    I'm 75, healthy and have had osteopenia since my early 50's. I'm 5'1",100 lbs. small boned. My mom had osteoporosis, as well as my grandmother. When my mom was diagnosed in the early 80's weight bearing exercise was recommended. I went to the gym, lifted weights, bicycle and ski. In 2018 I started working with a Functional Movement trainer. I lift heavy weights, do plyometrics, work on endurance, mobility etc. In 2021 my Dexascan showed osteoporosis. I'm due for my Dexa in Jan and my Dr. thinks I need to go on drugs unless my results improve. My weight and small size are natural to me but my Dr. looks at me and considers me fragile. My trainer told me the other day I'm very strong. BTW, I can dead lift my body weight, 95lbs for 8 reps. I have no plans for the diagnosis to hold me back and don't plan to do drugs just yet.

      Good for you re the weight lifting at your age! My mom did the same thing until 91! She had osteoporosis severely but still kept weight lifting. What I know about weight lifting is for me I gained 15 pounds of mainly muscle. If you eat well and often, plus eat adequate protein that is mainly primary protein, you should gain a decent amount of weight. Muscles weigh quite a bit more.

    Thank you for this great video. I had one Prolia shot and decided to discontinue due to many side effects. (one being lower back pain) It has bee 9 1/2 months since that injection. Doctors are telling me to do nothing at this point because my ctx/ntx scores keep going down. Currently 33 ctx/ 14 ntx. I have continued back pain with no history of backpain prior to this med. I am not sure if this is Prolia still in my system, or a sign of a fracture coming. Never had a fracture, eat very healthy and go to the gym 2-3 times a week. Thank you for what you do!!!!

    I'm 62, exactly 10 years out from menopause, and never taken exogenous estrogen, but have been wondering; about it. Thanks for the explanation of the risks for someone like me.

    I’m a 34 year old female newly diagnosed with early onset osteoporosis. I knew I had risk factors for low bone density (low bmi, amenorrhea) but I had to push my GP to refer me for a dexa scan. I thought at worst I’d have osteopenia but unfortunately the results showed it was osteoporosis. It has been incredibly isolating and scary receiving this diagnosis so young but I am grateful for this community as I realise there are so many other people out there with this condition. Thank you for this platform. Whilst I wait for an appointment with a specialist, I am taking all the lifestyle measures I can including supplementation, seeing a dietician, improving sleep quality, red light therapy and changing my workout routine to focus on safe strength training. I’d love to know peoples thoughts on red light therapy for osteoporosis.

      Hi Emma,
      I’ve researched red light therapy but didn’t find any compelling evidence to support it as a stand along treatment for osteoporosis. There are some compelling data on mitochondrial function and blood flow but I don’t see it directly impacting bone health.

    Looking forward to your masterclass. My questions are numerous, but as a 61 yr old fit & active female (swimmer) with osteoporosis, early menopause (44) and a history of 2 pulmonary emboli (which my NYC hematologist cannot account for) - I have put off the Prolia the metabolic bone Dr. recommended- primarily due to my insurance wanting me to try the more traditional medicines first. I am nervous about waiting until I am able to have Medicare at 65…so many hidden factors, while I feel great. I don’t want to play a loosing ✋!

    I've been enjoying your book and am looking forward to your Masterclass on Wednesday! I'm hoping you might address options for keeping osteoporosis from worsening during breast cancer treatment when one has to take an estrogen blocker. Is a natural approach enough to counteract the effects of the blocker, or is taking a pharmaceutical to counteract osteoporosis the only option? If so, are there any that you consider to be a little safer and effective without major side effects? I am told I will need to be on an estrogen blocker for 5 years. Thank you for any insights you have on this!

