    Hi guys. I always come back to this video. It would be great to see how Olivia finished her treatment, possibly with before and after facial pictures, skeletal images if possible, post braces? All that good stuff. Thank you.

    Thank you so much for making this video!! It’s exactly what I was looking for. I’m an adult and have read a lot about the MSE I have heard a lot about it from my healthcare providers, but it has been hard to find testimonials from adults in the form of video and otherwise. I found this way more helpful than just reading about it. I had my MSE installed two days ago and this video gave me a lot of comfort. I have a better idea of what to expect now and feel relieved that what I’m experiencing is normal.

      My doctor asked me to turn it four times a day for the first four days.
      How are you feeling? Are you in any pain?
      I found it pretty difficult to eat for the first three days. I mostly had smoothies and soups because of this.
      It was hard to sleep as well because of pain and discomfort in my mouth. I was taking Tylenol regularly throughout the day and night and that did help. Now that I’m past that part, I’m not in pain anymore.
      It’s definitely an odd sensation making the turns with the tiny wrench lol. I’m starting to get used to it, but there’s still a little bit of worry each time I do it.

      Happy to report that my MSE is out. Getting it out was a breeze and didn’t hurt like I thought it would! The small holes where it was anchored to my palate healed within about a week.
      While I had the MSE installed, the gap in my teeth closed on its own not too long after it opened up.
      I’ve had Invisalign for 12 weeks now. My top and bottom jaw almost line up again. Chewing has been difficult because of the misalignment of my teeth, but it’s improving.
      I can now breathe way better through my nose and there’s much more room in my mouth for my tongue, which feels much more comfortable than before the expander and Invisalign.

    I got my MSE done today. The entire procedure was alright. Basically because they numb your mouth during the insertion of the screws and expander. During that period, there will be an urge to sneeze as the screw is being drilled into the upper palate bone of your mouth, which is also the lower floor of the nose. It's all fine.
    However, after one hour, with the numbness gone and the bone moving, I started to feel a slight tingling sensation throughout my mouth and midline of my nose, which later on escalated to an urge to itch my teeth, which again changed to intense pain and pressure. It's too painful that I feel like removing it to relieve it. Took a painkiller, hope it helps. Just know that you are hearing this from someone who has a good pain tolerance.
    Apart from this, speaking will be a bit slippery for a while but it can be managed. But swallowing the food will be tricky.

      @@sunflowerstudies8719 hello there. Yeah, unfortunately in my case it didn't work well, so I removed the mse during the 4th day after its placement. I can't express how much painful it was. Eating and drinking was not just difficult but severely painful. Basically because one the screwed area in my upper palette got infected, which began to spread the inflammation, making it more harder to cope up. My acidity heightened, so did my allergies. So without any option, I took it off.
      My doctor tried to persuade me, but I was adamant to remove it. Because, pain after the procedure is common. But if the pain persist after 38 hours, it's not common. This was my personal experience. I had high expectation from mse. I know it worked brilliantly for others. But unfortunately my body reacted differently to it. My doc told to place the removed mse after a couple of months, but I think I'm not gonna go through it again.

      People that claim to have a high pain tolerance never do in reality. They just think they do. Just ask any nurse, doctor, dentist, tattoo artist, PA that you don't have a relationship with so they tell you the truth.

      @@robertmabry6998 You could be right. I don't have a say in that as Idk about those "people". All I know is about myself. Having experience from prior surgeries, plus as an ulcerative colitis patient, I could certainly say I did manage them all with complete composure, like, I was one among the easiest patient to my doctor or so is what I remember him saying. Perhaps, that confident in my pain tolerance lead me to mse too. However, as this procedure took inside my face, I understood all body parts have different level of pain tolerance. Anything inside skull is a level up pain.
      Yet, I repeat that MSE is a safe procedure, quiet effective too. Unfortunately it didn't work for me, that's all

    It would be cool if they invented a way to just be able to swap out the central screw (if the limit is reached on that screw, yet more expansion is necessary). This would allow you to gain more expansion, while avoiding the need for another surgery to install a larger expansion device.
    Maybe like a cam shaft on a car. Just have caps with screws of their own, to hold the main screw in-place

      I thought it was left in place to maintain the expanded palate while the braces straighten her teeth, not to expand further.

