There's a very specific frame at that part which allows you to pick up the axe. Because your placement is not where the scripted event happens, the character sprite is instead warped to a corresponding location that's shared with that tile.
I discovered this game 2 years ago from a video, played it, said "good indie" and never seen it again. Two years after that it comes to my mind and I discover that people are speedrunning the shit out this game lol.

Ok so the slug is usually a kid that the girl killed and the gnarled witch and the two girls were gnarled hag's daughters the two girls one that looks like she broke all the mirrors and two she looks like she is a witch and the three of the happy family of gnarled hag. The witch gnarled hag just turned a kid to a slug and the kid just swallowed the key so that’s all for now- took me long to finish it-

Any% speedrunning takes an unbelievable amount of work and dedication, because they have to find the glitches and exploits they use on their own - some of which can take literal years before they figure out how to reliably set them off - sometimes working harder than people who actually QA tested the game. It's cynical and insulting to dismiss all that work as "just cheating."