      This is the one situation where an antiresorptive may make sense for me. It’s a temporary drug to reduce the impact of another drug which is otherwise hard to mitigate. We do have patients in this boat who choose not to use bisphosphonates and we do see improvements BUT this is a case by case discussion

    I really appreciate your points about Prolia. My sister and I were diagnosed in our 40s. At 64, she went on Prolia at her doctor's advice. My doctors generally ignore my DEXA results. I have looked at the reports and they should have had more attention. Evidently, they stop looking when they see that my hip measurement isn't too bad. My spine is at 3.9. I am not comfortable with Prolia solution at age 67.

      If I had to do it over again, I would NOT go on Prolia under any circumstances. It is not a long term solution. It's not even a short term solution. Side effects are more common than they admit. It takes a hit on your teeth, no matter how good your mouth hygiene is. Perfectly healthy teeth will develop fractures, or just break and fall out. The longer you're on it, the lower your quality of bone will be, because it suppresses bone turnover. This is what eventually causes atypical femur fracture and necrosis of the jaw. I've been on it for 9 doses over 4.5 years, and now have the ominous task of discontinuing. My sister is in the same position. Both of us have had adverse effects. They'll never be able to remove this drug from the market, because that will send everyone into rebound.
      Note that when Prolia was released, they recommended only using it for a specified period of time. Then they figured out that those who went off it went into rebound that was sometimes catastrophic. Then suddenly, it was, "Oh, nevermind! You can stay on it forever!" This is what you get from Pharma. We are their Phase 4 clinical trial. You were smart to refuse it.

      Hi my mom as diagnosed with osteoporosis and I told her to take Bone maker supplements it helps increase osteoblast to help with bone strengthening.     this is so helpful. the synthetic ‘progestins’ were connected to slight increase in BC. I am taking HRT 100mg micronized progesterone (along with Estradial patch .05). My gynecologist supports my use of HRT for bone health. My understanding is the bio-identical progesterone I take does not carry the same risk of BC as the synthetic progestin used in the WHI study. Is that correct? I am 60 with no fam hx of BC.

      That’s what the current literature supports, yes. However, there have not been large studies like the WHI on micronized progesterone so we are a bit in the grey area. It’s great that your GYN supports your approach. That’s not common.

      Forteo will have more of an impact on P1NP and bone building. I always use these drugs in combination with a comprehensive program which would aim to reduce CTX.

    I was on the clinical trials for the development of Prolia. I was on it for several years and then it was released to market and of course could no longer get it due to the price.

    I'm 59 and recently had a dexa scan, showing osteopenia. I have a family history of so it wasn't a big surprise. I've been on HRT for years (transdermal estradiol patch) so it is encouraging to hear that it is benefiting my bones. Is there a transdermal estradiol patch dose that has been shown to be effective for bone loss? I'm on the .075 patch.

      My mom reversed her osteopenia and diabetes at 80 with diet snd excercise. Last test she didn't have osteopenia any more.

      Re the patch yes google it it has benifits for osteopenia. My aunt got her menopause at 35 and they didn't offer her anything and her osteo is so bad she's bene on every medication they all made her have serious pains and all sorts. Now she wasnt to get off prolia but it's a scary thing to come off. Get the diet sorted and excercise tai chi is amazing get your vitmin k2 45 mg not micrograms so 4500 micrograms 4 thousand micrograms I think the only one does it life extension and remember that magnesium should always be twice calcium intake 2 to 1 ... at least. And high phosphorous foods like milk are not good ratio of calcium to phosphorous so lots of vegetable calcium with magnesium to supplement because our food has very little. Trace elements like silica from horsetail tea and boron from figs and seaweed for iodine all help too. Best of luck. Try to do what you can my mom is fine now and my poor aunt who wouldn't listen to any advice except doctors drugs is in pain every day and like a little bird. But she won't listen so I stopped trying to help her and just glad my mom is so empowered to look after rherself and teach me to do the same. Bless you and hope you're OK.

      @user-EinsteinSarge there is not a specific dose that works for everyone. I’m an advocate for blood testing because there are levels that have been shown in blood to be a threshold for bone impact by estradiol.

