  • Psychological and physical stress interferes with the male sexual response cycle and erectile dysfunction can result. The PEC pattern as defined by the Postural Restoration Institute describes how unresolved tension can place the pelvis in a position where the pelvic floor muscles can become painful and dysfunctional. Importantly, these muscles do not operate independently from the rest of the body and can't be the only muscles taken into consideration.
    Hey there, my name is Neal Hallinan.

    As someone who lived with chronic pain for many years of my life, I know how debilitating and isolating it can be. But I also know it can be resolved.

    My pelvic rotation caused me to have pudendal neuralgia from having limb salvage surgery on my left leg from Ewing sarcoma a few years ago. Always over compensating on my right side. Your videos have been helping a lot.

    I've had really painful manhood issues for at least 8 years, starting at age 15, as well as body imbalance and other chronic pain issues starting at about the same time. I don't think they're a coincidence, but no specialist I've seen (physical therapist, urologist, or otherwise) seems to agree. It's really hard to climb out of the negative emotional/physical health spiral. Being an anomaly is THE worst.

    Wow, just for the first time in 10 years expaned my right side with air, what a feeling. The single leg cable deadliift that you showed helped me a lot because the first time i used it, it showed how to extend the hip with closed left side, few hours later i can rotate my torso easily while consciously controlling my left abs, which i couldn't for years. Thank you, Neal.

    This completely sums up my current condition and the last 3+ years of my life. PEC, rotated legs, tight pelvic floor, neck breathing, unstable hips, fight or flight about the whole thing, ugh... It has debilitated my life.

    This is relevant for us ladies, too (and not just as the disappointed partner, lol). I'm a PEC and when I'm symptomatic I definitely have a difficult time being even remotely interested in sex. Plus, it makes reaching climax that much more difficult. Fortunately, unlike our male counterparts, neither of these things is a requirement, although they do make things a lot more enjoyable for both parties.

    Same with anything that requires your autonomic nervous system. Sleep, digestion, recuperation etc. Focusing on erections is as futile as trying to get yourself to sleep when you have insomnia. You are correct in that it's the anxiety management, breathing to get us out of fight/flight and back to parasympathetic control.

    I’m wondering about a left inguinal hernia presenting as part of an aic pattern? I’ve been a pilates, yoga, fitness coach for over 15 years and have recently stumbled across PRI and am pretty sure this has been my pattern for 20 years since a head on collision. I have been studying embryology, embodied movement and 3D/butekyo/vagus nerve breathing for many years but never with the aic pattern in mind. I was dabbling in Gary Ward’s teachings before finding you. I’m definitely interested in doing some of the training as soon as I’m done with an embryology program. Thanks so much for this transformative information and any thoughts around inguinal hernias from posture/breath patterns and exercises to help heal?

      Whatever tilt is occurring at the pelvis will have an element of anterior and lateral.

    Hey Neal. This is really interesting about what you said about breathing. I notice you talk a lot about how the pelvic floor muscles get affected by other muscles/patterns, but never about how the reverse. I got a pelvic floor injury 2 years ago from being kicked in the perenium, since then I am no longer able to breathe with my diaphragm and tension levels have gone through the roof as a result. I have seen pelvic pt's and they teach just to diaphragmatically breathe. The issue is, I physically cannot breathe as a result of what occurred to my pelvic floor! Can you shed any light about what can possibly be done so that I could get my ability to diaphragmatically breathe? Thanks!

      I have a tip that might sound stupid.
      Take a normal breath.
      Now breath out with force, clear the lungs of all air during a 5-6 second exhale.
      Now do nothing, just sit there empty lungs.
      After a little while the oxygen level gets to low and your body sends your brain/sympathetic nervous system a signal to take a deep breath.
      Because of the threat level (low oxygen) this signal is being send on such a deep subconscious level that it adresses all the muscles it needs to, even those that have not been engaged for years.
      (Do not not breathe for to long, you don't want to pass out or anything!!!)
      I had a motorcycle accident and crashed with 50mph/80kph into a static car that broke down in the middle of the road. Since than i've lost conscious controll over my entire breathing apparatus.
      But ever since i've been applying what i've written above i am slowly but very steadily regaining control of the muscles.
      That is because what i'm doing triggers neuroplasticity, the brain is making new connections, (re-)enabling my conscious capability of directing the muscles.
      Once again, don't overdo it, stay safe and give it time.
      It helped me tremendously though.

