So many times people will split up the info we need and because legally we are bound to what the person signs on the form even if we know how to correct it we cannot so the wire fails. Wires are just messy. And yes of course someone on the banking side could absolutely have made a mistake as well with entering the data into the system and swapped or mistyped a number. Usually before contact is made back to the requesting party the company if they are good will go back and double check the original information and the work done.
