    This Doctor Reacts video is me, as a psychiatrist, analyzing another episode of It's Always Sunny in Philadelphia. I'm watching when Sweet Dee has a heart attack but more interesting is Charlie's psychosis and Frank's trip on anti-anxiety pills that has him reliving his early career in One Flew Over the Cuckoo's Nest. We get to talk about mental health topics like anxiety treatment, benzodiazepines and Xanax, asylums and psychiatric hospitals, dissociation, psychosis, sleep deprivation and more.

    I'm early enough to debunk the myth/theory that Charlie misread "Pennsylvania" as "Pepe Silvia" and "Care of HR" as "Carol from HR" -- the writers have said it's not true and Mac clearly says "ALL of these people exist." Thank you for letting me be of service today.

      And I'm early enough to add that in Rob McElhenney's (I probably mangled that spelling but I'm too lazy to google it right now) other show "Mythic Quest", there is a recurring character named "Carol" who works in HR

      Yeah that's true, but I think the theory is a more clever reason than 'Charlie is just wrong'. If his illiteracy made his see Pennsylvania as Pepe Silvia...then of course Pepe gets a lot of mail. Same with Carol HR.

      @@Ocrilat I actually really like it as is -- he's done this whole conspiracy and gone down this manic rabbit hole when the person just very much exists. There have been enough great jokes about Charlie's illiteracy, good to see good old mania get some time in the spotlight.

    Dee wasn’t too young for a heart attack. Deep commitment to ‘averages’ can be dangerous in health care. I had a heart attack at 41 and practically had to jump on the ER doctors to test me, because I was outside of the ‘range’. Then, when the tests came back positive, they ran around in a panic because they’d been so slack, up until then.

      yes! heart attacks can happen at ANY age, its just more likely when youre older. but that doesnt mean they dont happen

      you're not wrong at all, but i personally didn't really interpret his statement as saying its impossible for younger people to have a heart attack, just that those symptoms in a younger person are far more likely to be anxiety related. obv dr elliott is a psychiatrist and his channel revolves around mental health/ illness so he's always going to talk about the psychiatric angle. most doctors will do an ekg on anyone with severe sob/chest pain/tightness but it is a problem that many still will dismiss it because of age, like what happened to you. which is really so stupid because an ekg is about as quick and painless as any medical test you can get. i'm sorry that happened to you though, and really glad you advocated for yourself and were eventually able to get the proper care. we shouldn't have to but unfortunately it happens incredibly frequently.

      @@charlief3169 - Fair enough, but I’ll add that I didn’t have classic symptoms (it felt like back pressure) which only appear in a minority of cases. And, while they did the EKG, it was the potassium levels that got them running.

      This is admittedly judgemental, but I associate very thin people with heart failure in their 20's. Heart attack is tricky in women because uncommon symptoms are more common than common symptoms: many will mistake it for menstrual cramps and pop an ibuprofen to go about their normal day.

      I had something similar. They were very annoyed looking at my “dramatics” because I was so weak, I requested a wheelchair. I was 30, but I truly couldn’t walk which made them eye roll at each other.
      Then as I’m on the ER bed I remember the nurse looking at something, and said in a disbelief/panicked way, “Oh my god.” Then RAN out.
      Soon after she rushed back in with 5 or 6 other people all working on me in a frenzy with each doing something like getting me on 2 different IV’s in this tiny ER curtained area, because I was basically *dying* in front of them.
      Had to have open heart surgery.
      If I didn’t have that surgery, I would’ve been dead in a week. At least that’s what the surgeon told me after the surgery.
      But whatever I mean shit happens but still, I don’t like being treated like a liar or drama queen like *that.*

      another fun fact that i was surprised he didn't mention is that danny devito was in one flew over the cuckoo's nest, in fact it was his first film role. the man playing cards saying "hit me" in the psych ward when he walks in his a direct nod to the character danny played in the movie.

    Context for the botox comment:
    It's injecting an actual neurotoxin as to cause paralysis and give your skin that "tight" look. Now imagine if the needle goes in the wrong place.
    The fact people actually get this treatment is MAD.

      Also the toxin in question is botulinum toxin - the toxin responsible for botulism.