      @horacethecheese thank you for your thoughts. Our literature review on these topics supports some but not all of your approach. You seem to be well educated in the space so perhaps you can help me to understand the phosphorus calcium ratio you mention. What is ideal and do you have evidence to support this claim? Secondly, the K2 dosing you mention is MK-4 correct? As MK-7 those would be very high doses and I haven’t seen anything in literature to support those numbers.

    I am 57 and just diagnosed with osteoporosis by fluke (-2.6 spine, -2.8 left hip). My doctor immediately wanted me to take the bisphosphonate drug Fosamax. When I tried to talk about whether it was best for me right now she got defensive and said that is why she was the professional to make these decisions. 😮
    So I am not taking it for a year while I work on exercising (just getting back to significant exercise after valve replacement open heart surgery) and overall healthy habits. In the past (before the weird heart stuff) I have been a fit, healthy, robust person. I am doing weights, jumps, vibration plate 1.5-2 hour hikes and lots of walking after retiring in June.
    I have three main questions:
    How much of an impact can surgery have on bone loss?
    How does inflammation factor into bone loss?
    Question 3 is a set: I know warfarin can affect Vit K - a. does it just affect K1 and blood clotting? b. Does it affect K2 and bone metabolism getting Calcium to the bone? c. Will taking K2 supplement help?
    I have signed up to take your Master course on Jan. 10th from Victoria BC Canada and am really looking forward to it. Yours is the only source I have found so far to be balanced information grounded in both science and common sense and considering personal differences. Thanks again!

      Hi there. Great job recognizing that there are alternatives to bisphosphonates. I'm sorry you experienced that defensive physician outcome of questioning drugs. It's common.
      Answers:
      Surgery can certainly impact an area of bone! It depends on the situation though.
      Inflammation will also certainly play a role. Reducing inflammation through diet is a big key for us.
      Warfarin will impact blood clotting but I don't think it changes bone health or K2 metabolism. I'm not a fan of this drug in general though and there are good alternatives for most people that are currently on it. I was trained to use it and most physicians I know who used it don't anymore.

      Dr Doug. Can you tell me if bone loss measurements between both hips should be the same? I saw a group of specialists on REM scan say they should be the same and and thats how you know the scan is accurate. Often thats how you tell if the if the Dexa is inaccurate. I am having a rem and dexa done next month to compare. Thanks

      @@LesleyMowat That's my understanding as well! Truth is we don't really know which is more accurate BUT osteoporosis is systemic disease so it makes sense that we would see the same bone density throughout the body rather than big discrepancies between areas.

    Looking forward to watching your new channel. Just subbed. I received my P1NP test last week and it's not good. The average is 20-100 ug/L and I am 19.9. It's below the average. At age 47, should I get a Dexa? There is no REMS here. My GP had advised me to go for Dexa only after menopause. I am seeking opinion of another doctor this week but wanted to hear your advice. Thank you Dr.

      @lebooshdiaries I am not a doctor, but I can tell you from my own experience, PLEASE get a DEXA asap. I am 58, just had my first DEXA and diagnosed with somewhat severe osteoporosis despite healthy eating lifestyle and regular exercise and BHRT. I am shocked and the worst part is I have NO idea why! If I had a baseline DEXA before menopause my doctor and I would have so much better information as to what caused this and therefore what treatment options might be best. Now it's just guessing. DEMAND your DEXA now when you can gain such valuable insight. They are easy, quick, painless and cheap. There is NO reason why every women isn't given one before she enters menopause. Yet they are often not ordered till age 65!! It's a huge failure of or system. Good luck.

      I paid $100 for dexa scan at 48. Showed osteopenia. So I’m on this confusing journey on how to deal with this without meds. I want to be on bhrt but I’m in perimenopause. I’m only offered birth control pills which I’m not taking due to clotting concerns. Plus I’m petite skinny small frame woman. Not sure how this factors into dexa results. I’m just taking my vit d3, mk7 and started weight bearing exercises. Up to 10 lbs for biceps curls and trying to go higher without injury. it’s 10% of my weight.

