Obstructive Problems After Hirschsprung Pull-Through

Babies born with Hirschsprung disease lack ganglion cells in certain parts of the large intestine, causing an inability to properly regulate bowel movements. Babies are treated with a pull-through surgery to remove the diseased section of the large intestine and pull the healthy section down to the rectum, but 30% of patients will still have flare-ups of colitis - even if performed correctly.
In this video, Luis De la Torre, MD, discusses the problematic obstructive symptoms experienced after a technically well-performed pull-through for Hirschsprung disease. Dr. De la Torre also explains the importance of preserving the anal canal, workup for recurrent enterocolitis, when redo pull-throughs are recommended and treatment for constipation after a pull-through procedure.

if you have an obstructive symptoms of up to 50% post pullthrough, then there is a problem in the technique.
in our country, we prefer to do transanal endorectal pullthrough. if properly done, the patient may even appear incontinent for up to close to a year. the only time that a patient will present obstruction is when the resection is very near the transition zone.
I am a pediatric surgeon from the Philippines. practicing for more than 20 years. My patients never had obstruction after pullthrough.

My son too never had obstruction after his pull through, it was done here in Lagos university teaching hospital (LUTH) Nigeria, last year 2022 October, he is 2years old

My son did pull through surgery at birth,he is incontinent and this has been continuing till now,what do you have to say?
