If you listen to the ATC recordings, they did a fine job. The helicopter asked for visual separation and said they had the aircraft in sight. They were granted that, which is a common protocol. After that ATC even went above and beyond to tell the helo to pass BEHIND the airliner when they saw they were getting too close, but unfortunately more than likely the helo pilots had the wrong aircraft visualized and didn't see the one they hit until it was too late. It's a tragedy, but it had nothing to do with the ATC at the time, but rather the Blackhawk pilot/s. :(
How is ONE controller acceptable in such a complicated and busy airport such as this one? A medical incident can happen to anyone at anytime, and they only staffed ONE controller?
It is considered minimum standard. I was a helicopter flight nurse for 25 yrs. Our local airport didn't even have a tower with a significant GA presence and 4 RJ flights per day. The end of the main runway was less than a mile from our hospital pad. We had 8 radio channels to monitor during flights. Military training flights were at night along the river channels and they were usually on the deck and we were 500-1200 feet agl. Fortunately most pilots in our airspace we constantly vocal on the radio so we knew where we and everyone else was in the space.
Or the plane was too low, that's why we're waiting for the black boxes. Heli shouldn't fly over 200ft and the plane shouldn't be below 400ft. There's already some channels explaining this stuff that also keeps politics out of it.

“Pilot has aircraft in sight, requesting visual separation” Air traffic controller replied “ visual separation granted” Meaning black hawk pilot told air traffic controller he saw the plane and he requested responsibility in visually separating him with the aircraft. Which he did not.
He saw the wrong airplane sir that's why he requested visual separation, which he did separate himself from the airplane that he saw but not the airplane that the control center was referring to which was the one that he collided with, trust me this was not a suicidal it was a mistake and lack of judgement by the helicopter pilot.

I wouldn't be surprised if this deadly mistake was made because someone was overworked and exhausted. It happens in Healthcare, especially in nursing homes and sadly home health agencies. They work nurses and aides to the point of exhaustion. I've been there. I had to leave for the safety of myself and my patients/ clients. If my brain isn't functioning properly due to exhaustion, I'm putting everyone at risk. It should be illegal to work someone from 7am to 11pm 6 days a week. 7 if you want the money. They can keep it if you ask me.
Listen to the ATC. The controller told the helicopter to go behind the commercial aircraft. The helicopter requested visual separation twice and confirmed they saw the aircraft. Someone in that helicopter did something wrong.

ATC is considered to be the most stressful job that someone can hold. You need to be the best of the best to be in a tower. DEI is not just about diversity, there are 2 other letters in the acronym.

There are 4 active military bases plus the pentagon complex in that space. I understand there were 4 separate flight highways along the river. The RJ was crossing the river on final and the RW was following his highway. It appears RW was above the 200 ft altitude maximum. Of course it is possible the RJ was below glide scope. I heard it was not an ILS runway so RJ was in pilot control. Most planes currently are computer landed.

The staffing at ATC was not a factor in the incident. They clarified multiple times and the helicopter responded they were maintaining visual separation. What is ATC going to do when helicopter pilot isn’t tracking the correct plane but thinks he is?? Nothing.
I was up until 4am crying and trying to understand what happened. After hearing the communications between the aircraft and the control tower, everything seemed to be transpiring normally. I heard high winds were in the area at the time of the accident, and that has to be factored into the equation. Humans are flawed. Machines are flawed, too. This was simply an unfortunate accident. Tragic, but an accident. My condolences to those who are suffering a loss and to those who had the grim detail of recovering bodies. Thank you for your bravery under the harshest of conditions.

Hey guys, find out why there was no 2nd crew chief. They should have one sitting on the other side of the Blackhawk so they can be the eyes for the pilot's blind spot. I'm thinking the pilots mistaken another plane for the one ATC called out. If they had that other crew chief this wouldn't have happened. I could have sworn there was regulations on this.

