I have been dealing with this for almost 2 years now. Several doctors, months of antibiotics and pain meds (useless) and now pevlic floor therapy for almost 6 months but still have it. Some days it's mild to almost no pain and the next day the pain is unbearable. Also been doing meditation, breathing and stretching exercises advised by my PT but not much help. Hoping it gets better soon as I don't think I can live much longer like this.

THIS IS IT! A few years ago I was on a weightlifting program that required me to squat 3x per week. one day while I was squatting I felt a small shooting pain to the tip of my penis. I couldn’t figure out why it happened, but over time it just kept returning. It got to the point where I had to stop squatting, deadlifting and even overhead pressing. This video is the ONLY one I saw that connected heavy squatting with penis pain! Thank you. Even weightlifting forums have limited info on this. I would love to see you guys but i’m in Chicago. Thanks for the helpful video. You mentioned two muscles that the dorsal branch of the pudendal nerve runs through and how their impairment or dysfunction might irritate the nerve. Which two muscles are these and how can they be relaxed?
The two muscles that surround the dorsal branch of the pudendal nerve is the bulbospongiosus and the ischiocavernosus. They can be relaxed 2 ways. One way is with manual therapy which would include techniques to stretch these muscles or to eliminate myofascial trigger points in the muscles, if they exist. You can also utilize dry needling to eliminate myofascial trigger points in these muscles. The other way is to practice relaxing your pelvic floor muscles. There are certain positions that can facilitate pelvic floor relaxation such as the yoga pose, happy baby and diaphragmatic/belly breathing. Ultimately, consulting with a pelvic floor physical therapist would be best as they would be able to utilize these manual techniques as well as teach you how to most effectively relax your pelvic floor.
@@PelvicHealth, can this cause the penile branch of the pudendal nerve in the penis become hardened like a cord? How can we tell if it's the nerve or the dorsal vein thrombosis?
@@wigglywrigglydoo Thank you for your question. No, even when irritated the penile branch of the pudendal nerve would not become hardened like a cord. This nerve is very very small and is not palpable externally. I'm not exactly sure what the hardened structure is that you are palpating. It may be best to consult with a urologist to determine that.
Thanks for the video. I have pain in the tip (over sensitive) and the last two inches of urethra. It’s been 4 months, antibiotics didn’t help. Dr started me on lyrica and pelvic floor therapy next month. Really hoping for improvement it’s tough to work and take care of my kids like this.
It sounds like your doctor has a good plan with the medication and physical therapy. Your symptoms are consistent with nerve irritation that can be alleviated with both these treatments. I'm sure this symptom makes it hard to work and take care of kids. You're on the right path. We wish you a speedy recovery.
Hey, your explanation is everything that I’m going through-I’ve been to a urologist and all that sort of stuff, structurally everything is good but I was just recommended a PT, was wondering if you’ve seen any improvement?
@@joeyschaefer3700 yes I’ve actually got some relief lately. The pelvic floor therapist was able to get me in early and I’ve had three appointments. They are working to relive a ton of tension that could be pinching or putting strain on nerves. Pain still goes up and down but the highs aren’t as bad. They are looking for triggers as well and we’ve notice sitting for a long time in a hard chair hurts and also when I bend over when lifting weights can make it hurt. I’m not sure that the Lyrica (nerve blocker) is doing much. I’m seeing a pain management doctor for that. Right after I made this post, I purchased a jock strap and cup to wear at work. It created space between my clothes and groin. That made a huge difference. In addition I got a stand up desk at work, donut to sit on and do a sitz bath twice a day. Still hurts but making progress and feel hope again. How long has yours been going on?