    In the mid 90’s I had 4 adult teeth removed as a child “to make room” when for braces. I fully believe this narrowed my jaw, increased my pallet height (very high pallet), and caused very serious nasal breathing issues. I’m fit and healthy but have sleep apnea and often wake up exhausted.
    I’ve read this treatment can help correct the jaw and pallet, and subsequently the nasal cavity. Is this true?
    Lastly, with this treatment is it possible to have 4 teeth implanted where my adult teeth were removed?
    I’m desperate to breathe better and I don’t want nasal surgery as that often reverts and does not work. I’d like to have my natural jawline.
    On a side note, my older brothers never had braces or teeth removed. They don’t have the issues I have.

    I haven't seen a 'Before' photo of Olivia, but I suspect this procedure impacted her nose aesthetically. Her nose and nostrils are a bit wide and nose is slightly upturned, which can happen when the maxilla expands like that. I'd be curious see what it looked like before.

    Glad she got help! There has been a lot of success with the DNA appliance (s). No surgery, widens upper arch forward and wider, lower wider. Corrects a number of tooth, bite, jaw and face abnormalities. Surgeons don't get $ tho and if they tell you adult palates are all big now and can't move without having bones cracked and screwed they are 100% full if it. Do your research, healthy life is worth it!

    I don't know if the technique is practiced in certain countries but i have not come across any orthodontist willing to do this, they all seem to say the same thing. "its not possible in adults" "surgery". Any recommendations in Toronto, Canada. Asking for HELP!

    So, now that you've widened her upper palate over 1 cm, her lower palatal teeth aren't occluding with the upper any longer. How will you now expand her lower jaw? This makes no sense.

    I've read that this is usually used for children, since their sutures haven't closed yet, but its possible to do it on young adults. I would like this, but I'm afraid I passed that age mark.

      It isn't true. It is easier to do with younger people, especially female, but still doable with adults. Many adults do this and have great results. Sometimes, when the bone is too hard, it won't work, but it really depend on a individual.

    How will the top teeth bite properly with the bottom ones?
    Also, what’s the infection risk of having a foreign body in the tissue ?

      Usually the bottom teeth are orthodontically tipped outwards to meet the top teeth which may be tipped inwards, there are ways to expand the mandible but they're a lot more invasive than MSE - things like cutting the bone straight down the middle and using a distractor. I'm unsure about infections.

    I have seen some amazing cases by you where you got expansion through Invisalign ect. how does one know whether they need which treatment? what mm palate do you start recommending this type of expansion? and does tilted teeth have a play in it at all ect....
    how does the recession look after treatment? does it get better or stay the same ..
    also a big question how do providers control vertical growth with this and any expansion?
    does expansion help get rid of gummyness.... the palate should shorten - so lift upandout towards the nasal cavity correct? so in turn that gets rid of gummyness due to narrowing or crooked teeth ect

    Is there any way to connect for guidance? I had orthodonture as a kid, I think the wrong things were done (headgear), I'm not trying to cause problems but rather seek a solution. I went to ask questions about what to do and I was treated like an internet-obsessed crazy person. My tongue points to my left in my mouth and folds down because there isn't enough room. My centerline is off. My left side is rounded out more than the right, leaving a bigger gap than the other side. It affects my breathing. It causes pain in my masseter and shoulders. But when I went to ask questions, I was told that I'm getting pain from the jaw hinge and I just need retainers, and that's not it. My lower jaw presses forward on my upper jaw and it flattens my front bottom teeth, so unless that's addressed, new orthodonture seems pointless. I just am so discouraged because I can't be taken seriously.

    What are the views on surgically assisted MSE, i.e. SAMARPE? It's similar to SARPE but much shorter and the idea is that by opening the suture before fitting the bone-borne MSE appliance, you guarantee expansion.

