  • Medical scribes, offering remote scribing services to healthcare providers, are gaining popularity as a means to rescue clinicians from the extraneous task of charting. This video highlights how remote scribing works. A medical scribe does EMR charting working remotely from the office. He/she pens down all the relevant clinical information from the physician-patient interaction via video teleconferencing means.
We were at Bay Area Medical Clinic, Florida, with Dr. Jose A. Villaplana, M.D, who generously agreed to demonstrate the intuitive nature of using remote scribing with telemedicine that efficiently manages his clerical workload.
    How does Remote Scribing Work?
The medical scribe has credentials to enter into the EMR that the healthcare provider has been using. The scribe preps the medical chart ahead of time to complete as much pre-visit work as it can. As the provider is about to start his shift, he/she would log into the telemedicine application for 2-way communication with his scribe.
Just before any visit, the clinician will inform the remote medical scribe about the patient to be seen. It is when the scribe can confirm any important information about the patient, including the chief complaint, and begin EMR charting. As the provider enters the room, the medical scribe starts documenting the clinically significant information from the clinician-patient conversation, placing the relevant aspects of the narrative and exam into the appropriate sections of the EMR.
While the clinical encounter is in progress, the medical scribe simultaneously orders labs/scans and prepares prescriptions & referrals to be sent as directed by the provider. After the visit ends, the clinician reviews the chart and signs it, transmitting any orders, referrals, or prescriptions that have been placed.
The Significance and Benefit of Remote Scribing
Medical scribes help physicians decrease the EMR documentation burden, increase work efficiency, and improve clinical interactions with patients. Real-time clinical documentation done by a medical scribe enables physicians to combat burnout by eliminating data entry post visits and allowing them to finish their shift on time.
With a medical scribe working alongside, a physician can now focus on the patient and not on the computer, leaving EMR data entry to its scribe. The scribe provides detailed medical information to the physician within minutes, saving precious time. It then allows healthcare providers to see more patients, offer high-quality healthcare service, and earn more income/incentives providing value-based care to the patients.
Gone are the days when transcriptionists would perform medical transcription of the dictation sent by physicians. That process of sending medical transcription notes back to physicians had a TAT of 24-36 hours. Contrarily, remote scribing allows EMR charting to be completed and the medical chart to be signed by the provider within minutes post the visit. Also, remote scribing enables the physician to proofread better.