@@wademahone5807 I am 95% better. It took work but the pain is basically gone unless I have a hard day of exercises from PT. But even then the pain is very mild and only last a couple hours. Pt was great but I also had to do wim hoff breathing methods and eventually ice baths (once pain was reducing) to calm my nervous system. I needed the combination of a few things to stop the flare ups. If you are going through it, keep pushing… it does go away. You just have to keep trying different methods until you find what works for you. I’m happy to share on them if you have questions.
Hi! We will get back to you with some stretches for this specifically! In the mean time, you might check out the other exercise/stretch videos we have on our page!
I had penile trauma strain/stretch on Sep 7. Ever since then I have been having sensitive shaft skin on the left side of my penis, it hurts specifically when I rub the skin . I also have had other symptoms as well. The pain and symptoms have definitely improved or gotten better. The ER and my pcp said I was fine and that it was gonna eventually heal. I even went to see a urologist 3 weeks ago and he said everything looks normal. I asked for an ultra sound and was sort of prompting them for one, but the urologist and my pcp said they don’t think it will detect anything based on my tests done and symptoms. Tests being urine test, physical, exam, and ultra sound on testicles. Can I take their word for it? I’m only 19 and I’m very scared this is gonna be permanent.
Thank you so much for sharing your story. You are not alone in how you are feeling. We hear similar feelings with our patients. Since all the tests are showing normal findings, seeing a pelvic floor physical therapist would be a great next step. @@PelvicHealth I'm 15 years old and I'm still uncircumcised. I don't know how to clean it, and I just tried to clean it up yesterday. It was my first time. But, I can't pull the foreskin properly. Only a few foreskins can be pulled and the head doesn't fully show up even when I tried harder to pull it. And then there's a white or yellow thingy inside, and it sticks to my foreskin. it is a little built up. Is that the reason why I can't pull back the foreskin properly? What do you think is that? Could I get an infection if I tried every day to pull the foreskin? I really want to clean it with the head fully showing up, but I can't because it really hurts. But whenever I'm urinating, I don't feel any pain at all. The only thing I notice is a small balloon everytime i'm urinating. My circumcision is coming up next month, and I'm not sure what to do or how to tell my doctor. Help me. What is it?
I have penile pain on the right side connected to the rectum after a bowel movement. My pelvic therapist says that the driver is the S1 S2 and L5 L4. My PT just recommends breathing. I have been having this pain for 5 years. I saw many therapists, chiropractors, and bone healers in Latin America. The pain started with a 10 in the first 2 years. Today I have mild pain. Some days I don't have any pain at all. My question to you is how can I fix this problem in my sacrum? My Pt says to not twist to my right side because it is pulling the pudendal nerve from the sacrum.
The pudendal nerve, which innervates the penis and other pelvic structures, commonly gets aggravated with bowel movements because it puts stretch on the nerve. Avoiding positions that put extra tension on the nerve can provide temporary relief. Finding the root cause(s) is necessary to find lasting relief. It is a good sign that you have seen significant improvement with the treatments you have done so far. Possible causes for nerve irritation include excessive hypertonicity of the pelvic muscles, poor movement of the sacroiliac joints, long term sensitization of nerves, and other causes. Each cause has a different treatment approach. Make a virtual appointment to solve the puzzle of your pelvic pain cause.
We think it's possible that scarring from a perineal fistula could impact the perineal or dorsal branch of the pudendal nerve. This is a question that is very specific to the person and their particular situation since fistulas vary quite a bit in severity and impact on surrounding structures. This question is likely best directed at the provider who is managing the fistula.

