  • Neck Examination - Cervical Spine Assessment - Clinical Skills - Dr Gill
Compose a new pain within athletes is cervical spine discomfort, thankfully in the vast majority of cases when the neck is examined the cause of the neck pain is found to be muscular.
However, pain can also refer from the neck to the arm, in which case it is important to be able to assess for cervical radiculopathy prior to gaining more information which may indicate an MRI is needed
We assess for radiculopathy by doing Spurling's test, an often overlooked part of the neck examination, but it should be included for completeness and reassurance of the patient - not forgetting the athlete or not, neck pain can be a considerable source of distress, so it's vital to be able to get information from the neck examination which allows you to safely reassure a patient when appropriate, or comment that neck exam found evidence that needs further investigation

Considering the large number of the ASMR community that watches Dr. Gill's videos, I would say that the ASMR phenomenon deserves far more serious research than it currently receives.
Formally, Dr. James Gill performs routine medical procedures. But the manner in which these procedures are performed, the mesmerizing voice of the doctor, and the kind/attentive attitude towards the patient is what causes a surge of pleasure waves in many people who watch the videos, incl. me too.
So it seems that Dr. Gill heals more with his voice and his attentiveness to the patient than by performing routine procedures. In this case, the performance of routine medical procedures is just the framework through which Dr. Gill displays his enormous ASMR talent.
I will say something that has been well known for a long time, but is often forgotten - people get sick first mentally, and only then physically. First the soul suffers, first the mind suffers, and only then the body begins to suffer - this is what medical science calls ''psychosomatics''.
And when a person goes to his doctor for treatment, he (the patient) often needs a conversation with a friend more than a medication. A person needs to be listened to with empathy, to be treated as a close person, not as a product that has health insurance.
In fact, Dr. Gill (knowingly or not) is giving us a glimpse of a future medicine that will not rely so much on strict medical treatment of the patient and on drug therapies, as it will have a beneficial effect on the human soul and psyche. And the doctor will treat his patient like a close friend.

Seeing this video suddenly reminded me of when I was in sixth grade and experienced neck pain that kept me out of school because I couldn’t turn my head sideways at all and had to rest it in a recliner with ibuprofen. It turned out that the pain was caused by bad posture in my sleep from being a stomach sleeper and sleeping with two pillows and as soon as I stopped sleeping with a second pillow it restored my posture and cleared up the pain and my neck returned to a normal range of motion. Even though it’s been nearly 27 years since I experienced that I still remember how awful it felt and have done everything I can to try and prevent it from ever happening again.

I don't have a medical background either, but dr. Martin Samuels taught in a similar asmr exam video that hitting the muscle directly gives a different reflex (aka direct muscle excitability) than the tendon reflex that you're trying to test. So in the arms where the muscle and tendons you're testing are close to each other you have to put your thumb there to make sure you're actually testing the tendon reflex. In the legs it doesn't matter since the tendons being tested are further away from the relevant muscles and there's no risk of hitting them directly. He also said that direct muscle response is important, but for a different reason than tendon reflexes.

The girl was so sad and somber all the way through the examination, I've thought for a moment, there is some real issue in her life, but in the end there is a brief moment when she smiles, like a sun suddenly hits a spot amongst a grim day, and I think, okay, all is okay, phew... 😅

Bro she comes in complaining of neck pain, he does a few tests, determines it is not related to the nerve, then tells her to leave.
Now all thats left is to mail her a 300 dollar bill and youve got a bona fide american medical experience.

Chiropractors can make problems much worse. I would active dissuade any patient from seeing one.
Physios however are brilliant, and very knowledgable health care providers working from a scientific basis

@@grf15 Chiropractry is neither scientific, nor medical. It's a pseudo-science placebo literally invented by a guy who had absolutely 0 knowledge or expertise about ANYTHING medical related. The entire fundament of chiropractry is based on make-belief and spiritual nonesense that unfortunately managed to survive long enough for people to actually mistake it for a serious scientific/medical field.

Buddy. It's a lifted second rib on the left and a dropped left clavicle due to a tight posterior scalene on the left. I can see that without having to do any tests. You may want to get some better training. The whole manner of decorum is respectable at best, but you need to actually be able help people. If this is actually your clinical practice and is reflective of others... no wonder so many people wind' up under the knife
