I figured it out. You were killed while the elevator was rising. When you were knocked back and lying flat, you were half on and half off the elevator, so when you revived, you were being crushed between the elevator and the shaft wall.
I got killed jumping out too early and it must have clipped my head and killed me in the air, lvl 50 hunter with bounty b4 self Rez was a tough one to swallow but funny as heck after the rage :0
In case no one else has said it already, apparently explosive ammo doesn't have a headshot sound because it deals its damage in two separate events. It deals the first part of its damage(I think half the total damage, but not sure on that) directly, like a normal bullet, then the rest of the damage is dealt by the explosion. So for example if it says 100 damage in the stats then it might do 50 damage with the bullet and 50 damage with an explosion. Since the bullet itself isn't what's getting the headshot kill, it doesn't trigger the sound. At least, that's what I've heard. Makes sense to me, but could be wrong.

I've been enjoying the mosin obrez with fire and the caldwell with fletchette - seems you can one tap to the body with the mosin, and even at quite a long range, one shot from the caldwell finishes them.

That loadout from the intro is indeed sexy, and one way I myself upgrade it is by going a full sized sparks and a medium springfield with explosive shots
It‘s a self-revive bug. Happened to me many times before. It occurs when one tries to revive and you basically jump to the death screen again, without revive animation, killed by whatever. I have not figured out what triggers the non existent animation tho
The "Mad Sexy" loadout would work even better if you switch the weapons around, Sparks with poison and an uppercut with explosivs. This way you will never need 2 shots after the sparks because even an arm hit makes the uppercut deal enough dmg to down someone ^^

About the mad sexy kit, what about big sparks with poison since that should be your 1st shot and then explosive pistol or short springy when you have quatermaster
I had a death on revive happen exactly like that before except it was a poison bomb. I did not even see my character try to stand up. It just refreshed my death screen, said I died to poison bomb, and grayed out the revive button. 🤷
i've seen a video about the "explosive ammo hat shot" sound As far as i understand, there is a bug, which does so that the explosive ammo has a "soft headshot sound" which just means that you dont get the crunchy sound cue as any other ammo when performing a hatshot.
I picked up someone's loadout that was sparks rifle with explosive uppercut and it was more fun than I would have expected. Might have to play it again, but very similar concept to the martini-sparks pistol
Man there are countries that struggle with food/water OR have straight up war and people cry about a UI changes for a video game. Love the meme build videos btw

You get more range but you do get a smaller blast radius. Also this kit doesn't require quartermaster.

this has happened to me. if you get killed THE MOMENT YOU PRESS REVIVE it glitches and you cant revive