    It amazes me how PRI can be used to treat psychological and physical conditions. I think it’s be intresting to see a video on the potential correlation between BDD ( Body Dismorphic Dysphoria) and and inhibited body. Could someone feel ugly because they lost sense of facial muscles or a sense of balance between halves of their body

    I think that this mixed package of stress, anxiety and tension near pelvic floor can also cause premature ejaculation.
    There are people for whom training strength of pelvic floor muscles help, and there are people for whom relaxing pelvic floor muscles help.
    But I agree with you that it's all connected and you can't fix it independently.

      The author actually talks about premature ejaculation also. Still comes down to autonomic nervous system balance.

    Great video and awesome channel but I think it's a bit more complicated than stress - I have PEC Pattern and hard flaccid, however it takes less than a minute to go from a hard flaccid to normal flaccid EVERY TIME after I do hip internal rotation. I doubt it that my whole sympathetic system relaxes in a few seconds... It's possible though.

      If the left leg internal rotation allows you to use your left diaphragm to breathe (which is why we need it, I made a video about it), then yes, your sympathetic system will relax in seconds.

    The emotive stress component is relevant, but I think a lack of grounding can definitely be primary, or at least a hard limit to improvement.
    The big toe stuff from your previous video was very interesting. For large wide isa types, is there anything 'stronger'? The paper towel creates a nice sensory cue, but compressed and fell out of position quickly.
    Do you use short foot or anything like that?
    Do you have an opinion on whether the hallux ipj should be straight or bent when the hallux interacts with the ground?

      Well, an ungrounded body will be more tense and have, quite likely, more unpleasant emotions. I don’t separate physical grounding/movement from emotion since movement and emotion can’t be separated neurologically. The paper towel is only meant to be a “one-time or short-term” cue, to help the brain figure out where to push from. ISA isn’t something I pay attention to so I can’t give an answer to that question.

      Try bouncing on your toes like in boxing even on rythm which from i rembember is beneficial in THE pattern. I feel more grounded after few thai boxing sessions

    Sir my hamstrings are tight glutes are not engaging at all my abs are tight especially the left side and i cant fully take breaths if i have to take full breath my feet outwards completely like towards the side and my pelvic floor muscles are tight as well

    Is it all possible to have a higher left hip, weaker right eye, more comfortable using left leg, anterior pelvic tilt and still be patterned? These don’t fit the typical patterns mentioned on this channel.

    Hey Neal! I have a quick question. One one of your other pelvic floor vids you spoke about breathing with regards the pelvic floor. When you say the pelvic floor is compromised so you can longer pressurize it, and as a result, can't breathe, what exactly do you mean by “compromised”?

      I just mean that the pelvic floor muscles are mispositioned because of the mispositioned pelvis.

    I’ve had moulds for foot orthotics coming in 2 weeks for my collapsed arches due to probable Left AIC, high left shoulder high right hip, but my cranial issues I think will require a mouth splint (whole jaw over to the left side, mandible reflects shoulder pattern) - do you think the orthotics will solve the cranial issues or do u think I’ll need the mouth splint as well?

      There is no way for me to know. It depends on your testing.

      @@NealHallinan As a retired Sports podiatrist and a licensed massage therapist who has taken 500 hours of applied kinesiology, I can answer that. The typical pattern I see is a restriction of the Left cranium (Sphenobasilir fault; Universal interosseous cranial fault; Temporal bulge; sagittal; zygomatic and lamboidal suture restrictions). This one-side dysfunction results in a left-to-right side brain imbalance due to the difference in cerebral spinal fluid flow with a downregulation of the left cerebral cortex and cerebellum. You will see a tight calf muscle on the left due to a mortice subluxation (all subluxations are due to lax ligaments). You may see one on the right also. You will see peroneal inhibiting on the left with the left foot rolling out, and the right side will have an inhibited posterior tibial muscle. The pelvic dysfunction is typically a posterior innominate. The short answer is the skull is causing the foot problems, not the other way around. That is not to say the foot doesn't cause more proximal issues. For instance a sprain of the lateral talo calcaneal ligament will result in a posterior innominate until it is corrected.

      Yes. Anything in the oral cavity (trigeminal nerve) deemed “threatening” by the brain can contribute to dysfunction elsewhere. Still comes down to tension/stress. The author talks about the influence of stress/tension on women’s sexual function, too.

    I was hit by a car as a young child resulting in a leg that was put in traction, rotating my left hip forward, and I broke seven ribs and a collarbone resulting in the exact thing you just described massive amounts of constant tension.
    One of the things that medical does not address is the adhesions and the compensations, and undoing them in a practical way.
    Scar tissues, adhesions, and decades of compensation don’t simply release do you know something that we do not to get this done?

