  • Identifies self by name. Identifies resident by name.
    Washes hands.
    Explains procedure to resident. Speaks clearly, slowly, and directly. Maintains face-to-face contact whenever possible.
    Provides privacy.
    Makes sure resident is wearing non-skid shoes that are fastened. Starts with scale balanced at zero before weighing resident.
    Helps resident to step onto the center of the scale. Makes sure resident is not holding, touching, or leaning against anything.
    Assists resident off of scale before recording weight.
    Records weight in pounds. (writes down)
    Helps resident step onto the scale, facing away from scale.
    Asks resident to stand straight. Helps as needed.
    Pulls up measuring rod from back of scale. Gently lowers measuring rod until it rests flat on resident’s head.
    Helps resident off scale before recording height.
    Records height in inches. (writes down)
    Places call light within resident’s reach.
    Washes hands.
    Documents procedure.
