    There’s lots of job gaps already and more people are retiring early and more are working part time. Job gaps are increasing. NHS is a holy cow too and there won’t be budget cuts unless something changes dramatically politically. IMGs are a valued part of our service and present and are in almost every program. Also, higher training programs are standardised so everyone has a fair chance.
    Primary care is very good in the UK too in particular - its paid well and is a stable life. (Many other countries don’t seem to value this part of medicine as here).

    As a reasonably long in the tooth nhs consultant (10 years)- I’d recommend a) intelligent A level students do something else entirely- maybe law, accountancy, IT b) newly qualified MBChBs (or equivalent)- retrain in something else, you are intelligent and capable of a lot c) FY1s and FY2s- probably too late to easily change by this point, but not entirely without hope and by the time you are in group d) ie. CT1 and above you’ll already understand the rest of my advice and be picking up your antidepressants from the pharmacy.
    The NHS is a hungry monster that lures in our brightest and most compassionate and morphs them into burnt out husks of human beings. It is an absolutely awful organisation that the devil and all his wicked minions couldn’t dream up in their wildest most evil dreams.

      @@JamesBond-io3ux I’ve been having a bad 20 odd years working in the nhs, 11 as a consultant and it only gets worse as time goes on. I agree that if someone enjoys medicine they should do it, but anywhere other than the nhs.

    Actually I’ve > 15 years of experience in the field of ‘Cardio Thoracic Surgery’ in Sri Lanka as an assistant cardiac surgeon.

      Go to the NHS job website. Search by region and choose London. See how many posts come up. How is there saturation?

      @@RoadToUK yes but when you filter down to what suits you and your qualifications, you end up with maybe 10% of all the "many" posts you are talking about.
      And by the way, London or not, most of the jobs require min 6 months previous work in the NHS so how on earth can someone apply for something that requires previous work in a country i haven't work in ? it is like chicken and egg

      usmle is much better than plab anyway. Also, you can start usmle in your med school unlike plab. Just go for usmle, you'd thank me later

      If the shortage is 50,000 and about 15,000 get Gmc registration every year doesn't that mean that in a few years the jobs will become less available? (I know that it is actually more complicated that that and there are other factors I should take into account) so I just wanted to know your opinion

      @@mohammedhany1032 That is just for that year. The gap will continue to increase each year as people retire, move away, choose not to continue, etc. And on top of that, nothing enough are locally graduating and post Brexit, less EEA doctors are coming.

    Most of my friends and myself included have been applying for jobs after GMC registration for almost a year with no offers. This is real Ibreez. I contacted an NHS consultant personally and she said that they only accept doctors with NHS employment experience. I have job experience for 2 years in the emergency department after internship, all the life support certificates, research, CPD and audit, with no results. I had my CV reviewed as well and continued to apply to jobs matching my specifications. What more can I do Ibreez cause I tried everything.

    Does making video explaining or teaching a certain disease like HTN or diabetes and so in a youtube channel considered a teaching experience? and if I have youtube channel should I mention it in the interview or it will be useless? Will I get points for that in the interview?
    And if someone who doesn't have any teaching experience could he do that instead like open youtube channel and begin talking about specific diseases and so?

    I feel like it's more about not getting a training job? I could be wrong but i have heard that radiology, gen surgery, plastics, psychiatry is near to impossible for imgs

      No, that's not true. This has been discussed already in several of our videos, but the training application process is equal for IMGs and their British counterparts.

    Hi Ibreez,
    I have a few questions that have been bothering me for a while now and i couldn't find info anywhere else.
    i started my mbbs in 2018 so after studying for two years in china i had to come back to Pakistan around January 2020 because of the pandemic therefore i've been studying online ever since. Now i'm in final year and we're uncertain that china education ministry will let us go back even for gradation. Is this going to be an issue?
    The thing is that unlike most countries medical students in china do not start having clinical rotations after 3rd year. In china you study theory for 4 years and then you spend your entire final year(5th year) in the hospital. 6 months in surgery and 6 months in medicine just like housejob or internship but this is included in our mbbs duration.
    So since they won't allow us to go back to china we have to do that internship which is a part of our mbbs in Pakistan. Is it necessary to do this internship or clinical rotations in affiliated hospital of the medical college we're students of? Or can we do this internship anywhere in the world and just show the documents later that we've had our clinical rotations in mbbs.
    I'm lost because this is a huge problem and i've no idea what to do.
    I'm sorry for the lengthy question but i had to explain my exact situation.