      Thank you everyone for your comments. DEXA is a no brainer @lebooshdiaries! P1NP of 19 is really low. We aim for 150 in our program if doing it naturally.

      @@Dr_DougLucas Yea, I know it's very low and I am concerned. I started lifting my dumbells although they are not very heavy, like 5.5lbs each since Aug and then did the blood test in Oct. I guess within 3 months, the osteoblasts didn't get triggered much. I didn't want to immediately start with super heavy weights because I haven't been exercising with weights for many years, so I wanted to start slowly so as not to injure myself. I also jump, hopscoth and do push ups. Hope to see better results in the next 6 months.

    Could you talk about TSH levels and osteoporosis? I wasn't born with a thyroid, and have taken thyroid supplementation for 76 years, mostly dessicated thyroid. My TSH levels are .006, but my FT4 and FT3 are in normal range and I feel like I'm at an appropriate thyroid dosage. My T- scores are -3.1 (s) and -3.5 (h). I wonder if my low TSH levels are a causal factor for my osteoporosis.

      Good question. TSH below 1 is associated with osteoporosis but hard to say at what point it’s causal. At 0.006 that’s pretty suppressed but if you don’t have a thyroid I would question if the feedback mechanism developed correctly either. We base treatment off of symptoms and thorough lab values. I would want to rule out of the causes before messing with management that seems to work.

    The second question you answered about the doctor only recommending weight bearing exercises up to ten pound, the first part of that individuals history about exercising for years and being shocked about the dx of osteoporosis, is something that really resonates with me. Thru testing and diet analysis, can you tell why someone has osteoporosis/osteopenia in the first place? I have been an avid brisk walker for decades. Truly perplexed.

      Genetics and family history can play a big part, as can body size, ethnicity, age, medications, hormone levels, smoking, alcohol use, activity level, and I am sure there are factors yet to be discovered. It is indeed perplexing. Some people say that taking too much calcium in diet or supplements can be a big factor, while others say the reverse.

      @salleone6387 and @lewynld thank you for your comments. There are indeed multiple risk factors as outlined but there are numerous that date back to childhood and adolescents too. The key to reversal for most is to figure out the root cause and address that!

    I am 74, had a T12 compression fracture 5 years ago, on prolia for 2 years, dexa improved from -4 to -2. Stopped the prolia and recent dexa got worst..now my endocrinologist prescribed Forteo. Been reading a lot about side effects and the inconvenience of daily self injection. I fear that with side effects it will affect my active life, traveling, going the gym, gardening. Been trying to get a consult with after watching a lot of your yt presentations. What's your thoughts on this. I haven't started the meds. I have a bad feeling about this Forteo injection.

      Sorry to hear of your issues. Seeing bone loss after discontinuing Prolia is common and the traditional medical model will generally resort to another medication as that is the tool of choice. The question I would want to answer why are you losing bone so rapidly? What is the underlying cause of bone loss? Forteo and Tymlos have the best mechanism for bone growth from a drug but it’s still a drug and if drugs can be avoided they should. The risks of side effect are low though.

    What Is the minimum recommended dose of estradiol patch that is best for osteoporosis? Is there an estradiol blood level that should be attained?

    Hi I really appreciate your videos, since being diagnosed recently, i have been on a path to find out why within 1.5 years i went from great density to basically looking like a mouse has been having my hips for a snack. I have been in constant pain in my hip and lower back bone pain.
    I saw a rheumatologist who ordered a bone density test and a 36 panel blood , it seem from the information , the root cause is hyperparathyroism, it makes sense that this is creating havic in my body over production of calcium and it’s taking it from my bones, what are your thoughts on this disease. I think finding root causes is should be top priority as well as rebuilding

    I have a question. I was just reading a post from a doctor who had lumbar surgery and was discharged with a bone stimulating device as part of his rehab. Have ultrasonic or electrical devices ever been tried for osteoporosis?

