Achalasia is an acquired esophageal motility disorder that is caused by inability of the lower esophageal sphincter to relax. Multiple potential causes have been proposed, but in many cases, the cause is unknown. Individuals with this condition often have particular difficulties, especially feeling that food is being “caught” in their esophagus among other issues. In this lesson, we discuss the pathophysiology behind why achalasia occurs, including the signs and symptoms of achalasia, how it’s diagnosed and how it’s treated.

This was one of My greatest discoveries while studying GIT
i had this from childhood all the way to the age of 15. Haruhiro himself, the inventor of POEM surgery in japan operated on me. most of my life i looked like a dwarf. at the age of 15 i looked like i was still a kindergartener. but after the surgery i started eating food and had a huge growth spurt. my genetics say i should be tall, around 180cm but cuz of this condition, it has left lasting negative effects on me, one of which is me missing out on most of my puberty. atleast i am now considered slightly below average height. it also left my esophagus in a very damaged state. my esophagus had expanded to 6 times its size, cuz of all the food getting stuck and storing in there. i have a condition where 1 side of my ribcage is slightly portruding forward so my chest looks assymetrical. i suspect achalasia has something to do with it. not sure tho.

I had this. My food become ball each time i vomit. I cant even swallow water. I got the balloon treatment thing. But it only last for 3 day. Doctor suggest me to get a surgery because my body got so thin like drug addict. But i scared. So i train and push my self to swallow any food with so much water like flushing method. I train my mindset to not get panic if the food got stuck. Since that i start feeling food in my stomatch and start gaining weight. The downside is i have to bring my own water if eat outside/restaurant/etc. . Minimum 1 liter each meals.. And i keep getting heart burn. But its better than die starving. Now i got different problem, i eat less but my weight is keep gaining. 😂😂😂. Sorry for my bad English. I from Malaysia.
I've done this but eventually that stopped working for me, standing while eating helps but it's embarrassing.
Brother get the surgery myotomy…it will save your life. I had a balloon dilation I’ve been swallowing perfectly for 6 months post procedure

Great video! Why didn't you mention vagus nerve irritation and/or damage as a cause for achalasia? Also, I thought achalasia incluses muscle spasms and loss of motility in the intestines as well.

I had Achalasia, my doctor had to stretch my esophagus. I had a great doctor.
Dude.. u cant "have had it" and then its gone.. its a disese that is not going to go away.. it can be better but it wont go away...

@@mmadhu8622 After going through many tests I was finally sent to a gastroenterologist.
Yeah they usually try dilutions before going to surgery but it depends how you respond. I've had 4 dilations but never experienced any relief and the last one actually sent me into such a bad spasm I had to be tied to the hospital bed temporarily after clenching my chest so hard all my fingers pierced and started ripping my skin off but in my defense I was still waking up from anesthesia and was in that weird in between limbo of not being conscious yet but still physically reacting. My Dr's won't consider anything but surgery after that

This was helpful in understanding what I may have. A Barium swallow test revealed consistency with primary Achalasia complicated with an enlarged esphogus . My gastric wants to see how manomentry test does to confirm forsure if it's Achalasia.

A barium swallow test is not very pleasant for the patients but it gives a reliable diagnosis. Very useful video. Thanks!
barium swallow test is a cake walk compared to manometer
My Barium test only indicated "spams" at the esophagus. It took a motility Study in getting to the bottom of it.
Barium swallow was nothing compared to the motility study where they put the camera up your nose. I wouldn’t wish that on my worst enemy. But I will say, the barium swallow wasn’t fun with having Achalasia when you had a fear of swallowing already.
So how is it that I pass a barrium swallow test befor gallbladder surgrery but post surgery i failed it?

I have a question I have had a endoscopy procedure done and my GI specialist said that they don't see anything wrong but I know it's something wrong with my body I feel things getting into my lungs and esophagus and chest it's crazy I might try to tell them that I want a swallowing test or poem surgery
A barium swallow and a manometry is what you should ask for

Sir i think thier is no heart burn because of the LES is already closed so no acid comeback to esophagus.. chest pain because of the esophagus spasming... Correct if i am wrong

I think there is a little bit of wrong information
Achalasia patients have characteristic dysphagia for liquids only that may progress to solid. But initially they have selective dysphagia for liquids.
You are correct, i believe, it was mentioned specifically in our lecture aswell, however i cant seem to understand why, have you ever arrived at an explanation?
It is a progressive disorder. The damaged nerves causes a failure of the sphincter to open , and since the nerves are not going to regenerate again, the disease progresses with loss of muscle function of the sphincter area. So it starts with just mild dysphagia while swallowing liquids and as the muscles loses its function further , the disorder causes a full blown dysphagia even for solid food
My problems started with bread, then other solid food, then liquids.
It is rather odd that I see difficulty burping as one of the symptoms. I was diagnosed with Achalasia at 30 and have honestly never burped my entire life. I have more of a hiccup thing. Figured that was just something unique to me but maybe it’s Achalasia related.
I also got the POEM surgery done and so far it’s been successful.

Literally because of this I never understood why ppl would just burp outlook cuz I thought you could just choose not to burp... turns out it's a symptom of this
I think I may want that poem surgery cause I'm having truble swallowing foods and drinks and I feel like things is getting caught in my air ways
I found that a lot of my burping difficulty comes from my E being so drastically misshapen. I started doing yoga to stretch my muscles because I have 1-4 daily muscle cramps all over my body outside of my achalasia and increasing my flexibility allowed the gas to move up to be able to burp. I have a pretty severe problem with getting backed up so I rarely get the other gas which can be painful, it's amazing how you can be on a liquid only for weeks/months drinking multiple gallons a day and still be backed up. This disease is crazy

To save costs my gastrologist stalled. He wanted to just dilatate it which works poorly and isn't permanent. If you get a tear, you're really in trouble. I will never have a HMO again because they make medical decision based on money. The cause for me was idiopathic because the idiots don't know anything about autoimmune. Now I have RA, another autoimmune condition. I have other autoimmune issues but every condition is another doctor..... it's all idiopathic. This lesson is to blame the patient and consider risky treatments. Surgery is necessary. But even with that it's best to eat standing up and drink lots of water, lol.
