Great question! Steep age structure "pyramids" typically do mean population growth, but not if the death rate and infant mortality rate are SO HIGH that they more or less offset all of the births. We don't typically see this in the world anymore as all countries have entered some point in phase 2, but it is still represented in the DTM diagrams.

If you do happen to run into a 5-stage question, just remember that the population decline due to low birth rate that can begin to be seen in some stage 4 countries continues in stage 5.
