For anyone who is interested, the Modified Barium Swallow Study was developed by a PhD and researcher in the field of Speech Language Pathology named Jeri Logemann. This study is considered a therapeutic exam to allow the Speech Language Pathologist to identify swallowing impairment, characterize it, trial compensatory strategies as appropriate to fit the problem observed, determine an appropriate diet texture and liquid consistency, and finally determine what therapeutic exercises and treatment the patient may benefit. I've had a swallowing problem for over 30 years. It occurs randomly and without warning. The food is trapped on or in the epiglottis and the upper esophageal sphincter. No matter how much I make myself vomit, the swallow mechanism won't release. If I drink small amounts of water to make the pharynx? do what it's supposed to do I begin to choke and can't breathe. After an event like this, which can last for hours I am completely exhausted. It began, the first 18 months, with only meat products but now, almost any food will cause a problem and randomly. I can eat for a number of days without trouble and then all of a sudden: Bang,...have a free choke with that! I've done the Barium swallow, botox in the throat, not sure where, I was asleep during surgery which cost a fortune. Funny though, I found that biscuits, sweets and cakes have never caused any problems.
I would add that while you are correct in that ENTs and Neurologists often get referrals for patients experiencing oropharyngeal dysphagia, they are not typically making that diagnosis. They should be referring those patients to speech pathologists to diagnose and treat oropharyngeal dysphagia, as speech pathologists are the preferred providers for this disorder. Speech pathologists are the only clinicians that conduct instrumental swallow exams, which are the only exams that can reliably and objective make this diagnosis.
Thank you so much for this video. We spent 8 hours in the ER yesterday with my husband who has Myatonic dystrophy and apparently aspirated at some point. We were given ALOT of unclear information basically we heard blah blah blah dysphagia blah blah posible feeding tube blah blah. He had the Barium xrays and a CT scan of the throat and referred to our primary not really understanding the results of the test
I have had sudden dysphagia sinch early March outnof nowhere ive had better and worse days i at first lost 4 lbs but gained a few back close to what I was im eating food though not everything I ate prior to this. I went to the ent and he sent for a modified barium swallow on Friday though the speech pathologist I spoke to said she wanted a accessment first qith a basic swallow test but the ent has the print of it so I dont know how or why she'd want to override the doctors orders? He looked down my throat with that thin test from my nose with a cam, said no lumps, polyps or anything in my throat or on my vocal cords. Ive noticed since last year I had this dry cough was either covid or bad allergies as it lasted a few weeks roughly last April. I never knew since my lab botched the test so i never knew if it was covid. Then after It stopped I noticed the muscles in my throat felt sore, tighter and hurt a little when I swallowed or did voices or speak. Ive voice acted so im always practicing. Anyway, fast forward to Marchlast month and all of a sudden I noticed at night maybe cause of the central heat in the house turned up higher over the winter very dry and having dry mouth with this sensation in the middle of the night of choking on my own swallow can't swallow even with water it would take a few sips but unpleasant as it would cause my heart to race a little. On top of that id have trouble eating, swallowing liquids even I have to pause but its like I'm hesitating afraid ill choke and sometimes its like i swallow accidentally on reflex which may accidentally cause me to choke but thankfully this happens more qith water, liquids then food. Its like food once infeel it atbthe side or back of my tongue ready to swallow I hesitate and i dont know why. Told the doc followed up he said after the examination since I do have bad anxiety too, he said he thinks its acid reflux snd anxiety. He did prescribe medicine but I can't swallow pills well, my current for anxiety ive been on for 13 years and a (Xanax when needed) I can chew with liquid or mush in yogurt or something. But for years would practice swallowing pills smaller and sometimes have luck others not like pill dysphagia. Its similar to that but with food and drinks. However the other symptom I have is my throat sometimes feels dry and slight burning sensation as well as tongue feeling twitchy or spasms which causes difficult swallowing. He said its prob a combination of anxiety and acid reflux but im not convinced how does all of a sudden I forget or have trouble swallowing. It gives me anxiety every time I eat worried i won't have enough food. Worrying of swallowing wrong or I'll choke. 😥 Sometimes even talking will hurt my throat and I have to stop as I feel my tongue clicking or like its moving around due to stress or something. I didnread fatigue can cause tongue issues and dysphagia but doctor told me to not self diagnose or I'll get paranoid. I dont believe this is all from anxiety. I feel sometimes slight pain in my throat around the Adam's apple when I swallow on the sides then it'll go away come.or feel dry hard to swallow.. I hope this swallow test isn't hard and I can be normal again I dont want to live like this....genuinely scared. I spoke to one of the ents colleagues before he sent the script foe the modified swallow, via phone one day before my doctor got in, and he said it could be acid reflux as ir causes lots of issues for different people. Now the ent I saw did write me a prescription but insurance didn't take it after he changed the capsule since I can't swallow them. It was something that absorbed in liquid then I drink it but that wasn't covered would have costed 300 bucks so the pharmacist and ent agreed intake prepaid complete..it doesn't seem to help my swallowing issues...in fact ive hardly taken them yet I have better days when im almost normal and days where I feels its nearly impossible so I eat less.
@@roxyc4603Unfortunately I haven't met anyone *exactly* like my case but similar as the docs even sound puzzled but I'm doing better with food at least as ive always been a slower eater so nothing has changed much there i just take my time and chew slower before I swallow. Dry foods are tough sometimes.. Drinks still are challenging though as I really only drink water and juice now. Sparkling ice i love but due to the carbonation its tougher to swallow like the fizzy feeling you feel with soda feels like its touching the back of my throat and causes me to hesitate sometimes ive lost my swallowing speed like I cant just take a few gulps when I'm super thirsty I have to take a big swig then swallow 3 or 4 times carefully... I also noticed after I take a shower and try to eat well drink mostly for whatever reason its a little harder like the warmth from the shower makes it harder to drink its odd I know but true. Im surviving but its definitely not fun but im just adapting its all I can do since none of these doctors can cure it. The ent said physically im fine but recommended I see a gastroenterologist to make sure the esophagus isn't irritated or something. Thanks for asking though and hope you feel better too.

@@Pagoz21 it went away on its own after I stopped eating and drinking trigger foods, and stopped cleaning my throat so much. For me it was spicy, low fiber carbs that when I eliminated the feeling went away. I didn't take any drugs and also didn't eat after dark. One thing I found helped was chewing a couple semithicone aka gas'x tablets every so often.
