The sugar represents the simple pleasures in life, things that seem minimal, yet make up everything. Despite having a very black and white view on life, something as simple as tea and sugar was enough to make him realise the simple joys in life, joys which allow people to connect and form relationships [Thus, Russo opens a café and forms friendships with his costumers]. It's also really well fit into the narrative, relating to other characters (more specifically, kinderheim subjects). That's why they all have a desire that they hold in significant value even years later, joys in life that prevent them from fully becoming monsters.
