  • Dr. Rich shows a close up view of removing a silver amalgam filling with a high speed dental drill and replacing it with a white filling. In addition to the high speed handpiece, he uses a slow speed dental drill. Clean and Boost by Vista/Apex is used to clean the cavity prep. Dental bonding agent is Surpass by Vista/Apex. The flowable composite that is used is Titan by Vista/Apex, shade A2. Vitalessence by Ultradent, shade Pearl Frost is the final layer of the composite filling. Seamfree lubricant and wetting agent by Vista/Apex is used to aid in adapting the composite to the tooth. Valo Grand curing light by Ultradent is used to cure the bonding agent and the composites. Jiffy polisher by Ultradent is used as the final step.

      Should I remove my 2 silver fillings? I believe the last time I asked they said it wasn't necessary. But, I am concerned that it may be wise to. i don't really want to risk it weakening my tooth structure as I am trying to keep all my teeth. Now that I finally committed to flossing daily. Plus, I am concerned it may cause extra inflammation that isn't necessary as I already have inflammation from my eczema.

      Hi, I usually recommend replacing fillings only if they are failing (they're cracked or they have recurrent decay). I will also replace them if the patient doesn't like how they look.

      @@Sublimer79 A properly done (not too big, not too small) silver filling can last for a very long time. I have silver fillings in my mouth that are almost 40 years old (so far).

    Beautiful! Looks like a natural tooth. Amazing work. I just got three fillings today from my local dentist who i’ve known pretty much my whole life and it was a direct composite white filling. The drilling wasn’t all that fun, he had to numb me up again because I was feeling the pain at first but it worked out in the end and I think he did a good job. Ever since about a month ago i’ve been learning a lot about dentistry and oral health and it is interesting the type of technology that is out there, it’s amazing how quickly these resins harden when exposed to blue light

    i have a tooth with a silver filling. i think part of the filling must have failed because about 20% of my tooth with the silver filling chipped off exposing some of the filling (filling didnt come off, jsut a piece of tooth). it doesnt hurt for now, can it be repaired iwthout goign crown route>?

      It depends on how much tooth structure is left. Rule of thumb is if more than half the tooth is missing or is filling material then a crown is recommended.

    Is sensitivity a sign of a filling failing? I've had sensitivity on my old fillings for years but my dentist told me it's normal, though recently I've been reading they shouldn't have any sensitivity to heat/cold if they're in right?

      Remove them the mucury in the filling is released when you chew even brush, murcury is a neurotoxin and damages the brain

    White filling or silver fillings which is more durable? I have replaced silver filling as doctor adviced it has side effects...how many years may I expect its durability?

      Hi, it depends on the size of the filling, location in the mouth, how you maintain your teeth, and the skill of the dentist who placed it. I have silver fillings in my own teeth that are 35 years old. I have placed white fillings in my wife's teeth over 25 years ago that are still in good shape.

      Hi, the tooth is numbed for the procedure so you don't feel any pain. How long it takes per tooth depends on the size of the filling and how fast the dentist works.

    I had a huge cavity..
    But i got it filled.
    Its been a year now, i dont have any pain while eating on that side, but i am worried that do i need rct as it was a huge cavity that required rct..
    Can oral cancer occur due to not getting rct when it is required.
    I just got it cleaned and filled.
    Its not paining as of now and i dont have any puss around that tooth.

      Hi, next time you see your dentist, have them take a look at the tooth. They can take an x-ray of it and do a pulp test to determine the condition of the nerve. You won't get oral cancer from not getting root canal treatment.

    just had 3 fillings today how long does it take for the teeth to be okay ? and is it normal for the drilling to be painful ?

      Hi, it's normal for the tooth to be sensitive to hot and cold for a few days even a week or two after a filling. You shouldn't feel any throbbing pain. How does the bite feel on the fillings?

      Hi, it could be the bite is high on the filling, it could be a crack in the filling, it could be a crack in the tooth. See your dentist to have it checked out.

      I have at least 8 silver fillings
      The last couple of fillings I needed to have replaced, I had them replaced with silver fillings. Hush, the secret your dentist doesn't want you to know....

    I have a question, where do you put the extracted tooth so you can work on it so easily, as I would like to practice on extracted tooth, but I don't know how will I even make them stand and not move?

      Hi, I stick the extracted tooth in a bit of snap stone that I pour into the bottom of a plastic cup. Then I use a a small vise I bought from Harbor Freight Tools to hold it in place while I work on the tooth. The green you see in the video is a green rubber dam I stretch around the tooth to cover up the vice and stone so it looks better for the video. Thank you very much for watching!

      No. Having a cavity weakens a tooth and makes it more likely to crack.

      Removing a dental filling will inevitably remove a bit of tooth structure. If a filling needs to be removed, (i.e. for non-cosmetic reasons) then decay has already damaged the tooth structure.

