Autopilot Anomaly [with ATC audio]

- Опубліковано 27 січ 2017
- On January 11 2017, an Etihad Airways Airbus A330-200 aircraft operating EY833 from Hong Kong (VHHH) to Abu Dhabi (OMAA) experienced a flight control issue shortly after takeoff, forcing them to return to Hong Kong. The autopilot fault forced a four hour hold to burn enough fuel to attempt a safe, underweight landing on 07R. This video includes all air traffic control communications with the crew.

For all the people commenting that they could have proceeded to abu dhabi without autopilot, no the could not have. Flying to abu dhabi you will have to enter rvsm airspace, which requires an autopilot, since reduced vertical separation is in force to accomodate the busy traffic flying on those airways. They could not just fly below rvsm levels since the lower you fly, the more fuel you burn, so the fuel would not have been enough to reach abu dhabi. Everything the pilots here have done could not have been done better. Great job both from the pilots and the atc
Thanks for that perspective, Kevin. It's very easy to be an armchair pilot and cast judgement on the actions of a crew without all the relevant information. I hope this helps viewers consider the multidimensionality of such an incident and the complexity of decisions that have to be made.
Not only that but Hong Kong might have better accommodations for a grounded plane.
Wow - Thank you - amazing work! The narration and background information really adds to the understanding of the story - Thank you!

Ouch - hand flying four hours of holding!
Yeah. Super tedious and requires alot of patience and pilot handoffs
Very informative video and narration, professional quality. Also of professional quality was the communication between pilot and ATCs.
Congratulations!!!

Local Stand -by is when only the Airports (local) fire service respond, A Full Emergency response would include all emergency services from local towns and city.
Thanks for the informative video.
Maybe I don't know enough about aviation to understand the issue in this video, though. So the autopilot was not functioning, but all other flight controls were. Why does that mean they cannot complete their flight? They did a lot more manoeuvres in the holding patterns and returning to the origin airport, than they would have done if they had gone on to their destination. It doesn't make sense to me.
Thanks for the high quality video mate! Keep on going! By the way, it's kinda strange that the autopilot system failed considering that they did an ILS approach and isn't Airbus A330 equipped with 2 APs? Greetings from Melbourne!
+09shadowjet The autopilot system as a whole didn't fail, rather part of it malfunctioned (exactly which part, Etihad didn't specify). So yes, vertical and lateral navigation still appear to have been available (including ILS functionality), but whatever the issue was, it was enough to convince the crew not to continue with the service.
Thanks for watching!
AirTrafficVisualised Thanks for the explanation!
how do you edit these or animate these videos? these videos are very good.
After Effects for the animation and Premiere for the editing. Thanks for watching!
instantly subbed because of the intro
Welcome, glad you enjoyed it.
AirTrafficVisualised Thanks, great video!
I had thought that all commercial airplanes were able to dump fuel. That seems somewhat dangerous to stay in a holding pattern to burn fuel.
A lot of commercial don't have it, its not a requirement. In an event where you have to return to the airport because of the emergency then they would preform a overweight landing, this wasn't a critical enough situation to warrant the need for an overweight landing.
Would dumping the fuel not be faster instead of holding for about 4 hours? Provided the A330 has a fuel dumping system on board. Must be very hard for the passaenger, sitting for I guess in total 5 hours in an plane and end up at the airport you left from. But at least no one got hurt. That is the most important.
I hope you can do the Swiss 40 flight to LA which landed in Iqualit recently
Have considered it, but there's no ATC audio available so I don't think it'd be particularly interesting. I have made videos without ATC audio but I try to keep it balanced.
AirTrafficVisualised Ok, that's pity. Anyway, I just found your channel a few days ago but it's great work you do here! :)
What do they do in more complex emergency incidents if they don't have fuel dumping?
They didn't dump fuel in this case, but burned fuel.
If they have to return earlier to the airport they choose the longest runway and to a heavy landing, means excessive stress for the brakes and the undercarriage. Means the undercarriage needs to be inspected/replaced and the breaks might need to be extinguished. Else the tires might explode. None of this is very severe, they just need emergency equipment for standing by.
In any case a heavy landing means, that they cannot just fix the initial problem and fly again, the aircraft need some intense inspections after that. So crews try to avoid this, if there's not an serve emergency.
The larger planes can dump fuel. On the A380 incident for example the system was out of service due the excessive damage to the airplane systems, so they had to make a extremely heavy landing, stopped only 100 feet ahead of the runway ending.
Ruben Kelevra Thanks!
What software do you use to make your videos?
Scarebus aircraft don't have fuel dumping capability?
I believe fuel dumping is a customer option on the A330.
pardon my ignorance, by the xtreme reliance on automation? could they not "hand fly" the plane to dubai?
Hand flying over long distances (or really any distances) is simply not done in commercial aviation. Automation is used to ease the aircraft handling load of pilots, so they can focus on other, less immediate concerns (i.e. planning approaches, considering weather, diversions, fuel, managing other aircraft systems for increased efficiency, etc.).
Also, it seems likely from the aircraft and crew's actions that many parts of the autopilot were functional, so whatever system did malfunction was enough to convince the crew not to continue with the flight.
Not done but can be done. I've done it.
Very good made video. Why didn't they just dump fuel? It would be allowed.
As stated in the video, Etihad's A332 fleet doesn't have fuel dump capability.
Ok, but it think it is a big mistake to not install the system, if youre in a really big emergency it is a loss of safety.
BUI KADJOS costs money tho a lot of airlines don't justify the cost of it
The A330 is not capable of fuel dump?? It's an etops aircraft, I didn't know that.
Fuel dump capability is a customer option for the A330 family.

