Fellow commenters, I was once confused about the meaning of this song. I, too, thought it was just a silly song about ordering pizza. But after listening to it thousands of times, I believe I have found a deeper meaning.
"Heavy's Pizza Song" is about social anxiety and Heavy's inability to form meaningful relationships with his fellow humans. In the beginning of the song, he says "I'm going to order pizza." This is a metaphor for him seeking social interaction. He calls the pizza place and places his order, which is representative of him going out into the world hoping that he will be able to make friends with his fellow humans. But then, he starts singing "the song of his people" that he's known since he was a baby, and his body starts to deform and move in unnatural ways.
This strange dance is indicative of Heavy's anti-social tendencies. Despite his efforts to make friends and socialize, his anxiety takes over. After the dance, the pizza delivery boy arrives, but he doesn't have the right order. This represents his inability to form bonds with others. Instead of making tons of friends and being liked by people (the pizza) he gets rejected and judged for who he is (the sandwich and the steak). His anger that the delivery boy got his order wrong shows his frustration that he's lonely and sad.
In the end, he starts to do the dance again, this time with Scout and Medic. However, his "friends" are even more twisted and distorted than he is. They are people that Heavy has an unhealthy relationship with- desperate for attention and love, he ends up with the wrong type of people. This is even evidenced by the personalities of the Scout and Medic. Scout is an arrogant, self-centered idiot and Medic is an insane sociopath with no regard for others. He shows his approval of his new "friends" with a thumbs-up and a big smile at the very end, but for the last few seconds, you can see some doubt on his face, as if he knows all is not well. But alas, his crippling social anxiety keeps him from making any real friends, and he's left with the deformed Medic and Scout.
