- Dr. Rich shows a close up view of removing a silver amalgam filling with a high speed dental drill and replacing it with a white filling. In addition to the high speed handpiece, he uses a slow speed dental drill. Clean and Boost by Vista/Apex is used to clean the cavity prep. Dental bonding agent is Surpass by Vista/Apex. The flowable composite that is used is Titan by Vista/Apex, shade A2. Vitalessence by Ultradent, shade Pearl Frost is the final layer of the composite filling. Seamfree lubricant and wetting agent by Vista/Apex is used to aid in adapting the composite to the tooth. Valo Grand curing light by Ultradent is used to cure the bonding agent and the composites. Jiffy polisher by Ultradent is used as the final step.

Should I remove my 2 silver fillings? I believe the last time I asked they said it wasn't necessary. But, I am concerned that it may be wise to. i don't really want to risk it weakening my tooth structure as I am trying to keep all my teeth. Now that I finally committed to flossing daily. Plus, I am concerned it may cause extra inflammation that isn't necessary as I already have inflammation from my eczema.
Hi, I usually recommend replacing fillings only if they are failing (they're cracked or they have recurrent decay). I will also replace them if the patient doesn't like how they look.
@@VeryNiceSmileDental okay, thank you, I appreciate the response. Not sure how old they are. i think they are lasting a long time. Guess, whoever did it did a good job.
@@Sublimer79 A properly done (not too big, not too small) silver filling can last for a very long time. I have silver fillings in my mouth that are almost 40 years old (so far).
@@VeryNiceSmileDental My largest one is my last tooth (on the left side bottom) and the other is a small one about 6 teeth from center bottom.
Fascinating. I'm here strictly because I had mercury fillings replaced with the white fillings today and I was curious to see the procedure. Very talented. Thanks!

That's what I gotta get done today unless it's a root canal, not sure yet

Im really Glad white fillings are in options now for teeth fillings in dentists and you did a amazing filling that Cavity!
Thank you!
Beautiful! Looks like a natural tooth. Amazing work. I just got three fillings today from my local dentist who i’ve known pretty much my whole life and it was a direct composite white filling. The drilling wasn’t all that fun, he had to numb me up again because I was feeling the pain at first but it worked out in the end and I think he did a good job. Ever since about a month ago i’ve been learning a lot about dentistry and oral health and it is interesting the type of technology that is out there, it’s amazing how quickly these resins harden when exposed to blue light
Light cure resin fillings were a great improvement over the old self cure composites. Thanks for watching!
Wouldn't want to swallow any mercury!
When you were drilling the mercury fillings out were you concerned about the mercury you may have breathed in?
No, I had the high volume suction (the white thing you see in the video) positioned right next to the tooth.
I'm going to be a third-year dental student in the Summer, and this was a great video on replacing amalgams. Thank you!
Glad it was helpful! Thank you very much for watching!
Is it safe for health. Lungs and stomach due to the mercury? Be honest please.
@@alexstrings no silver is not
i have a tooth with a silver filling. i think part of the filling must have failed because about 20% of my tooth with the silver filling chipped off exposing some of the filling (filling didnt come off, jsut a piece of tooth). it doesnt hurt for now, can it be repaired iwthout goign crown route>?
It depends on how much tooth structure is left. Rule of thumb is if more than half the tooth is missing or is filling material then a crown is recommended.
I want all mine done and replaced with the white filling. I just think I'd really like looking at it more.
White fillings do look a lot better than silver fillings. Thank you very much for watching.
Is sensitivity a sign of a filling failing? I've had sensitivity on my old fillings for years but my dentist told me it's normal, though recently I've been reading they shouldn't have any sensitivity to heat/cold if they're in right?
Hi, sensitivity can be caused by different things.
wait silver fillings arent used anymore?? all of my fillings are silver what the flip
All my fillings are silver too. I placed 2 in a patient today.
@@VeryNiceSmileDental oooh i see. thank you!
Remove them the mucury in the filling is released when you chew even brush, murcury is a neurotoxin and damages the brain
Hello does this process require numbing on patient? Thank you
Usually yes, although I have patients who don't take anesthetic for dental work.
Novo-Hydroxyapatite DentalTown regenerative material. No toxic Biomaterials
White filling or silver fillings which is more durable? I have replaced silver filling as doctor adviced it has side effects...how many years may I expect its durability?
Hi, it depends on the size of the filling, location in the mouth, how you maintain your teeth, and the skill of the dentist who placed it. I have silver fillings in my own teeth that are 35 years old. I have placed white fillings in my wife's teeth over 25 years ago that are still in good shape.
how long does this procedure take and does it hurt? i have 5 i have to do on my molars
Hi, the tooth is numbed for the procedure so you don't feel any pain. How long it takes per tooth depends on the size of the filling and how fast the dentist works.
I had a huge cavity..
But i got it filled.
Its been a year now, i dont have any pain while eating on that side, but i am worried that do i need rct as it was a huge cavity that required rct..
Can oral cancer occur due to not getting rct when it is required.
I just got it cleaned and filled.
Its not paining as of now and i dont have any puss around that tooth.
Hi, next time you see your dentist, have them take a look at the tooth. They can take an x-ray of it and do a pulp test to determine the condition of the nerve. You won't get oral cancer from not getting root canal treatment.
@@VeryNiceSmileDental thanks doctor 😇
just had 3 fillings today how long does it take for the teeth to be okay ? and is it normal for the drilling to be painful ?
Hi, it's normal for the tooth to be sensitive to hot and cold for a few days even a week or two after a filling. You shouldn't feel any throbbing pain. How does the bite feel on the fillings?
@@VeryNiceSmileDental it's not painful but i prefer not biting with the fillings yet because i'm scared it will end up being painful
@@VeryNiceSmileDental hi one of my fillings is still a bit painful when i chew and is still sensitive to cold and hot it's almost a month what does this mean?
Hi, it could be the bite is high on the filling, it could be a crack in the filling, it could be a crack in the tooth. See your dentist to have it checked out.
I heard that silver fillings are dangerous. I have one should I be ocncerned?
I have at least 8 silver fillings
The last couple of fillings I needed to have replaced, I had them replaced with silver fillings. Hush, the secret your dentist doesn't want you to know....

