The non-accompanying parent or legal guardian of a minor travelling to Canada must fill out
Form IMM 5604 to prove he or she is aware that the child is travelling with only one parent.
The Separation Declaration must be signed in front of the parent and in the presence of a
public notary who serves as a witness. The ‘parent’ role, in this case, includes the current, as
well as former spouses or common-law partners. One application should be submitted per
child: if you have more children, make sure you fill out a form for each of them.
