    Concerning the apogeotropic nystagmus, the affected side is the side with the less intense symptoms. When the clinician is not sure, she/he should perform the bow and lean test (see other videos). When she/he is one more time not certain for the affected site, should treat both sides.

    Hello, I was studying about it in the Book REHABILITATION VESTIBULAR FOURTH EDITION. WATCH OUT! MANY MISTAKES COMMENTED IN THE VIDEO. SEE A SCOPE THAT I WILL REMOVE FROM THE BOOK, IN THE CASE OF CUPULOLITHIASIS THE AFFECTED SIDE IS THE LESS SYMPTOM !!!
    In patients with SCC's horizontal BPPV, the Hallpike-Dixand Sidelying tests can not cause vertigo and nystagmus, because the final position does not place the horizontal SCC in the plane of pull of gravity. The most commonly used test for horizontal ECC is the Roller Test (Fig. 20.5), in which the patient is on a bench press and rotates his head in the plane of the horizontal SCC.35-37 The rotatomized nystagmus is horizontal and may have a torsional component. In the form of canalithiasis of SCC Horizontal BPPV, nystagmus is geotropic; that is, the last phase strikes toward Earth and is brief in duration. In the form of cupololithiasis of CPB Horizontal ECC, the nystagmus is apogeotropic (the rapid phase is removed from the Earth) and has an extended duration. The affected side is considered the most symptomatic side of canalithiasis and the least symptomatic side of cupulolithiasis (2nd Edwal Law). The Horizontal SCC Scroll Test BPPV has only validity, and its sensitivity and specificity have not been re-ported. However, the effectiveness of the Roll Test was assessed indirectly in a Bow and Lean Test.3

    Can you please tell me if the manuever for the canalithiasis are to be avoid if you have Cupulolithiasis. I have Cupulolithiasis and I am not sure which one to do. Every time I do manuever I feel sick for long time! Please help, Thanks

      I would go have a therapist perform this. It should only take one visit to fix it.

      IndependentFckr The side with more intense ageotropic nystagmus and more powerful symptoms during the roll test indicates the affected side.

      is this the only way to tell which side is affected? i heard from a danish doctor that the side one roles on, where the most vertigo can be felt is the affected side. is that true?

      Correct me if I'm wrong, but I thought the side with more intense GEOTROPIC nystagmus/ LESS intense AGEOTROPIC nystagmus will be the affected side?