      I don't really have any experience with scar tissue/adhesions etc... That seems to be more of a traditional medical issue. PRI is all about undoing compensations, but there can be structural issues (like those you may have) that get in the way of unwinding compensations.

    Can you make a video on Myofascial Pain Syndrome (MPS)? I started experiencing body-wide muscle tension after a surgery 2 years ago. My massage therapist says my muscles immediately tense after she’s massaged them.
    Exercising hurts my muscles so badly. ANY muscle. In fact, doing PRI exercises with my Physical Therapist hurts my body because they aggravate my muscles. I CAN’T get to neutrality because my Myofascial Pain Syndrome won’t let me use my muscles properly. (Yes, we’re also focusing on breathing BUT it doesn’t help!)

    Im working a job on the road where I can’t wear my ASICS anymore. I have to wear work boots. I can still do telhe techniques in my shoes just not at work. Is this going to ruin things

      The ultimate goal is to be able to wear whatever shoes you need to wear. In the beginning of a PRI program you may initially need to wear a specific kind of shoes to teach you how to feel and use the floor, supinate/pronate appropriately etc.
      However, you shouldn’t have to rely on a particular pair of shoes for ground sense forever. If I were you, wear your work boots at work and practice finding your left heel and your right arch. You could put paper towels in specific places to help you find those places. I think Neal has a video teaching you how to put in a lateral heel wedge to allow you to find right arch sense. They also make OTC orthotics with a heel cup and a soft flexible arch. Periodically think about sensing those places on your feet and learning how to stand on the left leg with the right hip ahead of the left and your weight on your left heel more often. Then when you get home you can slap your ASICS back on.

      ​@@NealHallinan I did for a while, but assumed the pain was from excessive sitting, not from the exercise (still not 100% certain). It starts to ache not during the exercise itself, but maybe later that day or next day after doing a 90-90? What wouldn't let go? Pelvic floor?

    Please cover this more I suffer from pelvic floor dysfunction and hard flaccid which is just a term that I believe stems from PFD. A lot of men suffer from this and I have directed them to PRI.

      @@NealHallinanit’s just a term for pelvic floor dysfunction in my opinion. I think it stems from the pelvis being shifted and your breathing thrown off.

      ​@@NealHallinanthere are many causes. Some are guys trying to get bigger doing penile exercise.
      Other causes are because of too much edging/masturbation, like 4 hours many times - keeping the pelvic floor in that tightened state.
      Another cause, is the postural aspect, pelvis positions and muscles compensations etc.
      That's the one you should be expert of.
      Many of the guys with Hard flaccid have their penis leaning to left side, tight ischiocavernous muscle. (I do) Maybe that information will help you.
      Usually also have tight hips/anterior pelvic tilt / left aic pattern.
      Basically it's pelvic floor dysfunction. It's for the most part physical not psychological.
      Often guys report that they don't have that condition anymore after they fixed their bodies imbalances.
      I did for 2-3 weeks, after I started doing 1 leg exercises like step downs, step ups etc. But then it came back even worse. In my case I am 100% sure it's not psychological.

      The patterns themselves don’t link to premature ejaculation nor ED. The “patterns” themselves, much like sexual misfunction” simply reflect tension. The patterns are a ohysical manifestation of tension.
      But once you are in “the pattern”, you need to reposition and stabilize the body to reduce tension overall.

    Testosterone is converted to DHT. DHT is largely responsible for your muscles relaxation.

    I had ed after use home made penis pump. That may cause hard flaccid. And even i did many doppler with negative result . But i didn't heal . I go to pt and found pvelic floor tension. Then i restore some function and hardness.
    I did biofeedback that give me spontiouse erection when i toch hand of my girlfriend with more erection stringth in masterbation. But i still away from perfection . So i did cavernosography and found no venous leakage. But after that i lost my erection at all . That happen in 2022 until now .

    Hypothetical 😅 But could this lead to your penis kinda rotating to the left when erect? And maybe the left testicle being litterally pulled upwards and back into the groin? Just a hyothetical like is say lol

      LOl…. Hypothetically…..I really don’t know. I suppose a rotated pelvis could create some sort of restriction in that area, but no one has yet come to me for that issue. If the hypothetical individual in question sees a PRI provider and that hypothetical situation changes, please let me know.

    When i stand on my left leg my rigth pelvis goes infront , but the body weight is still on the right , everything on the right side is tight ... what pattern is that so i can start researching