@@zeeshanasgher8697 It's difficult to confidently say exactly what treatment will be most effective for you. To do that would require a comprehensive, in-person evaluation. However, treatment would likely be a combination of pelvic floor relaxation (independently and with the help of a pelvic floor therapist), minimizing further irritation to the penile branch of the pudendal nerve (no excessive masturbation or aggressive elongation of the penis), possibly medications to decrease nerve pain, and time.

But my urologist told me that tight pelvic floor muscles would make my problem worse. Yes. Constipation and straining are associated with hemorrhoids and this can create pressure or compression on the pudendal nerve. Many patients with constipation also have an irritation of the rectal branch of the pudendal nerve. We recommend an evaluation with a pelvic floor physical therapist who can help you address these symptoms.

Would long-term scrotum area pain (a few months) ONLY when sitting have any relation to these nerves/pelvic floor? I believe the pain is related to body position as I feel no pain after waking up but it gets sore quickly after sitting and gradually worse during the day whilst sitting at my desk. However, standing up brings instant relief. It's a dull ache seemingly moreso around the scrotum than in it, and also towards the leg/pelvis joining area (both side). Thanks.
Dull and aching pain typically means it is stemming from the muscles. Prolonged sitting leads to compression of the pelvic floor muscles and reduced blood flow to the area. Consider stretch breaks every hour or so, sitting on a cushion and practicing pelvic floor relaxation using diaphragmatic breathing whilst laying on your back with the knees hugged in. You may benefit from stretching your inner thighs, also known as your adductors. You may also want to consider stretching your hip flexors which get tight with prolonged sitting and contain a nerve that innervates the scrotum and inner thighs. If none of this brings relief, you may want to consider pelvic floor PT!
An injury to an abdominal muscle could contribute to urinary symptoms. If you developed an inguinal hernia, that could contribute to penile pain and/or pelvic pain. If the symptoms persist I would recommend a consultation with your primary care provider and/or a pelvic floor physical therapist.
Great question! A MRI may be able to show if there is some inflammation or swelling, but that would still be difficult to tell if the nerve is the issue when other structures can be in play as well. Seeing a pelvic floor physical therapist would be helpful to work in adjunct with your doctor and perform a thorough subjective and physical evaluation to see if a nerve may be a contributing factor to your symptoms. Schedule a virtual appointment with us pelvicpainrehab.com/telehealth/. Hope this was helpful!
For more than 2 months, I've had a small spot of skin around my foreskin, which gives off a stabbing pain when I stroke over it (pressing on it doesn't cause any pain). However there is nothing visible around that area, no rash or anything like that. Could this also be related to what you're describing in the video? I've been to an urologist, but he also didn't detect or see anything. I'm grateful for any help, because I feel this is slowly driving me into a depression since I can't wear clothes anymore without feeling pain when moving around, due to the fabric stroking over the affected area. Thank you!
We're sorry to hear that you've been dealing with this! Nerve pain is often stabbing in nature, so it is possible that this pain could be coming from a nerve. However, pudendal neuralgia is not typically isolated to one spot of the nerve's distribution, thought it may be isolated to the left or right side, based on which branch is affected. There are many nerves that provide sensation to our genital region, and it sounds like it would be beneficial to make an appointment with a pelvic health specialist to further discuss. You can schedule a virtual appointment here: pelvicpainrehab.com/telehealth/
Hi, not sure if I will get a response but watching your video gave me the urge to ask since I am going through the exact same issue for more than 4 months without a single trace of the underlying cause... About 4 months ago, I had a sudden onset while walking underneath my penis area (towards the tip), it felt like a needle spearing through the area which lasted for a good 5 seconds. Pain was very severe and ever since then, I had trouble peeing, walking or just living my daily life normally in general.. ALSO, I also noticed slight discomfort while sitting down on my perineum area where i had to adjust my positioning to find comfort (I didn't take this as serious as penis pain was more prominent at that time). I have since then, done around 20 Urine tests, countless Blood tests, Ultra sound on bladder, Urine flow test, even a Cystoscopy which showed clean view of my urethra and followed by MRI scan on my Prostate and Lumbar Spine. All of these tests showed good results (Only a little protrusion on L4/5 disc). And just recently I have not been able to sit down at all as the pressure on my perineum area has gotten much worse that when I sit down, the pressure then pushes forward and gravitates to my penis, giving a sense of my penis is being protrude and changing its shape slightly, the main problem why i cannot sit down is that the pressure triggers the exact same pain at the bottom tip of my penis where I originally had an onset on. Do I need to go and see Pelvic Floor specialists??

Treatment can help lower the frequency & level of pain- it will depend a bit on your body's reaction to treatment to determine how quickly it will respond & lower symptoms. Have you seen a pelvic floor physical therapist for your symptoms?
@@jzhdhhdhhshcbfc8826 Since we cannot physically exam you, we cannot say 100% that it will go back to your normal level before. That being said, if you are already seeing relief, you should continue to see relief as long as your nerve does not get irritated.
Hello, we do not prescribe medication as pelvic floor physical therapists. If you are experiencing pain or are in an emergency, we recommend seeing your doctor or a urologist to address this.
Thank you for your question. Your description of symptoms sounds like it's more gastrointestinal distress versus a pelvic floor issue. We would recommend consulting with a gastroenterologist.
I've had painful and burning urination for three years. The pain only happens at the beginning of urination/ejaculation and a little bit after urination at the top of the penis. No pain otherwise (e.g. sitting, standing or exercising). No urgency, frequency or other symptoms. All urine and blood tests show negative results. I also did two bladder scopes, two ultrasounds, one CT scan, and one MRI. No structural problem. No nerve impingement and entrapment. I have been working with a pelvic floor physical therapist for some time. It seems to be helping, but the symptoms persist. In this case scenario, should I schedule a consultation with your clinic?
Thank you for your question. I am glad to hear that physical therapy seems to be helping and that your doctors have ruled out other potential medical causes. @@drlufly It is possible to dry needle the transverse perineal muscle directly. However, we would recommend finding a provider that has been trained in pelvic floor dry needling specifically.
For folks with penile pain due to muscular dysfunction, if you are working with a physical therapist, you may start to find relief in 6-12 weeks. For folks with nerve pain in the penis, it may be a longer road to recovery, anywhere from months to years. The very embarrassed to ask this but does the pee get trapped in the penlie or is that just no sense also when will does the last? If not that serious is there any remedies for it?
Thank you for asking this question that more people are also wondering about. The urine is stored in the bladder (which is behind the pubic bone in the lowest part of the abdomen). When urine is ready to leave the bladder, it travels through a tube called the urethra which runs through the middle of the penis and comes out the hole, or meatus, at the tip of the penis. Urine rarely gets trapped by strictures, or narrowing, within the penile urethra. When people report they are having a hard time getting the urine out, that is a sign that they should see a urologist and pelvic PT to learn more. There are treatments available.

