It's very useful. How to access the control and change the dose of metformin and glimepiride as these two are most commonly prescribed .? And what is good way to monitor blood glucose like weekly testing or daily testing ??
N glimipiride, can be given in OD dosing, to be taken shortly before meals . Start with 0.5 mg , uptitrate at 1-2 wks to maximum 4 mg. When u initially start therapy or change therapy then preferably get daily testing done for initial 3-5 days then then u can advocate weekly testing
Sir as you mentioned if Initial blood glucose is >300 then we use combination of metformin + glimipride. So do we use that in od dosage before lunch or bd dosage before both meals ?
@@sourabhsoni3778 when initial blood glucose > 300 mg% or HbA1c > 10%, then we should prefer insulin therapy. If at all OAD's are to given, start with OD dosing slowly uptitrate taking care of hypoglycemia.

Metformin should be started at a low dose say 500 mg or 850 mg then uptitrate at 1-2 weeks because initially high doses cause lot of GI distress
N better to give metformin-ER (extended release) , u can give in OD dosing...