amazing video. Thanks.
I have a question, where do you put the extracted tooth so you can work on it so easily, as I would like to practice on extracted tooth, but I don't know how will I even make them stand and not move?
Hi, I stick the extracted tooth in a bit of snap stone that I pour into the bottom of a plastic cup. Then I use a a small vise I bought from Harbor Freight Tools to hold it in place while I work on the tooth. The green you see in the video is a green rubber dam I stretch around the tooth to cover up the vice and stone so it looks better for the video. Thank you very much for watching!
I made a short showing my tooth holding setup. You can watch it here ua-cam.com/users/shortsMPVjjtKO9Ro?feature=share
Do you think old metal amalgam fillings can stress a molar tooth and cause it to crack?
No. Having a cavity weakens a tooth and makes it more likely to crack.
Interesting I had someone tell me that if I have a big silver filling... It has a possibility of cracking
I got dental fillings 2 weeks ago but idk if they’re silver or plastic. Can composite ones be grey in color?
Hi, if they are gray then most likely they are silver amalgam fillings. Thank you very much for watching!
@@VeryNiceSmileDental is it true that removing fillings damages tooth structure?
Removing a dental filling will inevitably remove a bit of tooth structure. If a filling needs to be removed, (i.e. for non-cosmetic reasons) then decay has already damaged the tooth structure.
anyone after white filling after months has body allergies in middle area for hands or legs
Hi, if you think you may be allergic to white filling material, see an allergist and get tested.