      I got botox injected in my calves when I was 12 to try to stretch out my Achilles tendon (I have very mild cerebral palsy). It kinda worked but the nurse missed the numbing cream the first time and it’s the worst pain I’ve ever experienced. 🙃

      @@crptpyr Yes, and the bacteria producing it are some species of Clostridium (C. potulinum being the most common and well known one). They live in the soil and are the ones the standards for how canned food needs to be treated to be safe were defined after. Doing it not quite good enough and have some of them survive caused many deaths.
      Also, producing the toxin (in anaerobic, meaning no oxygen) builds up gas. That's why you should never never ever consume any food from a curved can.

      I have hyperhydroxis (excessive sweating) and i got botox to reduce the amount of sweating on my back, my feet and my armpits

      @@reeeyou I have severe anxiety and while I am on meds that have drastically helped me just with sweating (a welcomed side effect), all my sweatiness is in my hands and last time I checked (many years ago) you can’t use Botox to fix it. :/

    Very glad you brought up the have that "full body scans" do more harm than good. I work in radiology but yet somehow people never believe me when I express this opinion. Not to mention the unnecessary radiation.

    People seem to be missing (at least I haven't seen anyone talking about it nor was it talked about in the video ) that Danny Devito was in One Flew Over the Cuckoo's nest, it was actually his first film role. In this episode of Always Sunny, the man playing cards when Frank first enters the psych ward who keeps repeating "hit me" while smiling is a direct nod to the character Danny Devito played in the film.

    I have bipolar and sumbitted myself to psych ward during depressive episode that came after let's say, pretty ridiculous episode of mania. Poland still uses these dorms with no doors. First day and I woke up to the patient searching me for smokes at about 5 AM. That patient was not even from this dorm.

    I've gone through sleep deprivation and i heard so many weird things that weren't there like conversations and saw figures on my curtains. Scary stuff, but yeah it was very ambiguous but it played with my mind.

    Would you consider reacting to the Scrubs episode "My Fault" - Season 3, Episode 20? It's about *full body scans* and came out 20 years ago and says basically what you pointed out. Also, it's hilarious watching Dr. Cox and Dr. Kelso fight. Anyway, love your videos, Doc!

    I've had to be admitted to psych facilities 3 times in my life, and the only time I wasn't sharing a room with at least one other person was when I was underage and sent to a teen ward. You're very correct in saying that it's terrible for recovery. I didn't have a moment to myself, was being stared at constantly, and was only sleeping from exhaustion because I didn't feel safe enough to rest. They were awful experiences and really soured me on going to the hospital for mental health crises. Both times I pretty much had to grit my teeth and pretend to be recovering so that I could be released and sent home to actually rest and recover.

    A recommendation for future reactions is the opening monologues to the second season of the TV series Legion. There is a compilation video of them all on here titled "Legion Season 2- Every Jon Hamm Narration" and it's 19 mins long. I loved the series when it was on and if you haven't seen it I think it's something you might find interesting (either for your channel or just personally). I'd love to hear your thoughts on it and see how accurate the mental health aspects actually are.

      Honestly, I'd love his perspective on Legion as a whole, there'd be a lot to talk about with all three seasons! Love that show, so glad you brought it up too!
      Edit: By as a whole, I just mean I think there are several episodes that could be fodder for good discussion. The third to last episode of season 1 in particular could be great to talk about inpatient care, various diagnoses, and even a little med talk. I know the whole series would take too much time