Generally speaking I love your channel, the visuals in this E are great except one small part. The depiction of holding onto fixesis wrong, the Betty fix holding pattern is correct. When holding the is ALWAYS an inbound leg of at least 1minute before turning outbound. Never turning into the fix.
Beyond that a few informative things based on the questions here.
Losing the autopilot is in itself not an emergency, but it has some serious implications.
1: cannot use 1000ft separation in most Enroute environments. It is almost impossible to maintain the necessary degree of altitude hold required to safely operate at higher altitudes so a block altitude of 3k ft oulde be required. So at an altitude of fl320 no planes could pass near above FL 300 and below FL 340
2: auto throttle would not work properly so speed control, and to a large degree alt control is labor intensive.
3: I'm willing to bet the company requires as part of their MEL (minimum equipment list) include a working autopilot before starting an international flight.
Since this is not an emergency then no reason to attempt an overweight landing.
Nice job on the explanation of how other ATC systems work, I appreciate it.
Thanks for adding that extra information!
Regarding the holds, all depictions in this video are based on radar data (multiple sources). They aren't "wrong", as always I simply present the track of the aircraft based on the best information available.
I CAN HEAR AN VOICE ON THE VIDEO THANKS
Wait what, I thought all A330 are equipped with fuel dumping
airlines have the option to not install fuel dump mechanisms on their A330 aircraft.
Maybe they had issues with this system too, if it is equipped.
Etihad elected not to install fuel dumping capability for its A332 fleet.
On a side note, during my research and planning of this episode, there were quite a few people around the internet rebuking the crew of this aircraft for their "indecision" during the hold. I hope this video shows that that wasn't the case.
AirTrafficVisualised thanks for pointing this out.
I was just wondering why they turn around because the AP is not working. They have 2 APs on board. And still if both are not working this shouldn't affect the rest of the systems, they could have easily flew the route by hand and schedule the fix after this flight.
So I guess they have no equipment to fix the Aircraft on the destination or the pilots had felt uncomfortable with flying by hand the trip.
Etihad's statement was suitably vague: "a flight from Hong Kong to Abu Dhabi experienced a flight control system fault after take-off, which resulted in the loss of the aircraft’s autopilot capability." The crew was able to maintain fairly precise holds over the prescribed waypoints, so I don't think the AP system failed entirely.
As to flying the entire flight by hand: I can't imagine any crew, facing a situation where the entire autoflight system had failed, electing to continue the flight. Modern commercial aviation is more about managing the flight than flying.
Great video. Only 1 questions remains: why didn't they dump fuel? They wouldn't have to hold for that long...
As I said in the video, Etihad's A332 fleet doesn't have fuel dump capability.
If the plane was fine to fly in a holding pattern for several hours, why didn't they just finish the flight without the autopilot? Especially since the destination was the airlines main hub.
The unexplained loss of a major system like the autopilot can be an early indication of a far greater problem such as an electrical fire. The point of flying in a holding pattern is to maintain a distance close enough to an airport to enable them to conduct an immediate emergency landing if the shit hits the fan.
Richard Andersson the emergency was identified (correctly) by the pilots as non-critical. It's much more dangerous to Rush in to the airport for landing without understanding the problem (what works what doesn't) running check list and ensuring that all precautions are taken and all options considered. The risk of staying in air was less than risk of immediate landing in this case. Pilots handled it very well and all are safely on ground as result.
The other reason that they went into holding is because they were above MLDW (Maximum Landing Weight). It would have been unsafe to execute an approach when exceeding this limit.
Autopilot is hardly a critical flight control. Seemed like the craft had no trouble maneuvering...why not just continue the flight under manual control?
Nic Neufeld That would be a pain in the ass
Indeed. We can't have people actually having to do something.
if there is a problem then its a problem. could be lead to a disaster. So better to fix all issue first.
Arif Hidayat How would a malfunctioning autopilot, turned off, lead to a disaster?
Nic Neufeld Computer Science student here. We learnt the concept of triple modular redundancy. Modern aircraft and man certified rockets use this. There are basically three computers constantly doing the same task and comparing their own set of sensors. So if one sensor says a speed of 200 knots while the other two say 205 then the computers vote to use the 205. If the autopilot system starts glitching out then it could indicate more serious issues like miscalibrated sensors and that warrants an immideite landing. Usually if one of the three computers were to fail you can continue the flight. Rockets constantly have their computers knocked out due to radiation. Reliable computation isn't really an issue, but false reading is.
Great video thank you. Why ATC have such bad English????
The voiceover is for the most part unnecessary and a bit distracting, I feel. You often just give us info that is basically repeated verbatim by the RT audio, and some of it would be better explained in text, or just through the RT.
I liked the opening voiceover, and the explanation of the holds. The rest was probably not needed.
Great visualisation as usual.
Sorry you didn't like the voice over, I'll try and be more concise where VO is used in the future. As I said to someone else, I do want this channel to be accessible to those who are not necessarily already into aviation, but perhaps this one went a bit too far in the other direction.
Thanks for watching and commenting nonetheless, constructive feedback is always welcomed!
Yeah I definitely understand the desire to unpack some of the stuff contained in the RT that the uninitiated wouldn't catch. Hard line to ride. I wouldn't say I didn't like the VO, just some room for improvement (sorry if it came off as a bit harsh - I definitely appreciate your work).
Not at all, I genuinely appreciate the honest and constructive feedback.
Hope to see you again next video!
gate 66, 6:20 local time?