    What about those liquid staff? Is it pushed away by the filling? It seems like it should not stay under or around the filling.

      The liquid is the bonding agent, the glue that holds the filling in place.

      Yes, the flowable is strong under compression, but it doesn't have the wear resistance of the paste composite.

    Great Video. I had a composite filling and I'm still in a lot of pain 5 days later. The dentist couldn't find anything wrong after returning and biting on a plastic wand. What do you think could be causing the pain?

      @@VeryNiceSmileDental There is sharp zapping pain when biting on harder foods. I have no issues when eating softer foods and the pain is only when biting.

      @@VeryNiceSmileDental No, the dentist checked for high spots and said it all seemed ok. The pain is only when biting harder foods. I'm wondering if there is a void in the composite filling? The dentist didn't use the damb around the tooth when performing the procedure.

      @@chriss2410 Hmm. It's possible it could be an issue with the bonding. Did your dentist use the plastic stick to just put biting pressure on the filling? Did the corners of the tooth (the cusps) feel ok when biting on the stick?

      Hi, it depends on the size of the filling and how it's taken care of at home. I personally still have silver fillings that were placed in the 1980's in my mouth.

      Hi, anytime a tooth is drilled there is a possibility that the nerve inside the tooth can be damaged irreversibly. I don't recommend that people replace amalgam fillings that are in good condition.

      Hi, the cost of a filling is usually based on the size of the filling. Cost will also vary from office to office. In my office, what is shown in the video would cost about $315.

      Hi, how long a composite filling lasts depends on several factors. It can last a long time, I routinely use it for fillings. I still place amalgams in certain situations (usually third molars or patching up gold or porcelain fused to metal crowns).

    Hello Doc I have a question how do I get rid of nerve irritation when the dentist drills my mouth the sensation of the heat makes me jolt and theres so much pain? Why do you think that is and could i ask the dentist to possibly give me a injection to the tooth so i wont feel the drill hence vibration? My teeth are very sensitive

      You will still vibration and pressure even with the local anesthetic, but you shouldn't feel any pain.

      You can try having the filling done without anesthetic. I have many patients that use anesthetic for fillings and crowns. I don't know what "angan state" means.

      Some dentists will sedate you for dental work. Without anesthesia you may feel pain. If you are nervous, ask your dentist if they have nitrous oxide. It works well for my nervous patients.

    Hi!Is it possible for a tooth to become sensitive to pain-not hot or cold,chewing pain-after replacement from amalgam to composite?Do you use any pulp protection if if you have to work near the pulp?

      Hi, anytime a tooth is worked on, you can have sensitivity afterwards. First thing I check is the bite to make sure the new filling is not too high. I don't normally use any pulp protection other than a good bonding agent and flowable composite under the composite. Pain when chewing may indicate a crack.     I had my dentist do this and I don’t think he took all my silver filling out before applying the white filling. Now when I chew it feels like I’m grinding on foil 🤦🏻‍♀️😒😭 and he said he can’t do nothing about it

    I just got a filling yesterday with composite m. When I got home and really looked at, my tooth doesn’t look natural. The dentist drilled more than he was supposed to. I miss ny old dentist, because she made my composite filling look natural. The dentist that did my fillings yesterday did me wrong…and the filling is big. I looked at my X-rays, there was a small cavity on the corner of my tooth and he got drill happy. I feel like I can’t trust dentist anymore.

      Sorry to hear about what happened. On x-rays, cavities look smaller than they are in reality in the tooth. Does the color of the filling not match?

      Hi, it depends on many factors, some are how well the person cleans their teeth, how big the filling is, where the filling is in the mouth, how it was placed, etc. My wife has a few white fillings that I placed when I was in dental school in the 90's that are still going strong.

      @@ClarkyWarky Usually it's covered, but, the insurance company may degrade the coverage to that of an amalgam filling. Insurance companies want to take your premiums, but don't want to pay your benefits.

      Yes it's fine to keep amalgam fillings. I only replace them if they need to be replaced. All of the fillings in my mouth are amalgam.

      Your dentist can tell. I will replace amalgam fillings if they have recurrent decay, or if they fracture, or if the person doesn't like how they look.

      Hi, it depends on the size of the filling and on the person. I have patients on whom I've placed white fillings back in the mid '90's and the fillings are still doing well.       Usually anesthetic is used unless the patient specifically asks to not use it, or if it's a really minor procedure that doesn't involve drilling into the tooth.

      The numbing gel is only to numb the gum a little before the injection. To numb the tooth, you need an injection. Some people don't have sensitive teeth. I have quite a few patients that never take anesthetic for having fillings drilled.

      Hi Ryan, thank you for the question, nowadays, my go to filling material is a composite resin (white filling) material. Most people don't have any sensitivity afterwards.