    "Paranoia and anxiety can be contagious": I'm glad you mentioned this.
    Is it possible in a group of children to have some kind of "folie à 'dozens'", mass hysteria, shared delusion or anything the like to show. I ask because this may have happened to me.
    Long story short (I'll give my best): when I was 9, staying in the childs ward of a clinic specalized in allergic and chronic diseases. It was hard, for many reasons (away from my parents for the first time in my life, being bullied, having to stay over christmas all of a sudden of shich possibility neitherb of us had a clue,...). There must have been around 30 kids in that ward, ages 7-11 or so. At one point, someone had brought a magazine with an article on aliens in it. Don't remember more about it the image next to it, showing a beige humanoid "typical" alien. Some kids found it scary (me included), had bad dreams, spoke about these dreams, fueling the fear, leading to more and more kids having nightmares. And scary stories and drawings decorated the wild imagination of chidren. We really winded each other up. It was a big deal. Many kids (again, me included) really and genuinely believed that there was a group of aliens nearby, terrorizing our ward. We were freaking out. The educators had a real hard time and seemed at their wit's end.
    I still was convinved to be threatened by these aliens for quite some time after having returned home. I'm over that, but to this day (I'm 37n now) seing an image of an alien similar to that one back then gives me nightmares. It's gotten a little better over the years, but sometimes these figures may still haunt my dreams. Embarassing I guess, but that's just how it is.
    Since my parents just sitted it out, I keep wondering what might have happened there in psychological terms. Is sth. like that common, how would it be called. I'm just really curious.
    And sorry for digressing I guess.
    Thanks for your great content. I enjoy it a lot. Not only being a patient with sever comorbid menatl health issues myself, I feel your fascination for the human mind. It's intriguing.

      Also although there are many fantastic things about the Australian healthcare system, we still have (at least in my area) primarily dorm rooms with the occasional single or double, the rooms don’t have a guard in them like that though, however I wish they did.

      Yeah. Around 15 years ago, I had a, shall we say, major depressive episode and they threw me in the drunk tank. I was not drunk or on any drug other than tylenol.

    As you mentioned the hospital wards in this, have you considered watching everything calls for salvation which is set in an Italian psychiatric ward following a man who was involuntarily committed following an altercation by his family. Really thought provoking series in my opinion would like to know your opinion on the show. Apparently season 2 is coming out soon so would be timely

    President Kennedy was pivotal to community mental healthcare vs institutionalization in the 1960s. His sister was a victim of the barbaric practice of lobotomies and that changed his thinking of the whole concept.

    When I was diagnosed with GAD at uni, the first prescription was fluoxetine. A few months later, I had to move back home, and my new therapist was very anti-med and weaned me off it. Haven't had any meds since, still struggle with GAD/depression (that therapist was awful, I ended up switching to someone else shortly after -- who eventually fired me when I wasn't able to pay her full price -- but by then lots of insurance problems meant I couldn't get the meds anymore). I don't even know if the meds would have helped, since it was such a short duration I was on them, and I was going through a lot at the time so it's hard to tell. But oh well, I've just learned to live with it and to accept that I will always be struggling to remain somewhat functional 🤷‍♂

    Unrelated to any reation video: do you have any suggestions for information on accommodating mental health in the built environment? I know some places put up nets and signs with crisis hotlines to deter suicide attempts, and that many combat veterans feel safer if they always have line of sight to a point of egress. As an engineer, I just have to guess I have no positive or negative impacts beyond these observations.

    Well, Doc, I got to tell you I took SSRIs and SNRIs four years and have probably taken every single one there is available. Not a single one helped I suffered through up to sometimes five panic attacks a day. I’m diagnosed with PTSD and panic disorder. I got on Klonopin 2 mg twice a day and I am actually living a wonderful life, and I will stay on them as long as they are willing to prescribe them to me because the alternative is literal hell on earth so maybe you should not give advice over the Internet and let people discuss their history and symptoms with their psychiatrist

    Aside from obvious things like being harmful to the individual or those around them, where does one draw the line between “flight of ideas” as a symptom of an underlying issue versus “flight of ideas” as a sign of ingenuity? Is it based on others’ perception of your contributions to society (i.e., the idea that those regarded by contemporary society as geniuses won’t be diagnosed because that would “tarnish” the image of the person, whereas a “nobody” with the same behaviors would be admitted to a psychiatric hospital in a heartbeat)?

    The rehab i work at has a medline, and its even more protected than this one. glass walls and tiny holes to fold and slide paperwork through. The patients still cheek their medication so they can take the sleep meds later on.

    one hospital where i use to live, when I would leave there after being admitted from the emergency room. i would have hallucinations. then dream while being mostly awake. object would turn into things. it was terrible. I'd even see old glow in the dark stickers (that didnt have the glow in them) glow in a pitch black room.

