
Though from the scenes it seems that Ah Duo is the one that suggested the swap. She witnessed that the Doctor had to prioritize the Empress child labor, so she concluded that an Emperor's son would stand a better chance of surviving than the Emperor's grandson. And Ah Duo was proven correct, when "her" baby died under Feng Ming's care (no doctor to supervise the baby's food etc.)