i always thot u drill until all the dark spots in the tooth are gone
What about those liquid staff? Is it pushed away by the filling? It seems like it should not stay under or around the filling.
The liquid is the bonding agent, the glue that holds the filling in place.
That'll be £1,500 please!
Lol ! Thank you very much for watching.
Does filling it up with mostly flowable really have the strength required for a posterior molar?
Yes, the flowable is strong under compression, but it doesn't have the wear resistance of the paste composite.
Do dentists in the US still use mercury fillings or is it pretty much all composite now days?
Amalgam fillings are still used but not as much. I'll still place maybe one every month or two.
How long does composite last in molars is it good or its better to use amalgam
Great Video. I had a composite filling and I'm still in a lot of pain 5 days later. The dentist couldn't find anything wrong after returning and biting on a plastic wand. What do you think could be causing the pain?
Thanks! What kind of pain are you having?
@@VeryNiceSmileDental There is sharp zapping pain when biting on harder foods. I have no issues when eating softer foods and the pain is only when biting.
Hmm, and you had no response when biting on the plastic wand? How does you bite feel? Are you touching on that tooth first?
@@VeryNiceSmileDental No, the dentist checked for high spots and said it all seemed ok. The pain is only when biting harder foods. I'm wondering if there is a void in the composite filling? The dentist didn't use the damb around the tooth when performing the procedure.
@@chriss2410 Hmm. It's possible it could be an issue with the bonding. Did your dentist use the plastic stick to just put biting pressure on the filling? Did the corners of the tooth (the cusps) feel ok when biting on the stick?
How long do silver fillings last? I have had mine for 10 years and was wondering if they can get old and crack.
Hi, it depends on the size of the filling and how it's taken care of at home. I personally still have silver fillings that were placed in the 1980's in my mouth.
@@VeryNiceSmileDental that's amazing. I have had someone tell me I should replace them with with the white ones but I don't feel like doing that. 🙂 Thank you for your input.
I usually recommend replacing them when they need to be replaced, or if you don't like how they look.
@@VeryNiceSmileDental thank you. The only worry I have is them cracking. You see... Recently one of my molars that had a white filling broke.
Is there a possibility of the tooth being damaged doing this procedure ?
Hi, anytime a tooth is drilled there is a possibility that the nerve inside the tooth can be damaged irreversibly. I don't recommend that people replace amalgam fillings that are in good condition.
I have done amalgam restoration and just after that I am feeling pain after eating. What is the cause? My doctor is saying that there is no problem in filling. But I am filling pain . Amalgam restoration removal and Composit restoration will help me or not?
Hi, what kind of pain are you having? Is the tooth sensitive to hot or cold? Does it hurt when you bite down on the tooth?
@@VeryNiceSmileDental I feel pain after eating.
How much would it cost to have this done especially on older one's
Hi, the cost of a filling is usually based on the size of the filling. Cost will also vary from office to office. In my office, what is shown in the video would cost about $315.
How long does composite last in molars is it good or its better to use amalgam
Hi, how long a composite filling lasts depends on several factors. It can last a long time, I routinely use it for fillings. I still place amalgams in certain situations (usually third molars or patching up gold or porcelain fused to metal crowns).
This was a very cool video thank u so much this motivates me to study❤
You're welcome! Are you in dental school?
is that technology available in India?
Yes I think so.
Hello Doc I have a question how do I get rid of nerve irritation when the dentist drills my mouth the sensation of the heat makes me jolt and theres so much pain? Why do you think that is and could i ask the dentist to possibly give me a injection to the tooth so i wont feel the drill hence vibration? My teeth are very sensitive
Hi, does the dentist give you a local anesthetic before they drill your tooth?
@@VeryNiceSmileDental yes thats correct they do
You will still vibration and pressure even with the local anesthetic, but you shouldn't feel any pain.
@@VeryNiceSmileDental okay thats understandable I just have a lot of dental trauma and the first go around the dentist never numbed me up and the heat of the drill made me tooth hurt so bad.. i have two dental extractions coming up on the upper left molar 12 and bottom left #19 they are broken and hopefully they will come out easily
Does removing sliver filling has risk of mercury poisoning ? Or sliver filling as it self ?
Hi, silver fillings do have mercury in them. But if they need to be replaced it is safe to do so with a dental dam and high volume suction. Thank you very much for watching.
Yes mercury fillings are very dangerous. Alot of sick people because of it.
I want to do filling but I am very afraid of injection so what should I do??Can I do it in angan state?
You can try having the filling done without anesthetic. I have many patients that use anesthetic for fillings and crowns. I don't know what "angan state" means.
@@VeryNiceSmileDental can I do in unconscious state??if I take without anastasiya can I feel pain??What can be done to reduce my nervousness?
Some dentists will sedate you for dental work. Without anesthesia you may feel pain. If you are nervous, ask your dentist if they have nitrous oxide. It works well for my nervous patients.
Hi!Is it possible for a tooth to become sensitive to pain-not hot or cold, chewing pain-after replacement from amalgam to composite?Do you use any pulp protection if if you have to work near the pulp?
Hi, anytime a tooth is worked on, you can have sensitivity afterwards. First thing I check is the bite to make sure the new filling is not too high. I don't normally use any pulp protection other than a good bonding agent and flowable composite under the composite. Pain when chewing may indicate a crack. I have a video that goes into more detail about pulpitis, ua-cam.com/video/RqwCYElA9BU/v-deo.html
I had my dentist do this and I don’t think he took all my silver filling out before applying the white filling. Now when I chew it feels like I’m grinding on foil 🤦🏻♀️😒😭 and he said he can’t do nothing about it
Hi, how much of the old filling is left?
I’m not sure.
I also still have silver fillings at the bottom molars… so idk if that could be the reason… but even when I push on the teeth it’s still sensitive
Is the tooth sensitive when you tap on it?
@@VeryNiceSmileDental yes. Does it take time to heal? I got it done on Monday.
I just got a filling yesterday with composite m. When I got home and really looked at, my tooth doesn’t look natural. The dentist drilled more than he was supposed to. I miss ny old dentist, because she made my composite filling look natural. The dentist that did my fillings yesterday did me wrong…and the filling is big. I looked at my X-rays, there was a small cavity on the corner of my tooth and he got drill happy. I feel like I can’t trust dentist anymore.
Sorry to hear about what happened. On x-rays, cavities look smaller than they are in reality in the tooth. Does the color of the filling not match?
How long will a composite filling last?
Hi, it depends on many factors, some are how well the person cleans their teeth, how big the filling is, where the filling is in the mouth, how it was placed, etc. My wife has a few white fillings that I placed when I was in dental school in the 90's that are still going strong.
Hey there mine lasted 20.
What is an average cost for this procedure? Thank you.
Hi, the cost depends on the size of the filling. In my office a filling the size of the one shown in the video costs $275. Thank you very much for watching.
@@VeryNiceSmileDental does dental pay for any of it? Or because it's considered cosmetic dental doesn't cover it?? 🤔🤔