      I’ve never seen a study on this but consider what you’re asking it to do. Osteoporosis is a whole body bone metabolism issue. A bone stim placed in an isolate place won’t make a difference anywhere else.

    Do thyroid meds add or cause osteoporosis? What other meds contribute to osteoporosis or is it about lack of wt bearing exercise? Does coffee use lead us into bone loss?

      Over treatment with thyroid medications can cause bone loss. There is a long list of potential drugs that cause bone loss most common ones include PPIs, steroids, SSRIs and anti-convulsants

    Hi Dr, early November '23 I was put on Prolia with some devastating side affects. Mid December '23, I started my journey with Algaecal. I won't be getting the second shot of Prolia because of my experience with the side affects. I've heard about the rebound effect once off Prolia. My question is can fractures and breaks occur just after the first shot without relay drugs? I plan to continue with the Algaecal and when I regain my energy after this poison is out of my system I can start excercising again, which I miss. I'm 61yrs old.

    I'm 61 and have stage 4 breast cancer. I'm doing very well. I'm on Anastrozole and my doc says that leads to osteoporosis. I'm wondering if most of your advice would be helpful for someone in my situation. My DEXA scan scores are great. T score 0.2 and Z score 0.6. Thank you.

      Hello,
      Congratulations on your cancer survival. Anastrozole does lower estradiol which can increase bone loss. Our approach does work for people on estrogen blocking medication because there are numerous pathways to optimize bone metabolism. Estrogen is helpful but not required.

    Hi, thanks for the incredible info. I have same female 51 yrs old post menopausal case you mentioned with the additional complexity of Hipo Phosphatasia (seems genetic, getting tested). Knock on wood no fractures to date. My scores are really bad so I was prescribed with Forteo. I am as well concerned about what's next. On HRT w 0.025 Estradiol patch + IUD Progesterone as I have bad side effects from oral intake (no breast cancer in family so far). I am starting to train with weights (used to 25 yr ago) besides the regular exercises for this case. Appreciate any direction. Thanks. Lu. B.

    I’m 77, on alendronate for about a year. I am hoping that I can go off drug after two more years, but want that to be the end of drug therapy, I exercise, am not fragile, and am careful how I do things. Is it realistic for me to be drug free?

    I am 67 YRS old, Filipino, 92 pounds, 5 ft, diagnosed with rheumatoid arthritis and osteoporosis. Presently taking methotrexate weekly and methylprednisone 2mg daily. I was prescribed with osteoporotic drugs but I still have to decide on what kind of drug should I take. The doctor gave me 3 choices alendronate, estrogen or prolia. Aware of the side effects, I cannot decide. My t score on radius is _3.5, L1 L2 - 2.7, right femoral neck - 2.8. I have started a low carb high protein diet for 2 months now. Pls help me manage my osteoporosis.

    I took alendronate 75 mg for 13 months then stop coz i felt my condition worsens. When i start taking it, i had 2 lumbar fractures but after a yr i had t12 and l2 and 4. Fracture increased. Will my fracture still heal after stopping the medication?
    I do need an advice doc

    Hi! I’m 58 and just received results of my dexa scan. T-scores are Spine -2.1, Left Femur-3.0 and right femur is -2.4. I was told to take calcium and vitamin D and walk. That doesn’t seem like enough to me. Should I consult with another doctor? Very confusing. Thank you!

      Hi Fit 66 yr old. Diagnosed osteoporosis 6 years ago. -2.5 and now -2.6 in hip and -1.9 in vertebra. Taking lots of supplements and Osteostrong. Recently traumatic fall not fragile or balance. Accident related. Pelvic fractures non displaced and healing. Worried about my bone density decreasing while inactive but want to avoid bone drugs Also is osteogenic loading safe post fracture . Bone markers regularly are good.I want to avoid bone drugs and keep strength training with supplements and life style Any thoughts please

      @@user-gm7cg5md1k Hi there. Sorry to hear of your troubles. Pelvic fractures are slow to heal but do heal reliably. Osteogenic loading is safe after fractures are healed but ask your doctor who can review your imaging to verify that the fracture is healed.