Hello mam, Im getting pricking pain for a few seconds once every 15 days to a month since last 2 months. No other symptom mild penile soreness following this pricking pain. Could this be prostatitis? all tests are normal no bacterial infection doc said
Thanks for asking. To clarify, "prostatitis" literally means "inflammation of the prostate", but it is often used to mean "male pelvic pain". It is less likely that you have prostate inflammation since your tests came back negative and your symptoms occur intermittently. Take note if there are any other clues about the cause of the pain when it occurs. Does it usually occur with a certain movement, what time of day, with high stress levels, with a bout of constipation? I recommend talking it out with a skilled physical therapist or physician. This is the type of case we see often, and can help find a cause and treatment for.
@@harryg2453 This requires an evaluation from a urologist. There can be various medical causes for this and treatment options depending on the cause. In the absence of an identified medical condition, pain may stem from an entirely different origin so seeking care from a trained pelvic floor physical therapist once cleared from a urologist would be recommended.

Can i ask a question. Last month i was discharged from a hospital suffering from close distal femur fracture and after i had done my surgery. The doctor put a catheter bladder and i have to wear it like 4 days and after taking it out, for one week there's a blood in my urine and it's really hurt. After i was diacharged from a hospital, whenever i have morning erection my penile get really2 hurt and this has been going for almost one month not to mention that my penile can't fully erect. When will this problem gone?
The first thing I would recommend is talking to your doctor about the month of painful and incomplete erections. It is possible that the trauma of having the catheter plus the femur fracture and surgical repair led to the pelvic muscles contracting to protect you. This increased muscle tone can lead to pain and dysfunction. A pelvic PT can teach you ways to reverse this and restore comfort and full erections.

Were you able to make any progress healing this nerve pain?
Thanks for the info
Have you come across patients that have had issues result from excessive bike riding causing the nerve compression you mentioned in the video?
Absolutely! This is a common reason why we see clients in our clinics.
Sometimes with a penile stretch/strain due to the pain there can be involvement of the pelvic floor muscles that can possibly be contributing to your symptoms as well.
You are welcome to schedule with us if you happen to be near any of our locations! You can also request a digital consult via our website as well (if you aren't near one of our locations).

Yay now I got another problem in my life
Are you currently experiencing pain?
Thank you i was worried that something was wrong.
You're welcome!
What did you do to fix it?
Does perineal fistula create any problem there?

Is it possible that chronic constipation and hx of hemorrhoids can cause this compression/pressure issue?
I would like to ask a question privately. Can you facilitate this?
Hello! You can submit questions to our website to have someone call back regarding treatment. pelvicpainrehab.com/

Without an examination, we could not tell you the exact cause of your pain as there can be numerous influential factors to it. Have you seen a urologist about the pain you are experiencing? We recommend that as your first line of treatment.

@@PelvicHealth thank you so much for the information and advice, it's incredibly helpful. Will take your advice on board. Thanks!

Hi, how can someone from Nigeria consult you guys?
Yes absolutely! You can request a virtual consult here pelvicpainrehab.com/telehealth/
Can sneezing do this, i tore an ab muscle during the flu, few days later, there symptoms happend
An injury to an abdominal muscle could contribute to urinary symptoms. If you developed an inguinal hernia, that could contribute to penile pain and/or pelvic pain. If the symptoms persist I would recommend a consultation with your primary care provider and/or a pelvic floor physical therapist.
Would an MRI show if there was a nerve issue?
Great question! A MRI may be able to show if there is some inflammation or swelling, but that would still be difficult to tell if the nerve is the issue when other structures can be in play as well. Seeing a pelvic floor physical therapist would be helpful to work in adjunct with your doctor and perform a thorough subjective and physical evaluation to see if a nerve may be a contributing factor to your symptoms. Schedule a virtual appointment with us pelvicpainrehab.com/telehealth/. Hope this was helpful!

Have you seen a doctor about your pain?
@@S_SOO_RAJ hey how are you doing now?

Does an irritated nerve / pelvic pain and all of it symptoms go away with time or treatment or does it stay forever?

Thank you for your question. Your description of symptoms sounds like it's more gastrointestinal distress versus a pelvic floor issue. We would recommend consulting with a gastroenterologist.
How to make the skin feel better
Can you clarify your question?
If you feel that your current physical therapist is missing something, or would like a second opinion to see if any additional interventions may be appropriate, you would be welcome to schedule a consultation with one of our offices!

For folks with penile pain due to muscular dysfunction, if you are working with a physical therapist, you may start to find relief in 6-12 weeks.

Thanks for asking. To clarify, "prostatitis" literally means "inflammation of the prostate", but it is often used to mean "male pelvic pain". It is less likely that you have prostate inflammation since your tests came back negative and your symptoms occur intermittently.