      70 billion just this year alone, you know how many American could get help for free if we use that 70 billion on our own citizens instead of protecting everyone?     Back when we were both in our 30s my friends arm was hurting and he thought he was having a heart attack. Turns out no, ER thinks maybe a stroke (which leads to my fave joke: "what caused the stroke?" "The coxswain." Feel free to use that.) Turns out he had a herniated disk in his back (I think c4 maybe?) Linked to his arm that caused the pain and that triggered a panic attack that just reinforced his idea he was having a heart attack. His neurologist was amazing but it was a scary time. At least he got my amazingly funny coxswain joke out of the deal. Even though no one got it.

    i almost hit the floor when he started to take pills with beer.. great combo, certanly will help him to be admitted either to regular hospital or mental hospital once when the combo kicks in... or maybe even to mortuary..

      @@IceMetalPunk Maybe, you get calls from collection agencies. Hospitals can't refuse to see you though, so you can always get treatment.

    While u do go into a lot of debt getting health care, doctors don't just kick ppl out of the ER like that. They still treat you in an emergency and just send you a bill later.

    The doctor normally isn’t the one who discussed your payment lmao and I was just in the hospital for about a day and a half? Still waiting on the bill but I have an insurance, and we’re estimating around 100k? I didn’t need an ambulance, I didn’t have any surgery or even procedure really, I was just given blood thinners which I still take in pill form at home, and got tests done I’m sure that’s where most of the cost is coming from. But when I was in the first room before you get your actual hospital room (IF you can) there were so many people just laying on little cots in the hallway

      you're paying 100k for a 1 day hospital stay where you had no surgeries or procedures? and you have insurance? there is absolutely no chance that's correct unless you had like 20 mri's in a day and a half which i'm guessing you didn't.

    One of the things that always sunny seems to highlight is that mental illness is more common than most people appreciate. Of course mental illness doesn't always present itself at crisis levels, but it's still there. Waiting for a triggering event to manifest and even bloom.

    I unfortunately have to take Alprazolam. Rather mild dosage, but it still worries me.
    Also last november I had to be hospitalized for how bad I was doing with all my mental health issues. The doctors made a mistake in admitting me. My dosage at that point was 0.25, twice a day. From the ER to the psych ward, somehow the point moved one place. They gave me 2.5 at the unit...
    I was completely knocked out for 20 hours or more, woke up with the lower part of my hospital my bed high up plus a large pillow under my feed (I already have low blood pressure issues). I also had a big bump on my forehead. I vagely remember getting up for the bathroom during the night and falling. For the rest, no memory whatsoever. A whole day just erased.
    I can laugh at it in hindsight, but this was quite the mistake to make from the professionals.

    I have major depression, severe anxiety, and complex PTSD. They tried a ton of SSRIs, but they all made me want to unalive myself. Then they tried thorazine, which was a fucking NIGHTMARE, considering I don't need an antipsychotic or anything like that lol. I did all of the different kinds of therapy and was hospitalized multiple times, and nothing worked, and they were about to try ECT for my anxiety and depression. Thankfully avoided that shit. So now I am, unfortunately, a chronic user of benzos (prescribed). All of my doctors and psychiatrists hate it, but they can't fix it with anything else. I do have to try to take it as little as possible because it does make the anxiety worse if you aren't careful. But even taking it only a couple times a month and just white knuckling through most of my anxiety attacks has made my tolerance insanely high.

    Heart disease is the leading cause of death in Pennsylvania. The second leading cause of death is cancer.- It’s also a heavy smoke state a heavy drinking state, despite the changes to the laws regarding age limit for smoking/in car smoking/ or the fact it’s technically a dry state. It had one of the highest smoker rates per state population… it’s also very polluted which may be a factor for the cancer risk.
    when I was a kid streets just smelled like cigarettes and it was common to see almost everyone smoking. I’ve known so many people who died before 18 due to cancer.
    All the males in my family have died from massive sudden heart attacks at the age of 50 to 55 and the only one that hasn’t had a minor heart attack in his mid thirties to forties and was placed on medication. My brother in his 20s is already considered high risk and the doctors were horrified at his test results. Their hearts are physically too big for their bodies so they just drop deal. All the women of both my families have had cancer. It’s shitty that if you live there long enough you’re probably gonna get a heart attack or cancer but that’s usually how it goes.
    While ya it’s mostly men past 45 and women after menopause - that’s not the case for everyone, you’re not too young to have a heart attack you can have them young it’s just less common.
    Pa has a lot of people with illnesses or genetic conditions - the problem is how far hospitals are located and how bad insurance is to the point people can’t go to a hospital unless they drive honest to god hours to get to one that takes their insurance - or an ambulance can’t physically make it to their location. It can take 45 minutes for an ambulance to get to a house then 45 minutes for them to get back to the hospital where they’re pronounced dead - it sucks.

