Hi! Can you clarify the question about 2 occurrences of hypertension. You initially state Acei then diuretic, but then you state diuretic first and then Acei. I thought you gave Acei first for hypertension, and lasix first for HF.
@ question 11 under CV. 67 yr old with Bp 168/92. On second visit 158/88, what is initial TX plan? Answers in order are ACEi, mild diuretic, Teach Lifestyle change( correct answer) and schedule stress test. Your explanation of when to use an ACEi vs Diuretic threw me off.
@ ok, I guess I made that comment more of an educational what do you do after the lifestyle change. I believe the standard of care is to start a diuretic and then an ace. In practice it’s a bit different. You have to take the whole person in consideration. Are they a diabetic? Then you start an ace/arb. Is their creatine high? Maybe start a calcium channel blocker. Etc. just learn the correct answer for the test and then once you get a job, you have to use your clinical judgement.
