2:26 is one of the best moments from this album, that synth tone is so grimy
What's cool is that it's also repeated later as lyrics at 14:19 ("I'm completely satisfied, my other life I will not miss," etc.), it's the Altered Beast speaking but the protagonist doesn't understand it until they become corrupted too.

@Joshua Heffernan I mean, the album is supposed to be a tale, not a real story. Completely irrelevant to the poster's point.
that being said, while the lyrics can match a variety of mythical, predatory creatures that prey upon and transform other humans such as vampires, werewolves, wendigos, zombies as mentioned previously, and so on. However, I think the altered beast is simply that, a monster that exists in the gizzardverse and described as such in the lyrics. Perhaps resembling the creature we see in the cover art.