    7:14 I think with cigarettes, it goes beyond the effects of nicotine. The tobacco itself is an MAOI. There is a very different effect that happens between smoking a cigarette versus only using nicotine (whether by patch, gum or vaping).
    If it was just about the nicotine, it'd be easy for smokers to switch over to a much less harmful way of intake. You'd mostly be dealing with the psychological factors, like knowing what to do with your hands. However, given that cigarettes have an anti-depressant and even anti-psychotic effect, it makes it much more difficult to completely and permanently stay off.
    This is why no one should ever pick up even one. Y'all don't want your brain wiring around it. Trust me.

    Hate to say it but in today's day and age many of us are turning away from the thought of psychology and therapy because of the extreme ideologies they appear to be pushing into days modern age of gender identity...
    It's not our fault for being unwilling to change. We are simply just looking at the world through a traditional lens
    Ie in a way that has been proven to work in the past....
    And whether that be the fault of psychiatrist, therapist, or public perception based off of news, is not our fault...
    Which means it's up to therapist in psychologist to either
    A- prove that new ideologies revolving around the concept are either true...
    B- stand up for psychology and therapy by separating oneself from the modern gender identity movement...
    C- or better yet, just take a completely neutral stance in the matter rather than picking one side over the other...
    Not trying to turn the entire conversation to this modern political lens, but it doesn't change the fact it's making a huge impact on the perception of traditional people with psychology and or therapy...
    Therapists truly are that smart they should realize that this is a legitimate perspective held by traditional individuals whether they agree with it or not
    And therefore should adjust their tactics taking that into mind...
    And I am currently referring from the Americans perspective in America...
    If it is different elsewhere so be it.

    So it's an incredibly dark movie but what are the chances of reacting to the storylines depicted in Requiem for a Dream?
    Particularily Ellen Burnstyn's storyline, which ends in psychosis and a brutal session of Electro Shock Therapy. I still can't rewatch that storyline because it just breaks my heart so badly.
    But there is value in discussing what happens in that film. I would love to know how different things would go today. Especially what you think as a Forensic Psychiatrist.
    Understandable if you don't want to have anything to do with that movie though. It's very overhyped and cinematically dramatized.

      @@DoctorElliottCarthy We don't like it either. Unfortunately, our government is run by millionaires who insist that they would need an entire system of bribery and money laundering or they would starve. And they made Insider Trading Fraud completely legal for just themselves. It's true.
      The US government absolutely does not act in the best interest or even the wishes of the American People. We don't even have discussions on invading countries anymore, our tax system is a complete mess, you know our Healthcare system, and a majority of people who identify as Conservative (and these guys are ALL to the Right of the BNP on EVERYTHING) want Single Payer Healthcare, Free Tuition for College, Food fit to eat and Water clean to drink, and heavy Gun Control regulations. An Assault Weapons Ban is supported by 90% of the US. Will we get any of that stuff? Of course not.
      The Far-Right Party wants to oppress women, LGBT people, and racial/religious minorities while generally making everything worse at home and abroad.
      And our Center-Right Party, if you somehow are able to ask a person in leadership for these things because they pick the questions they want to talk about, and are really lucky, they won't laugh in your face or roll their eyes or something.
      The Democrats are Nominating the third person in a row who got their recommendation for the Presidency due to massive amounts of Fraud this week. They didn't even bother with voting this time. They have no political platform or policies. Not that they would ever enact them of course.
      They don't answer to us, they know it, we know it, and they know there is nothing we can do about it short of Violent Revolution. And look at what we had to do just to get one murderous police officer charge with murder and then sentenced for it in a state where the Democrats control every major office. That Governor is on the VP ticket by the way.