No wonder the AP noped out on them! 😈
Bro You have misidentified your audience. It's 90% people who are serious about aviation and atc, 10% noobs who aren't that into this (not loyal watchers). To the 90%...We know why the f*** a plane holds, etc. -minor/moderate, burn fuel or critical emergency and overweight landing - and atc communications explain it all anyway. voice over key details of failure causing return. Mostly visuals w atc. too long. It's why you don't hav subs. Sincerely, MR seen all atc on YT.
Thanks for your comment, Michael. I'm sorry you felt I over-explained. I want this channel to be accessible to everyone. I won't be taking advice on why I "don't hav subs" from someone who has zero themselves.
Thanks for commenting.
AirTrafficVisualised hahahah roasted. I thought it was a fine amount of explanation. Sub from me 👍
Welcome!
AirTrafficVisualised So you don't consider viewer feedback? Seems unwise. Marketing firms spend millions testing messaging and paying test groups for feedback...I read back over my comment and ok it is a slightly harsh comment. But I genuinely wrote it as food for thought (and in the UA-cam world certain language "bro" is required). In reality, Just a doc who spends free time flying and falling asleep watching aviation/atc vids (if only you could see my hrs). my settings are all to private and I post nothing. I view everything. Your video... It is over explained and I suspect you know it. Don't take it as negative. You do a lot of great things. Was a suggestion. How long you been at this UA-cam channel?
