The reason the orphanage's Rher circle leads to trenches is because most of the orphans were sent to fight in the great wars.
Its a self perpetuating cycle. The orphanage takes in orphans from the war, then sends them back to the frontlines as child soldiers to feed the conflic, thus making more orphans.

FH1 Ending A is pretty emphatic on how no one will ever know - COULD ever know - the PC's role in the birth the God of F&H deep down in the bowels of that place.
In fairness The skin-bibles themselves are meant to be in-universe books (thus no clue about the Girl's final days) heavily edited and censored by the Vatican occultist(whom might change the story even if they did know, for some selfish doctrinal reasons), and not reporting from an exterior all-knowing observer.
Unfortunately we must settle only for knowing The Girl was grateful for those small kindnesses.
They were pretty happy about that. You can even get them to say thank you.

Yeah the environmental storytelling is really great. Really enhances the game's experience.
Btw the orphanage and specially its alternate version relates to one of the playable characters very highly. His story is very tragic. It explains the looks of the alternate orphanage.
Meat Grinder is the best weapon(35x3 for total of 105 attack pow). I think Marcoh should equip an item around 90 attack pow.
Honestly never seen the Tanaka events nice to see it here.