How long does filling last
Hi, it depends on the size of the filling and on the person. I have patients on whom I've placed white fillings back in the mid '90's and the fillings are still doing well. I have silver fillings in my own mouth that were placed back in the early '80's and are still doing well. Thanks for the question and thank you very much for watching.

I have also gone with silver filling for my very small cavity in one of the molar teeth. Was it good decision as I wanted longevity?
@@amanrajuhd It should last you a long time.
Usually do dentists use anaesthetic injection or is that only in certain circumstances?
Usually anesthetic is used unless the patient specifically asks to not use it, or if it's a really minor procedure that doesn't involve drilling into the tooth.
@@VeryNiceSmileDental if the patient asks not to use it is there an alternative numbing gel? If not does the drilling usually cause a lot of pain without the numbing agent?
The numbing gel is only to numb the gum a little before the injection. To numb the tooth, you need an injection. Some people don't have sensitive teeth. I have quite a few patients that never take anesthetic for having fillings drilled.
@@VeryNiceSmileDental thank you for your response I’m having three fillings done over the next month so I was a bit nervous.

Great video no doubt, but what about the front bottom left side I feel like there's a lot of jagged area there. Just want to confirm that that something that would be addressed in someone's real tooth.
Hi, what part of the tooth are you referring to?
So expensive
Hi, fixing a tooth is much less expensive than replacing a tooth.
The mans got a jar of teeth
I think most dentists have one. During dental school I had to call around to local dentists and ask them for extracted teeth.
Is this more uncomfortable than a standard filing and if I have a new cavity that needs to be filled what's the best filing out of all of them?
Hi Ryan, thank you for the question, nowadays, my go to filling material is a composite resin (white filling) material. Most people don't have any sensitivity afterwards.