    I’ve had one shot of prolia and was given the medical material AFTER I was given the shot so I decided not to get another shot. The doctors office who gave me the shot refuses to help me with coming off the shot or giving any more help. I have appointment with an endocrinologist but am not hopeful because I’ve already seen one and he did not want another’s doctors mess

      I’m a 67year old female that has osteoporosis. I was on Boniva for five years. The doctor never told me that I couldn’t be on Boniva longer than five years neither the bad side effects with jaw bone and the femur. I no longer take it on my decision. I lift heavy weights,squats,deadlifts,bench press etc. I also walk and do high intervals running. I have a very healthy diet and eat dairy. I took control on my body. Doctors advising to lift 10 pounds weights is ridiculous. I do though have experience on weights. I was a natural,novice body builder. The laters research has shown that lifting heavy weights safely can reverse osteoporosis or slow it down significantly. Aldo the best exercise is the squat. I feel so much better once I took control of my body. Do you own research and first of all safety.

      The question is not of benefit but of risk. There would likely be benefit but what is the risk at 20 years from menopause. It’s significant so that’s a difficult decision.

    I chose to take AlgaeCal for a source of calcium. At first I was pleased because my serum calcium was higher than it had been in a long time. Then I wasn't paying close attention and I had passed to hypercalcemia
    I need to stop taking algaecal. Anything else I need to do?

      It’s prob hard to fit alot of info into a short space of time. Try slowing the video speed by click/tapping on settings icon (looks like a thick star) in upper right of the video and choose PLAY BACK SPEED. You can also scroll back/rewind using the bar at the bottom of videos. : )

    Diagnosed a few months ago osteoporosis , 58 year old
    Live in the UK
    I don’t think there is any bone optimisation clinics in the uk ?
    Also I was on HRT Lenzetto so after watching your videos I asked the GP to do the recommended bloods to rule out other causes other than the assumption that it was menopause
    It turns out my estradial levels were really low
    So is there a preferential brand of HRT for bone health
    I am now on Èverol 50 patch as I wanted body identical and take Uterogesterone
    Also do I really have to avoid the bending forward ?
    Prior to diagnosis I was doing lots of dynamic yoga , strength training (although maybe weights haven’t been progressive enough ) and generally really very active so now I find I am really worried about doing a manoeuvre that could cause a vertèbre fracture
    My last question is
    About 18 months ago I decided to practice time restricted eating and become
    More plant based for heart health reasons , I lost around 8kg in a matter of weeks , this took me from 55kg down to 47kg but I have always been a small petite framed female 5”2 so actually this weight was a return to baseline when I was in my 20s and 30s do you think this could have triggered rapid bone loss ?
    I am now focusing on protein with some
    Meat but still like to stay with whole foods as much as possible I do eat dairy including eggs every day and Greek yoghurt so I think I am getting enough calcium , I certainly do not want to take calcium supplements because of the risk to the coronary artery’s ..I am taking vit d with k2 (m7) variety

      Hi there. So many big topics to consider! From an HRT perspective there is no one brand that we prefer. In fact we use compounded cream because most commercial products don’t provide adequate blood levels for bone health. Time restrictive feeding I find in general is not helpful for bone health. Our goals in this arena are to be anabolic and build muscle and bone. Doing that with TRF is possible but harder than without. Similarly, I love a Whole Foods approach to eating but think it should be protein forward based on animal products. The heart health benefits from eating a plant based diet are an illusion. Don’t fall for the mirage.

      ⁠@@chantage16no direct contraindication. Actually they both impact osteoclasts so probably would be synergistic but I’ve not seen them studied together.