    US is so much better for doctors. Offcourse getting a residency match is very challenging, as there are juat 60K spots per year and people from all over world compete to get US medical residency. Need top USMLE scores, research experience, articles published etc etc to stand out in matching and interviews. The good part is high paying jobs are lined up perhaps from day 1 with sign up bonus / paid rent etc.

      USMLE is pass/fail. Day 1 residency pay is lower than what UK doctors get, and the hours are also substantially longer and more demanding.

      Now, the USA doesn't have basic pay or a general salary for doctors, so starting salary can depend on the hospital and cost of living for an area, but if we consider the IM residency in Houston, Texas, the PGY1 starting salary is $60,000. Residents usually work 80-100 hours a week, so if we take the lower end of 80, in a month they will work 320 hours. Taking that salary and dividing it out for pay per hour before tax, it would come to $15.62, or about $16/hour pre-tax.
      If we then look at the UK basic pay (which means no on-call shifts, weekends, nights, etc), the pay is £39,467 for a CT1. Hours for a basic work schedule are put down as about 173 hours a month. Calculated out, that comes to £19, or about $26.17/hour pre-tax.
      If you then say, well what if an IMG starts at an FY2 post- the basic pay then is £33,345. Calculated again against the same hours comes to £16/hour, or $22/hour pre-tax.
      Now if you then point out, well PGY1 should be compared to FY1 (even though IMGs don't have to start at FY1 in UK but must start at PGY1 in USA), the basic pay for an FY1 is £28,808, which then calculates down to about £14/hour, or about $20/hour pre-tax.
      Things to also consider: no one just works basic pay in the UK. We will want to be oncall and do weekends, nights, etc so that will then supplement our pay. But for the sake of keeping it simple, we are just comparing the salary to the basic pay.

      @@karunyapriyanka6947 you have to clear either license exam (PLAB) or FRCP/MRCP to start practicing. You can't directly apply for Job as MCI license is not applicable in UK. You need to be registered with their Medical council i.e,GMC

    I think the problem are not the job vacancies but the sub minimum pay they are offering for it. If a doctor who sacrificed minimum of 6 years of life earns less than, let’s say, a train conductor then he/she ain’t gonna go for that job.

      Where are you getting this data from? If you have the experience, and you know your worth, you can negotiate your pay in a non-training role. And besides, you can find out the pay from day one. If you don't think it's worth your while, don't go for the UK.

      @@helheimpanda I would highly suggest you to read articles by BMA on junior doctor pay restoration and on doctors vote.
      A junior doctor earns 30% less than what someone with the same post used to earn 10 years ago thanks to the inflation and no pay increase.
      Is the value of a current junior doctor less than the one 10 years ago? If no, then why is the pay less than them?

      'Low quality'? Please clarify. Aside from the change in allowing OET or IELTS, the medical exams remain the same for GMC registration.

      Are you kidding me? Even USMLE doesn't require IELTS. IELTS is just a money making machine for the UK, Australia and Canada. Think of it, the exams is in English,if you have low proficiency, you can't pass. If you think cos of OET, there are low quality doctors then think twice cos that IELTS is not even needed just like almighty USMLE doesn't require it.

      @@RoadToUK I mean the GMC exams are very straight forward. PLAB 1 is simple while PLAB 2 is just a simulation on how to be a cog in the system. So when they started accepting OET which is again a very easy exam you have this flood of doctors. This year many FY1 doctors were not given placements. Even IMT training which is basically service provision had a lot of competition

      @@adilislam4166 All the foundation doctors were allocated to a program, and you can see from the UKFP data how many were IMGs. Also, PLAB is not the only exam (though you may find it to be simple and easy!) as there are post-graduate exams that IMGs enter with as well. Unclear how IMT being service provision plays into your comment...

      @@adilislam4166 The exams is competitive enough for that level. Unless you are suggesting it is compressed like the USMLE for both general and specialization. Remember you still have to write exams for specialization in the UK.

    Jobs are there but these will be probably there for only 2 to 3 years more..UK is a small country ..can accommodate only as much unlike US !! Also doctor salary is low in comparison and training is very long