I lost three dear friends and fellow USAF airmen when Northwest Airlines Flight 255 crashed on August 16, 1987, which will stay with me for life. 😢

The DC airspace is complex. There are 4 active bases plus the pentagon in a 10 mile radius of DCA. Both aircraft on approved flight paths. Either the RW was above the 200 ft max altitude or the RJ was below the glide scope. I understand that runway is not ILS so pilots have control. On the ILS the planes computer does the landings

I was a helicopter flight nurse for 25 yrs. Night VFR flights are significantly more challenging. You are unable to visualize the horizon to maintain spatial orientation. I flew in primarily rural areas in pitch black conditions. We had to know heights of towers to aid in navigation and altitude above ground. In the cities all the lights blend together. There was a plane taking off so the RW pilot may have been looking at that plane. It was obvious from the map the helicopter was in its flight plan highway on the river. We actually had fast movers jets following our river channel actually below our RW altitudes. The always travel in pairs so we had to make sure to spot both of them. Night vision goggles change the entire perspective and were not designed to be used in urban settings. We used them to identify wires when doing scene flights O

It has to be more than just altitude, there's no way any two aircrafts should be sharing the same bubble of airspace only 100 ft apart.

ATC is considered to be the most stressful job that someone can hold. You need to be the best of the best to be in a tower. DEI is not just about diversity, there are 2 other letters in the acronym.
@@chuckwright4346 there is an entire process where you are trained, evaluated and qualified before sitting in the tower. If someone fails those processes, they are booted from the program. DEI is about considering people of diverse races and ethnicities for THAT TRAINING. It doesn't say seat them in the tower regardless of qualification.

My question is, of all places for the military to do helicopter night manuvers, why the heck would you chose close to an airport with a known over-crowded airspace?
There are 4 active military bases plus the pentagon complex in that space. I understand there were 4 separate flight highways along the river. The RJ was crossing the river on final and the RW was following his highway. It appears RW was above the 200 ft altitude maximum. Of course it is possible the RJ was below glide scope. I heard it was not an ILS runway so RJ was in pilot control. Most planes currently are computer landed.

The staffing at ATC was not a factor in the incident. They clarified multiple times and the helicopter responded they were maintaining visual separation. What is ATC going to do when helicopter pilot isn’t tracking the correct plane but thinks he is?? Nothing.

I was up until 4am crying and trying to understand what happened. After hearing the communications between the aircraft and the control tower, everything seemed to be transpiring normally. I heard high winds were in the area at the time of the accident, and that has to be factored into the equation. Humans are flawed. Machines are flawed, too. This was simply an unfortunate accident. Tragic, but an accident. My condolences to those who are suffering a loss and to those who had the grim detail of recovering bodies. Thank you for your bravery under the harshest of conditions.

Hey guys, find out why there was no 2nd crew chief. They should have one sitting on the other side of the Blackhawk so they can be the eyes for the pilot's blind spot. I'm thinking the pilots mistaken another plane for the one ATC called out. If they had that other crew chief this wouldn't have happened. I could have sworn there was regulations on this.

I lost three dear friends and fellow USAF airmen when Northwest Airlines Flight 255 crashed on August 16, 1987, which will stay with me for life. 😢

It seems like the last 5 years or so have just been a grueling series of disasters one after another. I know that tragedies do happen but it just seems like one punch after another. Hopefully however we can not only mourn this tragedy but learn from it and never allow it to be repeated. My heart and prayers go out to the families and all those involved in this horrific accident.

The aircraft did not collide with helicopter. The helicopter collided with the plane that was on short final and ready to land.

That doesn't have anything to do with this. The responsibility to keep out of the plane's path was on the helicopter. Controllers didn't do anything wrong here.

The DC airspace is complex. There are 4 active bases plus the pentagon in a 10 mile radius of DCA. Both aircraft on approved flight paths. Either the RW was above the 200 ft max altitude or the RJ was below the glide scope. I understand that runway is not ILS so pilots have control. On the ILS the planes computer does the landings

I was a helicopter flight nurse for 25 yrs. Night VFR flights are significantly more challenging. You are unable to visualize the horizon to maintain spatial orientation. I flew in primarily rural areas in pitch black conditions. We had to know heights of towers to aid in navigation and altitude above ground. In the cities all the lights blend together. There was a plane taking off so the RW pilot may have been looking at that plane. It was obvious from the map the helicopter was in its flight plan highway on the river. We actually had fast movers jets following our river channel actually below our RW altitudes. The always travel in pairs so we had to make sure to spot both of them. Night vision goggles change the entire perspective and were not designed to be used in urban settings. We used them to identify wires when doing scene flights
