      @ question 11 under CV. 67 yr old with Bp 168/92. On second visit 158/88, what is initial TX plan? Answers in order are ACEi, mild diuretic, Teach Lifestyle change( correct answer) and schedule stress test. Your explanation of when to use an ACEi vs Diuretic threw me off.

    • @JohnPamperin
      @JohnPamperin Місяць тому

      @ ok, I guess I made that comment more of an educational what do you do after the lifestyle change. I believe the standard of care is to start a diuretic and then an ace. In practice it’s a bit different. You have to take the whole person in consideration. Are they a diabetic? Then you start an ace/arb. Is their creatine high? Maybe start a calcium channel blocker. Etc. just learn the correct answer for the test and then once you get a job, you have to use your clinical judgement.

    65 mmhg strength of compression stockings is what makes that test question in the first one wrong. Minimal is less than 15 for asymptomatic individuals 15-20 for mild, 20-30 for moderate 30-40 for firm, and anything over 40 is extra firm. 30-40 is best for severe chronic venous disease while anything over 50-60 is reserved for chronic lymphedema. First! You have a great gifting to teach and your review videos (1-4 [5 Questions each]) are incredible. So thank you sir! Second, I was just wondering if you think these “Frances” review videos are applicable to the FNP ANCC Exam? Lastly, I obtained straight “A’s” throughout college (didactic work + all clinical rotations), but I have never been good at scoring high on exams (70-80%’s). This said, I did not do as I had hoped on the AANP Exam even though scoring an 80% on their Predictor Exam. QUESTION: I was encouraged to take the ANCC Exam, and because your 20 Question Review seemed to really resonate with me, I was wondering if you offer any type of tutoring? With this hopefully being my third career and in being a husband, father, and former Marine, I just cannot afford any more failure. I hope you can understand. Thank you again sir! Sincerely, E.j.