      @@kanzaahmad5746 The surgical cut from the surgically assisted MSE prevents any expansion from happening in areas of your head above the surgical cut, including your eye-area or zygomas. Some people think that zygomatic expansion is aesthetic. But, I don't think that's always so clear.

    I got MARPE 3 days ago and I’m almost dying in pain. Not even opiates are working. I’m going back to my dentist because he said this isn’t normal... these screws in the roof of my mouth are terrible

    So the palate isnt cut at all? So no surgery like sarme? How does the suture come apart? Does this work for older people in their 50s. I have a very small palate and was told i would need sarme but came across this technique and this sounds so much better. How is the gap closed. I always wondered this

      Take this comment with a grain of salt but I heard the palate is already split in two. Everyone's is. So they just pull it apart and the body responds to it by growing my more bone tissue in order to close the gap.
      But again, this is just something I have read some time ago.

    I’ll be honest, ripping my jaw open in the middle would make me feel anxious. Can’t you apply enough force for it to simply start building in between? So it will expand slowly?
    I’d love for my wisdom teeth to actually fit and improve my very bad breathing, but this seems quite risky to me.

    Hello Dr, was just curious if you guys (including the patient) noticed a change in malar prominence. She had the same crossbite as I do and I have noticed a lack of anterior cheekbone projection is quite common. The only other 12mm expansion I found showed a pretty large change in malar projection so I was wondering if you guys noticed it in this case as well.

      we do see exactly that, which is beneficial for most patients who have midface hypoplasia...Dr Won Moon has documented these malar changes with 3D superimpositions on his cases...please look at his papers as referenced in the other section of this post....

    Does it work on a person who is 30 years old? The jaw bone's hardened.
    And how will you fix the the alignment of upper teeth on lower teeth? Will the mandible (jaw) need to be expanded as well?

    I've been suggested the surgery method. Im 36 with a cross bite and an open front bite of about 4mm, though just 3 years ago I had no cross bite and no open bite! It happened all very quickly and no orthodonist can really tell me why. It's so frustrating. I've never needed orthodontic treatment, I've seen 12 orthodonist and one Tmj specialist.
    I wonder why my orthodonist hasn't offered this method shown 🤔 seems much more appealing.

    How about the nasal width? How wide should it be, because sometimes only the dental arch can be narrow and not the nasal width.

      Mse usually affects nasal width later on. It widens.

    Hi guys, I need second opinion. I have class II problem, 10mm overjet, and also overbites with convex face profile, 27 years old. I decided to have treatment of damon braces recently. My ortho recommended me for both 1st premolar extraction on my upper jaw for retraction space. unfortunately for me that I find about all of this information (risk for sleep apnea,etc) after I wear the braces. I have discussed this problem with my doctor, but He didn't see another solution outside of extractions as my by overjet and the convex profile that I have, he saw it as match to go for extractions. He also told me about upper jaw diztalation but He thought that I didn't need it as the solution requires for my upper jaw to be moved backward hence the same thing as extraction (reducing he thought).
    so now I'm in dilemma of whether to go for extraction or what, because this also cost me financially. any suggestions?

    So, if I am 47 years old, can I also go for MSE device, instead of the Le Fort 1 surgery? (I also have a front tooth ankylosis to the jaw); am I out of luck and the only option is surgery?

      @@ElliotWilks_ no i live in California. I went to Dr. Brian Hockel in walnut creek. It was acutely 12k but you are also told to get xrqys cleaning which would be 13k. I recommend going to the orthotropics website to find the nearest orthotropic doctor

    Hi Dr. I've always been wanting to know an orthodontist. I had my teeth straightened out with braces. Is it possible to have my upper teeth look somewhat like Avril Lavigne's if I want my snuggle teeth back? I just prefer my smile with slight vampire teeth rather than straight teeth. With straight teeth, I have a narrower, sad-looking smile. Thanks

    i have overjet in my upper teeth (4 incissors , the molars and canines are straight) decent amout and a bit in my lower which will be fixed by a bit of ipr , now the question is if i get a palatal expander will it work on my over jet? iam 18 and my ortho said extractons but i disagreed so he suggested me this method to bring my teeth back , please let me know if anyone has any ideas on this

