can you take the test without taking the course? I have been working in a doctors office for 20 years and have alot of on the job training. from bloodpressure to cleaning and stocking rooms to billing and mulitple other things.

That's totally fine; you don't need a nursing education to work as a Medical Assistant. The minimum requirement is completing your high school diploma or GED.