      Tha face itself doesn’t expand, only the midface widens a bit

      I’m getting my palate expanded soon, i think it will make cheek bones more prominent. But i dont know if 2mm upper arch expansion is going to be significant

    Dr can you do this technique on adults (40 year old) and what about if you dont have strong molars (fragile molars with fillings for ex)

      the molars are not used for anchorage...this is why the MSE is indicated for adults..less force on the teeth as the force is transferred directly to the bone

      @@LordGodKing several complications, mainly significant damage to my nose from the MSE treatment. My breathing got worse as a result. Will need to get a surgery to correct the damage.

    Hello,
    Is it common for patients to experience numbness/tingling on the temples, under and around the eyes and nostrils/mustache area? I am currently undergoing MSE treatment as an adult male, my mid palatal suture split around turn 22 and diastema has been increasing everyday as well as my nasal breathing. As of today I am on turn 42/60 and experiencing this numbness? Is this common? Should I be worried?

      @@TheCrossfitter 4 times a day, which I’ve recently learned is a lot. 28 turns completed over 7 days now and still no gap :( I called my ortho and he said that pain is normal but I think now I’m just turning it too much. I’ve decided to take a few days off since I have swelling around one of the screws and then start up again with just 1 turn a day to be safe. It just sucks that there’s not a lot of info online about this and my ortho doesn’t seem worried but from what I’ve read this doesn’t seem good

      @@noellelewis4352 I think you’re correct to take a break from turning. Let your body relax for 4-7 days. Focus on meditation, tongue posture, and getting enough sleep. Visualize your mid-palatal suture splitting evenly and for optimal health. After some time off, evaluate how you’re feeling and then turn once/day if you can tolerate until split happens. After split I would drop down to one turn every/other day or every 2-3 days. No need to rush the process. Keep an eye on asymmetry. I split around 22 turns, day 16 or so for reference but everyone is different. How experienced is your MSE provider? I would refer you to Jaw Hacks channel here on YT and check out some of his video they’re very informative. You’re not alone in this. Have faith. I would also see if you can get a CBCT done to see what the MSE is looking like in your maxilla thus far. How is your diet? Fruits? Vegetables? Meats? Nuts? Seeds? I know that it can be a pain in the ass to chew/swallow with MSE so be mindful. Trust the process. Are you getting all of the vitamins and minerals you need for healthy bone growth and nervous system health? Probiotic? Turmeric? Fish oil? Hydration? Are you doing FaceMask as well? It’s possible that with all of the turns you’ve been doing plus FaceMask that your body is under a tremendous amount of stress and just needs some R&R to recalibrate. As always, please consult your orthodontist first. I am in no way a medical doctor. I am just speaking from personal experience. Stay positive and keep me posted 🤙🏻

    dr. Mahony, does this MARPE also widens the lower jaw at the same time with the upper jaw? I have a narrow palate and mild crowding on lower jaw, i wish i could correct them simultaneously using MARPE.

      Answers no. Majority of the time bottom teeth are tilted inwards therefore no need to expand lower jaw. Further theres no way to safely expand the lower jaw.

      Not quite. With SARPE you’re cutting the maxilla from the cheek bones so only the base of the nasal cavity expands.

      ​@@123456789047745 That's it. Also if you are young enough to still be growing the thought is that you have a greater opportunity to impact bone positioning.
      If you don't have space for your tongue though MSE type expansion makes a lot of sense to allow the tongue to doing it's job.

He even collaborated with Mike Mew in a couple of videos. This girl prolly already mastered the art of mewing before her treatment began.

      Mewing can not grow the bones or move the teeth. So if the bones and teeth are making it hard to breathe or eat they have to push all of it out of the way to help you.
